3. What is Patient case presentation: is a formal
communication between health care professionals
regarding a patient’s clinical information.
Includes: Demographic information, past medical history,
current medical history, cause of success and failure
treatment…..,
4. Patient cases are presented to colleagues on a daily basis,
for three primary reasons.
Practitioner needs another to assume responsibility for
a patient when leaving a shift, taking a vacation, or
sharing responsibilities.
A practitioner needs advice from a colleague
concerning the care of a patient.
Most frequently in the case of student practitioners, to
present patient cases to a mentor/practitioner when
learning to care for patients.
5. The case presentation in pharmaceutical care practice is
structured to present patient information for the purpose of
identifying, resolving, and preventing drug therapy
problems.
6. Allows to access patient information, Disease
information, Drug information
To determine patient drug related needs( which is
appropriate, effective, safe and convenient for the
patient
Establish goals therapy and treatment alternatives
make decisions and recommendations they are able to
defend.
7. Height and body weight : help to calculate total body
surface area(BSA) which help to obtain appropriate dose
for the patient.
Race: Help in dose adjustment
Age: Help to chose appropriate dosage form, also dose
adjustment, help to differentiate diseases.
Gender: Avoidance of irrational medicine use like
pregnancy, breast feeding patient, under contraceptive
pill.
8. Prevent drug-drug interaction and adverse drug
events(ADEs) to previously taken drug therapy.
Asses the effect of past medication and help to change
new medicine if didn’t work out.
