National Healthcare Decisions Day is April 16.We recognize the importance of encouraging advance care planning
conversations for all adults – young and old, healthy and sick – and we want to offer you resources and tools to support
you and your patients.
By spreading the word about the importance of advance care
planning, you can help people:
› Voice and document their wishes for end-of-life care
› Live free from the three most common fears associated with the
end of life – pain, being alone, and being a burden to loved ones
› Avoid unintentionally placing an emotional burden on loved ones
who are forced to make end-of-life decisions
