4. • Have you checked your site for hazards?
• Do you know where your First Aid kit is?
• Do your Employees know? Have you
informed them of their responsibilities?
• Do you know where the nearest Medical
clinic/ER is located to your job site?
• Do you know where your paper work is?
Plan Ahead
5. • STOP WORK – the employee involved
should stop work IMMEDIATELY and place
operations in a safe and stable
environment if they are able.
• CONTACT A SUPERVISOR – The employee
involved or their co-workers MUST
immediately notify the supervisor of the
incident.
• OBTAIN MEDICAL TREATMENT if
necessary.
• Submit to Post Accident Drug Screen.

6. Supervisor Responsibilites
• Supervisor Preserves the Scene – Ensure
that the incident scene is preserved until
all details of the accident can be obtained
and recorded by a Safety Representative.
(Not all accidents require preservation.)
• Supervisor and Employee determine if
First Aid/Medical Clinic or ER services are
required.
7. Supervisor Responsibilities
• Supervisor notifies Office Safety
Representative
• Supervisor or Designee accompanies
employee to medical clinic or emergency
room and provides a ride home if
necessary.
• Supervisor or Designee performs the
accident investigation using the Accident
Investigation Sheet and First Report of
Injury.

• Medical Treatment – Assess the
situation
– First Aid
– Medical Clinic
– Emergency Room
• Mitigation of Immediate Hazards
– Clean up any spills
– Lockout/Tag-out Equipment
– Secure the scene

occurred in chronological order.
– Details so reader can clearly picture the
incident

– Specific motions of injured employee
just before, during, and after incident
