Failure of lifting tackle/sling can result in a dropped load, which may result in serious injuries or fatalities. The below mentioned precautionary measures must be followed.  All Lifting tackle shall be inspected by a third party (accredited by local authority) and certified “safe to use”.  A copy of third-party inspection and test certificates must be available on-site for all lifting accessories.  When the lifting tackle is not in use, the tackle shall be stored properly (i.e. hanging and away from exposure to harsh weather conditions).  The rigger and lifting supervisor shall inspect all the lifting tackle prior to daily use to ensure they are free from defects and safe to use.  If any lifting gear or accessories are out of certification or showing signs of damage, they must be withdrawn from service immediately.  The SWL of the lifting accessories must be clearly displayed and the same must not be exceeded during carrying out any lifting of loads.  Lifting equipment and lifting gear must be colour-coded on a monthly or quarterly basis to ensure that only certified tackle is in use. Coloured tags must only be attached to equipment or accessories that have valid inspection certificate and in good condition.  Slings shall not be shortened with knots or bolts or other makeshift devices.  Responsible supervisor and HSE must conduct regular monitoring of the above mandatory requirements.
