3. Do You Frequently Bump Into This Hiccup?
Radiology practices and radiologists are the crucial
entity of the healthcare delivery system especially in the
diagnostic segment.
Mistakes in the dictation pattern not only culminate in
erroneous claims and denied reimbursements, but it also
questions the disease management in the patients.
So, take this chance to design a clear exit-route. Just
read this!
4. Funny Dictation Bites
Through Twitter and other social media, I have collected
some of the hilarious dictation errors in radiology,
shared by my healthcare colleagues:::;:
37 years old occasional (Caucasian) male had x-ray of
his hip bowel (bone)…
CT scan of the patient’s heart (head) revealed cerebrovascular clot …
Abnormal diaphragm was observed in the breast (chest)
x-ray of …
5.
Uterus is normal in the… The patient is a male… (What
a miracle?)
Ultra-sonography displayed pulmonary enema
(edema)…
Borderline respectable (resectable) cancer was found in
the liver…
6. Dictation Standards
Some radiologists have very tough accent and even the
experienced transcriptionists may fail to transcribe the
dictation flawlessly, leaving almost half of the patient
history blank or performing an erroneous transcription.
7. The American College of Radiology (ACR)
recommendations for a Good Radiology Dictation
Practice:
Efficient communication without any interferences:
No dictations while you are on the move.
No eating or drinking while you dictate.
No instrumental sounds (radiology devices, a/c
blow, vacuum cleaner, recording device positioning,
etc.).
Speak slowly and audibly.
8. Dictate with words that are on a standard radiology
glossary and radiology lexicon.
Avoid non-standard radiology acronyms and
abbreviations.
Comprehensive patient demographics (Name, DOB,
age, gender, location, referring physician, date/time of
examination)
Examination parts, procedure & limitations, observation,
interpretation/impression.
9. Data of further diagnosis (if any).
Relevant clinical information (eg: ICD-9 codes).
Any noteworthy patient reactions.
Standard and easy-to-understand report template.
