Lifetrack is a champion for families
facing the greatest disparities in income,
employment, education and health. Our
families experience the most significant
barriers and too often lack the critical
support network needed to succeed.
At Lifetrack, we build that network and
bring real tools so parents, kids and
individuals can create and experience
a new vision for what is possible, and a
renewed trust in their inherit strengths.
With you in their corner, we are investing
in positive futures for families, children
and our communities. Last year, 1,554
people became active participants in
our economy through career-focused
employment. Additionally, 24 refugee
families, who were earning less than
$1,000 a year before partnering with
Lifetrack, landed long-term employment
and are building equity through safe and
healthy homes they purchased in
southern Minnesota.
MESSAGE FROM THE PRESIDENT
AND BOARD CHAIR
Also in 2015, 1,400 new parents were
supported in exploring options and
making positive, proactive health,
education and communications decisions
for their children who are deaf or hard
of hearing because of Lifetrack’s Parent
to Parent Outreach Services. In Families
Together Services, 258 at-risk young
children learned healthy ways to manage
stress, express their emotions and prepare
for kindergarten, getting them off to the
best start possible.
Families who face significant challenges
also hold the greatest promise for our
community. Lifetrack’s deep legacy
of supporting families is only possible
because of our partnership with you and

A Young Mom Gets Help Landing Her First Job
Anna has a passion for helping young children
with disabilities. She knows what it’s like to
have a learning disability and have a hard time
in school. But this struggle is exactly what fuels
her desire to help others. It inspired her to enroll
in the Early Childhood Education program at
Minneapolis Community and Technical College.
“In high school, I had this crazy idea that I
wanted to open up a school for special needs
kids,” said Anna. “I think that they deserve to
have a lot more attention.”
Anna was working towards her degree when
her son was born. She enrolled in the Minnesota
Family Investment Program and got in touch
with Lifetrack. That’s when she met Mohamed, a Lifetrack Employment Counselor, who
worked to help her gain the confidence she needed for the job search. As a young mom,
Anna didn’t have any experience in the workforce. “It was hard to find a job because
I never had a job in my life,” said Anna. “I didn’t have the qualifications to do retail or
cashier work or anything.”
Mohamed helped push Anna out of her comfort zone and commit to the sometimes
grueling process of a job hunt. “Without Mohamed, I wouldn’t have gotten the courage
to look for a job. I would have been stuck at home, feeling helpless,” said Anna. When
she heard she was offered a job, she was excited to tell Mohamed.
With her first job under her belt, Anna now has more skills and experience to be
successful in the next chapter of her life. Today, Anna is focused on her volunteer
efforts, pursuing her dream in Early Childhood Education, and looking for more
part-time work to support her family while she completes school. She sees Mohamed
and Lifetrack as part of the support network that helped her to learn and do better.
“It’s just great that somewhere out there, there’s people that want to help you,” said
Anna. “It’s been awesome.”

A Pioneer for People with Mental Health Diagnoses
Lifetrack was recognized by the Minnesota Department
of Employment and Economic Development as a pioneer
for our evidence-based approach that fosters recovery
through employment.
Minneapolis Employment Partner of the Year
Lifetrack received the Partner of the Year Award by the
City of Minneapolis for our career pathways program,
helping people in poverty begin careers in construction
and commercial trucking.
Expanded Services in Rochester, MN
Lifetrack opened a new employment office in Rochester,
MN, where we offer services to help people re-enter the
workforce after being laid off.
Largest Refugee Employment Services
Lifetrack has been the largest refugee employment
services program in Minnesota for the last 8 years.

FINDING CONNECTION IN A
HEARING-CENTERED WORLD
A family finds community & support to
help their son thrive
Wayne and Allison are the type of parents who
gush unapologetically about their kids. Wayne
will even refer to his son as the “apple of my
eye” in public. This endearing quality gives you
a sense of just the kind of nourishing home
Wayne and Allison were building as a family,
when they found out they were expecting their
second child, Frederick.
What they didn’t expect was that Frederick
would have certain challenges that other kids do
not. Early hearing tests revealed that Frederick
had severe/profound hearing loss in both of his
ears. “It was something we were not prepared
for and we didn’t know what to do next,” said Wayne.
In their home in Illinois, the hospital connected them with a variety of resources that
helped them start to understand how to help their son. However, when Wayne accepted
a position at 3M in Minnesota, all of that changed. “We felt a little lost again once we got
out here,” said Wayne.
That’s when Wayne and Allison’s simple Google search brought up Minnesota Hands &
Voices. They met with a Parent Guide, Robin, who helped them understand the medical
and educational options available. But what made the greatest impact was attending
Minnesota Hands & Voices family events. “When Frederick was diagnosed, our hope
was that he would grow up to lead a normal life,” said Wayne. “And having family
events, where you can see kids a few years older than Frederick just being typical kids is
unbelievably comforting to me as a parent.”
That is the ultimate goal of Minnesota Hands & Voices – to offer parents tools to help
their kids succeed while building a rich community where families find understanding,
support and friends.

The Minnesota Coalition for Targeted Home Visiting,
a project of Lifetrack, partnered with the Minnesota
Department of Health to create a framework for baseline
training for all home visiting professionals to ensure best
practices across the state.
Over 200 attend the Work/Life and Flexibility Expo
LifeBalance Solutions, a program of Lifetrack, hosted
the 6th annual Work/Life and Flexibility Expo gathering
human resources and diversity professionals from across
the nation to discuss the complexities of balancing work
and family.

Cellastine’s kids receive emotional and
social support through
Lifetrack’s therapeutic preschool
Cellastine is a powerhouse of a parent. She’s
the mom of a band of eight spirited and
insightful daughters. When all the girls get in a
room together, laughter and chatter flow easily.
Even music seems to emerge from nowhere, as
the girls sing songs in unison. It’s an energy that
anyone from a big family might know.
But being a single mom to a big family, without
much support has been a challenge. Cellastine is
the first to say that after her daughter Ja’Niyha
was born, she was struggling. A public health
nurse saw this and suggested she look into the
programs at Lifetrack. Cellastine was a little doubtful at first, but ultimately, she decided
to give it a try. “I just don’t hesitate with anything that can make me better with my
kids,” said Cellastine.
Cellastine’s biggest worry was that her children needed to develop better social skills
and ways to process their feelings. “We’ve had issues with the kids trying to take on the
stress and worries of an adult,” she said. “I also didn’t have a vehicle four years ago, so
my children were never in a daycare setting to pick up social skills with anybody outside
of my home,” said Cellastine. The family signed up for Lifetrack’s Home Visiting program,
and subsequently, her three youngest daughters enrolled in the therapeutic preschool.
With therapy and help from their teachers, Cellastine’s daughters learned how to express
themselves better and work through stress. “The thing I appreciate the most was the
counseling because it helped me communicate with my kids better,” said Cellastine. “We
talk about feelings a lot now… How they are young don’t need to worry about anything
else going on. They’re just being kids now.”
Cellastine said the growth and accomplishments of her family make her the most
proud. “The kids have grown, grades are better, reports are better at school, there’s
more stability and consistency in the house now, versus before. Everything else has
fallen in place.”

TRAINING
Each year, Lifetrack recognizes local businesses
for their outstanding dedication to providing
job seekers with employment and the support
needed to succeed in the workforce. In 2015,
Heavy Metal Truck Training worked closely
with the Lifetrack team to help more families
realize stability through employment in the
commercial trucking industry. Working in
partnership with Heavy Metal Truck training
leaders Gary Pressley and Lisa Wiger, Lifetrack
clients like Marlon were able to chart a new,
more hopeful future.

A SPARK OF GENEROSITY
A SPARK OF GENEROSITY
A 7th Grader ignites a new family tradition to help others
Vinny, a 7th grader from Plymouth, MN spent his last Christmas thinking more of others
than himself. His stepmom, Tara, said that after searching the internet for charities to
give to this holiday season, he stumbled upon Lifetrack’s Adopt-A-Family Program.
“Underprivileged families mean a lot to him,” Tara told us when she explained why he
wanted to get involved with the program. The whole idea of giving people presents that
might not have the opportunity to get presents really appealed to him.”
Vinny not only bought gifts, with some help from Tara, but also wrapped the gifts
for two boys in the program. “I think it made him think about other kids and when he
received his own gifts, he was more appreciative than a typical 13 year-old boy would
typically be.” When Vinny and his family completed the project he said “I totally want
to do that again next year.”

  • 4. Building healthy, stable families. Lifetrack is a champion for families facing the greatest disparities in income, employment, education and health. Our families experience the most significant barriers and too often lack the critical support network needed to succeed. At Lifetrack, we build that network and bring real tools so parents, kids and individuals can create and experience a new vision for what is possible, and a renewed trust in their inherit strengths. With you in their corner, we are investing in positive futures for families, children and our communities. Last year, 1,554 people became active participants in our economy through career-focused employment. Additionally, 24 refugee families, who were earning less than $1,000 a year before partnering with Lifetrack, landed long-term employment and are building equity through safe and healthy homes they purchased in southern Minnesota. MESSAGE FROM THE PRESIDENT AND BOARD CHAIR Also in 2015, 1,400 new parents were supported in exploring options and making positive, proactive health, education and communications decisions for their children who are deaf or hard of hearing because of Lifetrack’s Parent to Parent Outreach Services. In Families Together Services, 258 at-risk young children learned healthy ways to manage stress, express their emotions and prepare for kindergarten, getting them off to the best start possible. Families who face significant challenges also hold the greatest promise for our community. Lifetrack’s deep legacy of supporting families is only possible because of our partnership with you and your family.   • 6. A Young Mom Gets Help Landing Her First Job Anna has a passion for helping young children with disabilities. She knows what it’s like to have a learning disability and have a hard time in school. But this struggle is exactly what fuels her desire to help others. It inspired her to enroll in the Early Childhood Education program at Minneapolis Community and Technical College. “In high school, I had this crazy idea that I wanted to open up a school for special needs kids,” said Anna. “I think that they deserve to have a lot more attention.” Anna was working towards her degree when her son was born. She enrolled in the Minnesota Family Investment Program and got in touch with Lifetrack. That’s when she met Mohamed, a Lifetrack Employment Counselor, who worked to help her gain the confidence she needed for the job search. As a young mom, Anna didn’t have any experience in the workforce. “It was hard to find a job because I never had a job in my life,” said Anna. “I didn’t have the qualifications to do retail or cashier work or anything.” Mohamed helped push Anna out of her comfort zone and commit to the sometimes grueling process of a job hunt. “Without Mohamed, I wouldn’t have gotten the courage to look for a job. I would have been stuck at home, feeling helpless,” said Anna. When she heard she was offered a job, she was excited to tell Mohamed. With her first job under her belt, Anna now has more skills and experience to be successful in the next chapter of her life. Today, Anna is focused on her volunteer efforts, pursuing her dream in Early Childhood Education, and looking for more part-time work to support her family while she completes school. She sees Mohamed and Lifetrack as part of the support network that helped her to learn and do better. “It’s just great that somewhere out there, there’s people that want to help you,” said Anna. “It’s been awesome.” Lifetrack partners with Hennepin County to deliver employment services, such as job search counseling for low-income parents.   • 7. STABILIZING FAMILIES When adults have sustainable employment, they create stability for themselves and provide a solid foundation for the next generation. Lifetrack helps people with disabilities, immigrants and refugees and people experiencing poverty begin and develop careers that are meaningful to them. CELEBRATING OUR SUCCESS A Pioneer for People with Mental Health Diagnoses Lifetrack was recognized by the Minnesota Department of Employment and Economic Development as a pioneer for our evidence-based approach that fosters recovery through employment. Minneapolis Employment Partner of the Year Lifetrack received the Partner of the Year Award by the City of Minneapolis for our career pathways program, helping people in poverty begin careers in construction and commercial trucking. Expanded Services in Rochester, MN Lifetrack opened a new employment office in Rochester, MN, where we offer services to help people re-enter the workforce after being laid off. Largest Refugee Employment Services Lifetrack has been the largest refugee employment services program in Minnesota for the last 8 years.   • 8. FINDING CONNECTION IN A HEARING-CENTERED WORLD A family finds community & support to help their son thrive Wayne and Allison are the type of parents who gush unapologetically about their kids. Wayne will even refer to his son as the “apple of my eye” in public. This endearing quality gives you a sense of just the kind of nourishing home Wayne and Allison were building as a family, when they found out they were expecting their second child, Frederick. What they didn’t expect was that Frederick would have certain challenges that other kids do not. Early hearing tests revealed that Frederick had severe/profound hearing loss in both of his ears. “It was something we were not prepared for and we didn’t know what to do next,” said Wayne. In their home in Illinois, the hospital connected them with a variety of resources that helped them start to understand how to help their son. However, when Wayne accepted a position at 3M in Minnesota, all of that changed. “We felt a little lost again once we got out here,” said Wayne. That’s when Wayne and Allison’s simple Google search brought up Minnesota Hands & Voices. They met with a Parent Guide, Robin, who helped them understand the medical and educational options available. But what made the greatest impact was attending Minnesota Hands & Voices family events. “When Frederick was diagnosed, our hope was that he would grow up to lead a normal life,” said Wayne. “And having family events, where you can see kids a few years older than Frederick just being typical kids is unbelievably comforting to me as a parent.” That is the ultimate goal of Minnesota Hands & Voices – to offer parents tools to help their kids succeed while building a rich community where families find understanding, support and friends.   • 9. EMPOWERING PARENTS A strong and nurturing parent fulfills the central role in a child’s growth and happiness. Lifetrack offers parenting education and connections to community resources for families experiencing poverty, families with children who are deaf or hard of hearing and adults balancing work and family demands. CELEBRATING OUR SUCCESS Push for best practices The Minnesota Coalition for Targeted Home Visiting, a project of Lifetrack, partnered with the Minnesota Department of Health to create a framework for baseline training for all home visiting professionals to ensure best practices across the state. Over 200 attend the Work/Life and Flexibility Expo LifeBalance Solutions, a program of Lifetrack, hosted the 6th annual Work/Life and Flexibility Expo gathering human resources and diversity professionals from across the nation to discuss the complexities of balancing work and family. .   • 12. Cellastine’s kids receive emotional and social support through Lifetrack’s therapeutic preschool Cellastine is a powerhouse of a parent. She’s the mom of a band of eight spirited and insightful daughters. When all the girls get in a room together, laughter and chatter flow easily. Even music seems to emerge from nowhere, as the girls sing songs in unison. It’s an energy that anyone from a big family might know. But being a single mom to a big family, without much support has been a challenge. Cellastine is the first to say that after her daughter Ja’Niyha was born, she was struggling. A public health nurse saw this and suggested she look into the programs at Lifetrack. Cellastine was a little doubtful at first, but ultimately, she decided to give it a try. “I just don’t hesitate with anything that can make me better with my kids,” said Cellastine. Cellastine’s biggest worry was that her children needed to develop better social skills and ways to process their feelings. “We’ve had issues with the kids trying to take on the stress and worries of an adult,” she said. “I also didn’t have a vehicle four years ago, so my children were never in a daycare setting to pick up social skills with anybody outside of my home,” said Cellastine. The family signed up for Lifetrack’s Home Visiting program, and subsequently, her three youngest daughters enrolled in the therapeutic preschool. With therapy and help from their teachers, Cellastine’s daughters learned how to express themselves better and work through stress. “The thing I appreciate the most was the counseling because it helped me communicate with my kids better,” said Cellastine. “We talk about feelings a lot now… How they are young don’t need to worry about anything else going on. They’re just being kids now.” Cellastine said the growth and accomplishments of her family make her the most proud. “The kids have grown, grades are better, reports are better at school, there’s more stability and consistency in the house now, versus before. Everything else has fallen in place.” The Families Together Program is an intensive, relationship-based early childhood intervention program for families who face critical challenges in life.   • 13. FOSTERING RESILIENCE IN KIDS Kids are resilient when they are able to move forward after facing minor setbacks, daily challenges or even a traumatic event. Lifetrack helps kids who experience the stresses of poverty find their strengths and prepare for kindergarten through our unique therapeutic preschool. We help kids who are deaf or hard of hearing build confidence with Role Models, American Sign Language instruction and community support. CELEBRATING OUR SUCCESSES Better Early Reading Support The Families Together Therapeutic Preschool hired two Reading Corps members, enabling all four classrooms to have additional early literacy support. Partnership with Wilder Research Working in partnership with Wilder Research, Lifetrack published a statewide needs assessment for families with children who are deaf or hard of hearing. HEAVY METAL TRUCK TRAINING Each year, Lifetrack recognizes local businesses for their outstanding dedication to providing job seekers with employment and the support needed to succeed in the workforce. In 2015, Heavy Metal Truck Training worked closely with the Lifetrack team to help more families realize stability through employment in the commercial trucking industry. Working in partnership with Heavy Metal Truck training leaders Gary Pressley and Lisa Wiger, Lifetrack clients like Marlon were able to chart a new, more hopeful future. VOLUNTEER SPOTLIGHT A SPARK OF GENEROSITY A SPARK OF GENEROSITY A 7th Grader ignites a new family tradition to help others Vinny, a 7th grader from Plymouth, MN spent his last Christmas thinking more of others than himself. His stepmom, Tara, said that after searching the internet for charities to give to this holiday season, he stumbled upon Lifetrack’s Adopt-A-Family Program. “Underprivileged families mean a lot to him,” Tara told us when she explained why he wanted to get involved with the program. The whole idea of giving people presents that might not have the opportunity to get presents really appealed to him.” Vinny not only bought gifts, with some help from Tara, but also wrapped the gifts for two boys in the program. “I think it made him think about other kids and when he received his own gifts, he was more appreciative than a typical 13 year-old boy would typically be.” When Vinny and his family completed the project he said “I totally want to do that again next year.” Interested in Volunteering?
