Program focused on improving the health of seniors through awareness campaign and blood pressure readings, risk assessments, and catered recommendations to improve lifestyle factors
Housing First is an evidence-based approach that provides permanent housing at the start of the rehabilitation process for these individuals
Through At Home / Chez Soi, Housing First has been tested in 5 cities across Canada with very positive results in housing stability, reduction in public sector usage and improvement in quality of life and community functioning.
The Obama administration in the Unites States has set ambitious targets for system reform. In at an attempt to reward the quality of care delivered rather than the quantity of care delivered, they have set ambitious targets for Medicare payment reform. By 2018, the goal is for 50% of payments to be tied to quality or value. This reorientation towards value rather than volume represents a bold step to alter the healthcare spending trajectory in order to make health care delivery more affordable in the years ahead.
