Hospital-based emergency care
The large public hospitals in the UAE provide
services free of charge for all conditions except
minor emergencies. Private hospitals require
upfront payment.
Statistics on ED utilisation in the UAE are not
available but are being collected by government
entities. There are a reported 124 hospitals
in the seven emirates: 40 in Abu Dhabi, 40 in
Dubai, 12 in Sharjah, 12 in Ajman, 11 in Ras Al
Kaimah, six in Fujairah, and three in Umm Al
Quwain.
Trauma care
The World Health Organisation (WHO)
has collected statistical data stating that
emergencies and accidents caused 32 deaths
per 100,000 in the UAE, compared to United
Kingdom’s four deaths per 100,000, and that the
most common causes of trauma are road traffic
accidents (RTAs), falls, suicide attempts and
penetrative injuries. The World Report on Road
Traffic Injury Prevention projects that in 2020,
road trauma will be responsible for 546,000
deaths worldwide, making it one of the leading
causes of death.

Staffing
EDs in the UAE are staffed mostly with
expatriate physicians trained in medicine or
surgery. The larger EDs also have residents,
house officers/interns, and medical students.
It is estimated that there are 1,000 physicians
working in EDs in the UAE. Standardisation of
paramedic levels and competencies is lacking in
most of the smaller emirates.

Emergency medicine specialists
In 2000, hospital administrators in the UAE
began to recruit emergency medicine-trained
physicians to their facilities. Prior to that time,
emergency medical treatment was provided in
accident and emergency (A&E) departments
in public hospitals, staffed by general medical
officers trained in medical or surgical disciplines.
Emergency medical services
Pre-hospital care has been one of the most
challenging aspects of the emergency spectrum
in the Middle East. EMS in the region are largely
organised and run by the police departments.
Larger public and private medical facilities
also own and run ambulance services. Their
ambulances are used primarily for inter-hospital
transfers, while police-run EMS ambulance crews
provide acute pre-hospital care. The majority of
ambulances offer basic life support (BLS/Type 2)
with a growing minority offering advanced life
support (ALS/Type 3). At the present time, there
is minimal coordination between these groups
and many gaps exist in the services they provide.
EMS equipment
Recent studies revealed a lack of awareness in
the general population about how to activate
and utilise EMS resources. The use of EMS
by patients with acute coronary syndromes
was shown to be lowest in the region (17%),
compared with 23% in Bahrain, 30% in Qatar,
and 37% in Oman.
The quality of pre-hospital care varies
considerably amongst the emirates for
several reasons. The most significant are the
differences in EMS authorities, resulting in
a lack of coordination and synergy. Another
factor is the limited availability of EMS-trained
professionals in the country.
Disaster management
Over the past decade, disasters, both manmade
and natural, have made disaster medicine
and emergency management priorities for
hospitals and healthcare systems. With the
UAE being a major oil-rich financial and tourism
hub, preparedness for incidents such as oil
spills, plane crashes, and even mass casualty
events forms an important component of the
country’s comprehensive emergency medical
system. Authorities in the region are focusing
on all phases of the emergency management
continuum, after studying the large number of
emergency and disaster victim contingencies
and their overwhelming financial burden.
