IMPORTANT NOTES:
¾ Renewal application form will be accepted only with the certification of Authority competent
specified.
¾ Last date for submission of renewal form is 30-04-2022.
¾ Medical insurance coverage under the scheme will be provided for the PRMA members for those who
have renewed their membership on time.
¾ PRMA and PRENA amount will be paid to the bank account separately through E-payment mode.
¾ Changes in Residential address, Mobile No, Bank Account may be communicated to this Office.
¾ In case of death of a member, a communication may be given to this Office along with copy of death
certificate.
¾ M/s. National Insurance Co. Ltd (Neyveli) is our insurer and M/s.Vidal Health TPA is our TPA (from
01.09.2021).
¾ The filled Annexure-A shall be sent to this office by post (or) the same shall be scanned (Pdf or Image
format) & uploaded through online renewal system available at
https://web.nlcindia.in/retir_portal/main.php
(If a member uploaded renewal form through online system, is need not to be sent it again to
BCC)

ஏற்றுக்ெகாள்ளப்படும்.
¾ புதுப்பித்தல் விண்ணப்பம் சமர்ப்பிப்பதற்கு கைடசி ேததி 30-04-2022.
¾ புதுப்பித்தல் விண்ணப்பத்திைன குறித்த காலத்தில் சமர்ப்பிப்பவர்கள் மட்டுேம, இந்த திட்டத்தின்
மூலம் மருத்துவ காப்பீடு பயன்ெபற தகுதியுைடயவர் ஆவார்கள்.
¾ வருடாந்திர மருத்துவ உதவி ெதாைக மற்றும் அவசர கால உதவித் ெதாைக பட்டுவாடா, மின்
பட்டுவாடா மூலம் வங்கி கணக்கில் தனித்தனியாக ெசலுத்தப்படும்,
¾ உறுப்பினர்களின் விலாசம், ைகேபசி எண், வங்கி கணக்கு ேபான்ற விபரங்களில் மாற்றம்
இருப்பின், அதன் விபரத்ைத உடனடியாக அலுவலகத்திற்கு ெதrவிக்குமாறு ேகட்டுக்
ெகாள்ளப்படுகிறீர்கள்.
¾ உறுப்பினர் இறப்பு சம்பந்தப்பட்ட தகவைல இறப்புச் சான்றிதழுடன் நமது அலுவலகத்தில்
ெதrவிக்குமாறு ேகட்டுக் ெகாள்ளப்படுகிறீர்கள்.
¾ 01-09-2021 முதல் மருத்துவ காப்பீடு காலத்திற்கு ேநஷனல் இன்சூரன்ஸ் நிறுவனம், (ெநய்ேவலி)
நமது காப்பீட்டாளர் என்றும் (ம) விடால் ெஹல்த் டி.பி.ஏ மூன்றாம் தரப்பு நிர்வாகம் என்றும்
ெதrவித்துக் ெகாள்கிேறாம்.
¾ பூர்த்தி ெசய்யப்பட்ட Annexure-A ஐ தபால் மூலமாகேவா (அ) அதைன ஸ்ேகன் (Pdf or Image
