5. Pilot Project
• Elemental pilot study was carried out with our
female population
• The Mental Health team took the lead in identifying
suitable patients and making referrals through
Elemental
• The Senior Nursing Assistants undertook the
SWEMWBS scoring pre and post series of
interventions
6. Case Study
Age: 39 Crime: Murder
Chaotic lifestyle, abuse, prostitution, drug addiction
Presenting problems:
Low self esteem
Trouble sleeping
Anxiety
Physical Inactivity
Low mood
7. Case Study
- Initial SWEMWBS score was 7
- Attended sleep hygiene and relaxation classes
- Participated for the full twelve week course
- SWEMWBS score mid course increased to a
staggering 19.25 and remained at this at the
end of the course
- This demonstrated a significant improvement
in her health and wellbeing score

- Started going to yoga and the gym
- Commenced voice of release
9. What now?
• The project has been extended
• This is now offered to the young male population in our care
• The future for us is to use the Elemental platform to integrate care from all
involved agencies and continue social prescribing through the gate and beyond
