First of all, thank you very much for taking the time and effort to provide a supervised
internship experience for our ASU intern. Please click on the text box for each statement
and type in the number below that corresponds to how the student intern performed at

1. People skills

Comments:
Elizabeth worked extremely well with all of my staff and co-workers. We could have had her out
in the field working with the children and parents a little more than we did so I think from the
time she did spend in the field at our sites, she worked very well with those involved.
2. Communication skills

Comments:
Elizabeth and myself taught a group of middle schoolers at one particular event and neither of us
had too much knowledge on the subject we were asked to teach about. Elizabeth came up with
questions and comments to speak to the children right of the top of her head. She is a very quick
thinker and can provide expertise to clients right on the spot.
3. Knowledge of subject matter
a. 10 Knowledge of basic concepts in relevant health/fitness areas
Comments:
Elizabeth spent every day throwing health and fitness tips into our newsletters, announcements,
staff evaluations, presentations, and pretty much everything she participated in with our program.
She could always fit some kind of information about health and wellness into the project she was

2. working on. This helped us a lot when it came to being able to complete our health and wellness
initiative within our program for the semester.
4. Computer skills

Elizabeth always took the work that was given her for data entry and reports and completed it by
the time she was asked. Very prompt!
5. Program development

Comments:
Elizabeth helped head up the planning for our fundraiser for the semester and she did a great job
of incorporating her own ideas into our program. She has helped us realize easier ways to
provide health and wellness activities in our program without spending a lot of money and by
using our own expertise that she has taught us this semester.

Comments:
Elizabeth was completely professional in every meeting, training, and day to day work she
completed with us. She was always on time and very organized. I really love that she has a
separate notebook completely for our program and her internship. This helps interns keep their
paperwork and projects they are working on organized and keeps them on time with due dates.
She always had a high amount of energy and was willing to do whatever was asked of her.

10- I believe that Elizabeth deserves a satisfactory for her final grade. She has been such a
delight to have as a part of our program and she has brought a lot of helpful information to our
attention this semester. She was helpful to have as a part of our team and we were able to do
many more presentations and provide more services due to having Elizabeth on our support
team. We would love to have many more interns like Elizabeth from the program in the future
and we wish her luck in her future career!
