We will try to separate fact from fiction surrounding five of the most common myths
Myth 1 - If men ejaculate they are fertile
Truth - Men may have a very low sperm count or poor quality sperms and still ejaculate. In some cases there
may not be any sperms in the ejaculate. A simple Semen Analysis is required to rule out a male subfertility.
Myth 2 - Fertility issues are more likely to affect women
Truth - Difficulty in conception can be attributed to men as much as to
women. Male factors are responsible for nearly 30 % of all infertility
cases and in another 20 % there is a combination of both male and
female.
Myth 3 - There is time to keep trying until the age
of 40
Truth - Age of the female partner affects the success rate of all fertility
treatment. After 37 years, the fertility declines steeply. It has also been
seen in some studies that the pregnancy rates are lower if the male
partner is over 40 years of age. Do not delay your treatment decisions
and give yourself the best chance.
Myth 4 - Fertility treatments go against Nature
Truth - Being able to have a baby is the most natural thing for humans. Fertility treatment only removes the
barriers causing conception difficulties and nature does the rest.

Men start producing sperm as soon as they reach puberty
and this continues lifelong, but their sperm quality and
quantity may dip occasionally. Though a man is capable of
reproducing all through his life there is a decline in both
quality and quantity of sperms with age.
The female, on the other hand, is born with a finite number
of eggs in their ovaries. After the females reach the age of puberty, every month some of the eggs in the ovaries
mature but only one egg reach complete fertility potential, the rest degenerate and die. When the eggs are less
than a certain number in the ovaries, the woman enters the menopausal state. So female age is the single most
important variable when it comes to fertility and timely intervention is very important in fertility treatment.
A woman can conceive during the mid cycle, 2 days before and after ovulation –that is the time of release of the
egg from the ovary. There are only a handful of days that a woman is most likely to get pregnant, intercourse in
this period can result to a pregnancy.
Natural Conception
Before we learn about conception it is important to understand what happens during the menstrual cycle. In the
first half of the menstrual cycle, follicles start growing under the influence of hormones from the ovaries which
are controlled by hormones from the brain.These hormones also result in the growth of the uterine lining. During
midcycle the pituitary gland (Brain) releases LH (luteinizing hormone) which triggers release of the mature egg
(ovulation) within 48 hours. The follicle ruptures and egg is released which then starts the long journey of
travelling through the Fallopian tubes toward the uterus.
After ovulation, the follicle starts to produce progesterone. The endometrial lining to the uterus now is ready for
nourishing of the fertilized egg.
The sperms, after getting ejaculated into the vagina, will travel toward the
opening of the cervix and will enter the fallopian tube to fertilize the mature
egg. The sperms can live in the female genital tract for upto 72 hours so the
timing of the intercourse is very important.
After one sperm penetrates the outer covering and its genetic matter reaches
the egg, the egg is fertilized. This fertilized oocyte then travels to the uterus
and implants itself in the lining. This will continuously divide itself and will
become a baby.
If pregnancy does not occur, the corpus luteum creases to produce progesterone and the soft lining of the
womb, the endometrium, is shed leading to start of menstruation. The levels of estrogen decrease, which again
triggers the increase in brain hormones and the menstrual cycle starts over again.

What is Infertility?
According to WHO-Infertility is defined as the inability of a couple to conceive after 12 months of unprotected
sexual intercourse.
Causes of infertility
Lifestyle Causes
Unhealthy lifestyle contributes to infertility and quite often these
causes are overlooked. We are mentioning these causes at the
start as simple lifestyle modifications often lead to improved
fertility.
• Unhealthy diet high in unsaturated fat and trans fat
Intake of high levels of unsaturated fat and trans fat not only cause high levels of cholesterol in the
bloodstream leading to heart disease and strokes but also can cause hormone imbalance resulting in
disruption of the menstrual cycle in women and increases the risk of erectile dysfunction in men.
• Caffeine
Excessive intake of caffeine increases the risk of hormonal imbalance and miscarriage.
• Alcohol
Excessive drinking in women may increase the production of hormone prolactin which interferes with the
ovulation leading to infertility. In men, alcohol interferes with production of testosterone, the male hormone.
• Exercise
Moderate exercise improves health and consequently fertility. But excessive exercise sometimes results in
stoppage of ovulation resulting decreasing fertility in women.
• Hot bath and Sun bath
For sperm production temperature slightly below the core body temperature is required. Hot tubs, saunas,
steam room or working in the high heat environment may decrease quality and quantity of sperm.
• Tight underwear
Tight clothing raises scrotal temperature, thereby reducing the sperm production.
• Weight
Low body weight and excessive weight are not very conducive to fertility.
• Drugs and smoking
Both men and women have been linked to infertility and miscarriages along with congenital malformations.
Environmental Factors
Pesticides are environment toxins which cause damage or interfere with reproductive processes and hormone
imbalance. Chemicals such as DDT, PCP, furans and man made estrogen reduce the sperm quality and lower

consequently infertility. PCOS is a common disorder associated with high level of male hormones.
• Tubal blockage- If the tubes are blocked then fertilization cannot occur and results in infertility. Common
causes are infection or scarring after a previous tubal pregnancy or surgery.
• Abnormal uterine shape size- Any abnormality in the uterine shape, size can result in pregnancy loses.
Myxomas and fibroids are quite common.
• Endometrial causes - The endometrial layer has to be optimum for implantation of the zygote so any
problems in the endometrium lead to infertility. Aberrant endometrial tissue in the tubes, ovaries
(Endometriosis) creates an adverse environment.
• Cervix / Vagina - There could also be problems in the cervix and sometimes vagina like stenosis strictures
etc. which would result in the sperms not being able to reach the uterus.
Investigations in Infertility
The first investigation that should be done for a couple who are not able to conceive is a semen analysis.
It is a simple noninvasive test and rules out any male factor.
For the females initially a hormonal profile, tubal patency tests and ultrasound are advised.
The specialist may also request some other specialized tests as per the medical condition of the couple.
Minimally invasive procedures like laparoscopy and hysteroscopy are done to further assess the tubes, ovary
and uterus and also to correct any abnormality.

• Surgical Treatment
Sometimes surgical interventions are required to reverse the factors that cause infertility. Most of these
surgeries can be done by minimally invasive routes.
• Planned Relations
Couples are advised to have relations on alternate days during the middle of the menstrual cycle. This is
sometimes combined with follicular monitoring to identify the most fertile period to improve the chances of
conception.
• Ovulation Induction
Oral or injectable medications are given for stimulation of the development of ovarian follicles, and then for
triggering oocyte release from mature ovarian follicles. This is often done in conjunction with follicular
monitoring and planned relations.
The common treatments like follicular monitoring with planned relations, ovulation induction are often
advised empirically to young couples with no apparent cause of infertility. These can be carried out at a
gynecologist’s clinic.
• IUI (Intrauterine Insemination)
In IUI the husband provides us with a semen sample which is washed and then in a simple office procedure
introduced into the wife’s uterus. Superovulation IUI referes to the treatment process when in the IUI cycle
some drugs are given to the wife either oral/ injectable for minimal stimulation of her ovaries. The success of
IUI is around 10% to 14% and depends on various factors like sperm count and morphology, ovulation and
timing of IUI. Most pregnancies generally happen in the first 3 to 6 IUIs. Hence, further trial with IUI after 6
failed IUIs is not recommended. However, this decision is left to the discretion of your treating doctor.
If these interventions do not lead to pregnancy, specialized treatment options are available like test tube baby
-In Vitro Fertilization (IVF), Intra Cytoplasmic Sperm Injection (ICSI). These need state of the art treatment
facilities and trained specialists. Before these are planned, thorough investigations of both partners are
carried out.
• IVF (In Vitro Fertilization)
In the IVF cycle hormonal injections are administered daily to the wife for stimulation of her ovaries. Then
these eggs are taken out of the body under suitable anesthesia in a small procedure in the operation room.
These eggs are layered with the husbands’ sperms and eggs are fertilized naturally. 2-3 of the resultant
embryos are transferred back to the womb of the wife.

the wife for stimulation of her ovaries and the eggs are taken out of the body under suitable anesthesia in a
small procedure in the operation room. The difference is that these eggs are injected with the husband’s
sperms in the laboratory to fertilize them. This requires specialized equipments and technical expertise. 2-3
of the resultant embryos are transferred to the womb.
Recent Advances
In Males
In cases of male infertility the sperms can be retrieved using small surgical methods PESE (percutaneus), TESE
and then these sperms can be used for ICSI.
• IMSI (Intracytoplasmic Morphologically-Selected Sperm Injection)
In this advancement over ICSI, the sperm is more closely examined under magnification levels of more than
7000 times. Looking at sperm at this magnification shows defects in sperm head that cannot be seen in
conventional microscopes used for ICSI. This has been an exciting new development and numerous births
have been reported even in extreme cases of male infertility.
In Females
Laser Hatching is an advanced technique to thin the thickened outer covering which increases the success rates
of ICSI.
Zona Birefringence is a specialized technique advanced techniques which can be used to chose the best eggs
for ICSI.
Others
Blastocyst culture - The Embryos are sometimes incubated for five days so that it reaches the blastocyst stage
which improves the chances of implantation.
• Donor Programmes
ICSI treatments using donor gametes give very good results in couples with dwindling number and quality of
eggs and sperms and in premature ovarian failure.
• Surrogacy
In surrogacy, the wife’s eggs and the husband’s sperms are fertilized to make embryos that will later be

We will try to separate fact from fiction surrounding five of the most common myths Myth 1 - If men ejaculate they are fertile Truth - Men may have a very low sperm count or poor quality sperms and still ejaculate. The endometrial lining to the uterus now is ready for nourishing of the fertilized egg. The sperms, after getting ejaculated into the vagina, will travel toward the opening of the cervix and will enter the fallopian tube to fertilize the mature egg. The sperms can live in the female genital tract for upto 72 hours so the timing of the intercourse is very important. After one sperm penetrates the outer covering and its genetic matter reaches the egg, the egg is fertilized. This fertilized oocyte then travels to the uterus and implants itself in the lining. This will continuously divide itself and will become a baby. If pregnancy does not occur, the corpus luteum creases to produce progesterone and the soft lining of the womb, the endometrium, is shed leading to start of menstruation. Causes of infertility Lifestyle Causes Unhealthy lifestyle contributes to infertility and quite often these causes are overlooked. We are mentioning these causes at the start as simple lifestyle modifications often lead to improved fertility. • Unhealthy diet high in unsaturated fat and trans fat Intake of high levels of unsaturated fat and trans fat not only cause high levels of cholesterol in the bloodstream leading to heart disease and strokes but also can cause hormone imbalance resulting in disruption of the menstrual cycle in women and increases the risk of erectile dysfunction in men. • Caffeine Excessive intake of caffeine increases the risk of hormonal imbalance and miscarriage. • Alcohol Excessive drinking in women may increase the production of hormone prolactin which interferes with the ovulation leading to infertility. In men, alcohol interferes with production of testosterone, the male hormone. • Exercise Moderate exercise improves health and consequently fertility. The success of IUI is around 10% to 14% and depends on various factors like sperm count and morphology, ovulation and timing of IUI. Most pregnancies generally happen in the first 3 to 6 IUIs. Hence, further trial with IUI after 6 failed IUIs is not recommended. However, this decision is left to the discretion of your treating doctor. If these interventions do not lead to pregnancy, specialized treatment options are available like test tube baby -In Vitro Fertilization (IVF), Intra Cytoplasmic Sperm Injection (ICSI). These need state of the art treatment facilities and trained specialists. Before these are planned, thorough investigations of both partners are carried out. • IVF (In Vitro Fertilization) In the IVF cycle hormonal injections are administered daily to the wife for stimulation of her ovaries. Then these eggs are taken out of the body under suitable anesthesia in a small procedure in the operation room. As in the IVF cycle, hormonal injections are administered daily to the wife for stimulation of her ovaries and the eggs are taken out of the body under suitable anesthesia in a small procedure in the operation room. The difference is that these eggs are injected with the husband’s sperms in the laboratory to fertilize them. This requires specialized equipments and technical expertise. 2-3 of the resultant embryos are transferred to the womb. Recent Advances In Males In cases of male infertility the sperms can be retrieved using small surgical methods PESE (percutaneus), TESE and then these sperms can be used for ICSI. • IMSI (Intracytoplasmic Morphologically-Selected Sperm Injection) In this advancement over ICSI, the sperm is more closely examined under magnification levels of more than 7000 times. Looking at sperm at this magnification shows defects in sperm head that cannot be seen in conventional microscopes used for ICSI. This has been an exciting new development and numerous births have been reported even in extreme cases of male infertility. In Females Laser Hatching is an advanced technique to thin the thickened outer covering which increases the success rates of ICSI. Zona Birefringence is a specialized technique advanced techniques which can be used to chose the best eggs for ICSI. Others Blastocyst culture - The Embryos are sometimes incubated for five days so that it reaches the blastocyst stage which improves the chances of implantation. • Donor Programmes ICSI treatments using donor gametes give very good results in couples with dwindling number and quality of eggs and sperms and in premature ovarian failure. • Surrogacy In surrogacy, the wife’s eggs and the husband’s sperms are fertilized to make embryos that will later be transferred in a surrogate’s womb. Surrogacy, is indicated, where carrying a pregnancy is not advised for medical reasons for a woman. As in the IVF cycle, hormonal injections are administered daily to the wife for stimulation of her ovaries and the eggs are taken out of the body under suitable anesthesia in a small procedure in the operation room. The difference is that these eggs are injected with the husband’s sperms in the laboratory to fertilize them. This requires specialized equipments and technical expertise. 2-3 of the resultant embryos are transferred to the womb. Recent Advances In Males In cases of male infertility the sperms can be retrieved using small surgical methods PESE (percutaneus), TESE and then these sperms can be used for ICSI. • IMSI (Intracytoplasmic Morphologically-Selected Sperm Injection) In this advancement over ICSI, the sperm is more closely examined under magnification levels of more than 7000 times. Looking at sperm at this magnification shows defects in sperm head that cannot be seen in conventional microscopes used for ICSI. This has been an exciting new development and numerous births have been reported even in extreme cases of male infertility. In Females Laser Hatching is an advanced technique to thin the thickened outer covering which increases the success rates of ICSI. Zona Birefringence is a specialized technique advanced techniques which can be used to chose the best eggs for ICSI. Others Blastocyst culture - The Embryos are sometimes incubated for five days so that it reaches the blastocyst stage which improves the chances of implantation. • Donor Programmes ICSI treatments using donor gametes give very good results in couples with dwindling number and quality of eggs and sperms and in premature ovarian failure. • Surrogacy In surrogacy, the wife’s eggs and the husband’s sperms are fertilized to make embryos that will later be transferred in a surrogate’s womb.
