Right throughoutof my career I was fortunateenough to work in reputed institutions with secondary and tertiary care and a pretty busy routine, intheoverseas
and in the inland as well, gaining a tremendous experiencein the surgicalfield.
For the last 18years I havebeenworking in Paediatric surgery in RoyalHospital,Muscat in theSultanate ofOman.
Jan 2004 -2010 as Registrar and then from 2010onwards as a Seniorregistrar / Senior Specialist in PaediatricSurgery, Royal Hospital
As a Senior Registrar/Registrar , thedaily routineofwork involved is as follows:
1.Morning endorsement with the entire Paed. Surgicalteam:
- Discussion ( investigations, reaching a diagnosis andmanagement)
ofthe previous day’s wardadmissions (emergencies via A&E/electives ) ,
- Discussion of thein ward criticalcases,
- Discussion of theprevious day’s referrals fromthegeneralpaediatricwards /
neonatal unit(SCBU) / maternitywards / PICU and from allover the
Sultanateof Oman
2.Daily ward rounds (Paed. Surg .ward/ Paed. ICU / SCBU (NICU)
and reviewing allthein-hospital referrals with a Senior consultant.
3.Managing outpatient’s clinic ( S. Consultant available for advice)
4.Managing dailyadmissions both electiveand emergencies ofinpatients
-Investigations, reaching a diagnosis and management
-Guiding /supervising theResidents intheaboveand at theA&Edept.
5.Planning andsupervising the Preop and Post op care ofwardpatients.

Our Paediatric Surgical unit is a wellestablishedin a 24bedded ward, with additional outlying patients intheSpecialCareBaby Unit (SCBU/NICU) andthevarious
paediatricmedical wards covering about 150 neonates and 2000surgical admissions a year.The rangeofpathology seenin our departmentincludes Neurosurgery
(Spina Bifida & Hydrocephalus), Urology, ThoracicSurgery (excluding cardiac), AbdominalSurgery, Endoscopy (Bronchoscopy,Oe sophagoscopy) and Trauma.
Out ofan averageof 1600 operations in a year, major operations contributeto a 75%and a 10 % falling in to the emergency category.
The SCBU/NICU is themainneonataltertiary carecenterfor SultanateofOman, with30 beds including 8 ventilator beds. Thereareabout 200 admissions a year and
the surgicalwork loadbeing 15% ofthetotal admissions along with 25 -30 cross consultations per month. Approximately 130“IndexCase”neonates are being
operated per yearincluding Oesophageal Artesia,Ano Rectalanomalies, Hirschsprungs disease, Omphalocoele and other abdominal wall defects,Congenital
DiaphragmaticHernia andGastrointestinalArtesia.
And the Royal Hospital being theapex tertiary care center , during my 18 years stayin the Department Paed. Surgery ,itis fairto saythat I have comeacross most –if
not almost all-of the Paed. Surgicalcases: congenitalor otherwise.
I have gainedvast experience ofperforming Paed. Surgicaloperations in thelast 18yrs and summarized as follows:

Impalpabletestes –Stephen-Fowler technique(2 stage Orchidopexy )- !0 yrs experience in Oman
A prospectivestudy of 104patients who underwent a stagedprocedure
- Stephen –Fowler technique –for impalpabletestis during theperiod between 1990 and 2000 in thePaediatricSurgicalDept of Royal Hospital .
At the first stageOrchidopexy, ligation ofthetesticular vessels werecarriedoutand aftera reasonablegap oftime( 6 months) for theflourishment ofthe
anastamoticchannels between theartery ofthe vas and thetesticular artery, thesecond stage was performedbringing down thetestis in to thedartos pouch,
The range of follow up was from firstvisit insixmonths to4-6 years and theassessment ofthe sizeofthetestis atthefollow upwas noted.The patients who were
lost for followup werecontactedto attend theclosesthealthcenter andthe localdoctor’s assessment was noteddown. The sizeof theother side normaltestis was
taken as thenorm.
The success rateof our operations was foundto bearound 62% comparedto the range of70s in theoutside world.

Longterm outcome ofHSCR – Oman Experience (2001- 2009)
The surgical management oftheHSCRhas progressedfrom a 2-3 stagedprocedure toa primary operationover thelast fewdecades. In the pastdecade, definitive
surgery for theHSCR has beenperformedusing minimallyinvasive techniques.
We have beenoperating onHSCR children sincelast 25years andhere weshareour recentexperience. From2001to 2009, weoperated85children with HSCR and
have been ableto collectfollow up clinicaldata regarding functionaloutcome for 79children.Followups were divided into thetypeofpullthrough, age at time of
procedureand length of thelast follow up.Follow upperiod ranged from1month to 8years.
We performed differentoperativeprocedures for HSCR; Duhamel in50,Soavein 23, Swenson in 2and Transanal procedurein10 children. Post-operative
complications includeenterocolitis
(10%) and bowelobstruction secondary to post opadhesions (10%). Therewas no mortality. Post-operatively constipationwas observedmoreinchildrenwho had
Duhamelprocedure,when comparedwith other groups. Functional outcomes were not significantly differentamong the various operativetechniques.
