Uplift Our Local CommunityNominees in this category are agents of local change. Their
work is centered around improving their local community. Nominations for this award
should include a summary of the nominee’s service, the rationale for the service the person
provides and data that demonstrates the impact of his/her service has had on the
community. This information should be included in the “supporting statements” portion of
the nomination form.Submission Must IncludeAn introductory statement, supporting
statements and a closing statement. The goal of the narrative is to persuasively explain the
difference the nominee has made in his/her community.Introductory Statement:In 4-5
sentences, clearly state why the nominee deserves the honor. Include enough information
for the selection committee to become familiar with the nominee and intrigued to learn
more about him/her.Supporting Statements:In 3-4 paragraphs, elaborate on the nominee’s
contributions and accomplishments. Include specific outcomes, results and/or activities
that make him/her worthy of the award.Closing Statement:In 4-5 sentences, describe how
others regard the nominee and his/her work (e.g., colleagues, those impacted by his/her
service, other community leaders, etc.).
