(gastrointestinal toxicity) กำรหลุดเข้ำสู่กระแสเลือดหรือไป
ที่อวัยวะอื่น (transmigration) เป็นต้น ส่วนผลข้ำงเคียงที่เคยมี
กำรรำยงำน ได้แก่ กำรติดเชื้อในกระแสเลือด เยื่อหุ้มหัวใจ
อักเสบ ฝีในช่องท้อง เป็นต้น ดังนั้นจึงต้องมีกำรใช้อย่ำง
ระมัดระวังในผู้ป่วยที่มีประวัติเป็นโรคหัวใจหรือควำมผิดปกติ
ที่ลิ้นหัวใจ ทำรกมีประวัติคลอดก่อนกำหนดหรือมีควำมพิกำร
แต่กำเนิด ผู้ป่วยที่มีควำมผิดปกติที่ลำไส้ เช่น ภำวะลำไส้สั้น
ผู้ที่มีภำวะภูมิคุ้มกันบกพร่อง ผู้ป่ วยที่มีกำรใส่สำยสวน
ผู้สูงอำยุ ผู้ป่วยเบำหวำน ภูมิแพ้ ผู้ที่แพ้ยีสต์ เป็นต้น
สำหรับอำกำรข้ำงเคียงที่ไม่รุนแรงที่อำจพบได้บ่อย
จำกกำรใช้จุลินทรีย์โพรไบโอติก ได้แก่ ท้องผูก ท้องอืด สะอึก
คลื่นไส้ กำรติดเชื้อ และผื่น

ข้อบ่งใช้ สายพันธุ์จุลินทรีย์และขนาดที่ใช้
1. Acute infectious diarrhea
in infants and children
LGG = 1010
CFU in 250 mL oral rehydration solution
L. reuteri = 1010–11
CFU daily x 5 days
S. boulardii = 250–750 mg x 5–6 days
2. Traveler’s diarrhea LGG = 2 x 109
CFU daily start 2 days before departure and continue till return
S. boulardii = 5 x 109– 10
CFU daily, start 5 days before departure
3. Antibiotic-associated
diarrhea (AAD)
LGG = 6 x 109
CFU daily x 1–4 weeks
S. boulardii = 4 x 109
CFU daily x 1–4 weeks
L. acidophilus and L. bulgaricus = 2 x 109
CFU daily x 7 days
Clostridium difficile 2 x 1010
CFU daily x 4 weeks with vancomycin and/or metronidazole
4. Irritable bowel syndrome
(IBS)
VSL#3 = 9 x 1011 CFU daily x 8 weeks
Bifidobacterium infantis = 106-10
CFU daily x 4 weeks
LGG = 8-9 x 109
CFU daily x 6 month
5. Ulcerative colitis (UC) Escherichia coli Nissle 1917 = 5 x 1010

สรุป
ในปัจจุบันควำมรู้ด้ำนโพรไบโอติก รวมทั้งหลักฐำน
ทำงวิชำกำรที่แสดงถึงประสิทธิภำพและควำมปลอดภัยของ
โพรไบโอติก มีจำนวนเพิ่มมำกขึ้น และผู้บริโภคมีควำมสนใจ
ในกำรดูแลสุขภำพมำกขึ้น จึงมีผลิตภัณฑ์โพรไบโอติก
วำงจำหน่ำยอย่ำงแพร่หลำย คุณประโยชน์ของจุลินทรีย์
โพรไบโอติกต่อร่ำงกำยมีมำกมำยหลำยด้ำน ครอบคลุมทั้งที่
ระบบทำงเดินอำหำรเอง และนอกระบบทำงเดินอำหำร หำก
พิจำรณำจำกคำจำกัดควำมของคำว่ำ “โพรไบโอติก” จะพบว่ำ
คำสำคัญ (keywords) มีอยู่ 3 ส่วน คือ 1. ต้องเป็นจุลินทรีย์ที่
มีชีวิต 2. ต้องได้รับในปริมำณที่เพียงพอ และ 3. ต้องก่อให้
เกิดประโยชน์ต่อสุขภำพ ดังนั้นในกำรพัฒนำผลิตภัณฑ์ กำรให้
คำแนะนำ และกำรเลือกซื้อผลิตภัณฑ์ เพื่อให้ได้ประสิทธิผล
สูงสุด จึงต้องคำนึงถึงสำมองค์ประกอบนี้เป็นสำคัญ
