General Information
Scope: The eForm is used to request authorization for employees of the Bank of
Tokyo-MitsXXXXXXXXXishi UFJ who require access to Bank applications
How to sign-on 1. URL: https://myrequest.btmna.com:8543
2. a. User Name: AD Login
b. Password:
How to Request Access 1. Using the ID or Name field, search for the employee’s name.
a. ID: Enter the employee’s ID and press the magnifying
glass icon; or
b. Name: Enter the employee’s First and Last name and
press the magnifying glass icon
2. The Search Employees window will appear, displaying the
employee’s ID and name. Press the User ID.
3. The employee’s data, i.e., ID, assigned Branch, Department, etc.
will appear on screen along with a second page labeled with the
section header, Select Applications
4. Under Select Applications, a maximum of 5 applications can be
selected. When finished, press Done
5. The name of the application(s) selected will appear in a tab at
the top of the page

6. Open the tab for the application which was selected and
complete the following fields:
a. Application: Select an option under Environment
b. User: Select Add, Modify, or Delete
c. Function Entitlement:
d. Comments: Only if necessary
e. Select Save
 A message will confirm that the application has been
saved
 Repeat steps a thru d for each, individual application
selected
Review Open the Review tab
1. The page provides an overview of information about employees
inquired about and a record referencing all data entered by the
User.
2. When finished reviewing the information, press the Submit
Request button.
3. The message, “The request was Submitted successfully” should
appear.
4. Press OK
5. The request is automatically forwarded to the Security
Department where it will remain in queue pending investigation.

2. On the left side of the page, scroll down and open Search
Requests
3. Locate the User or Requestor
4. Press Req ID.
5. The program will display the Status for Request page

 The bottom of the page references the name of the person(s)
who will review the request.

General Information
Scope: The User Access Request (“UAR”) is utilized by Users who have standard
access rights, i.e. Network, Internet, and Exchange, but require additional
access to systems and applications.
Access Information
Accessing and opening a
New UAR
1. UAR: http://uar/sites/uar/Admin_Pages/UAR_admin.aspx
2. On the left hand-side of the User Access Request screen, under
the heading, UAR Actions, select Create New UAR
3. Under Systems not handled by SSA, press the Create UAR
button located at the bottom of the window
How to Request Access What kind of request is this?
1. Request Type: Open the drop-down menu and select
Additional Access to existing ID
2. Type in a business reason why access is required. Use the
Additional Comments field to enter additional information
Who is this request for?
1. User (Prefill): Enter the name or User ID of the employee for
whom access is being requested.
 Names can either be typed into the name field or identified
using the Directory icon located to the immediate right of the
name field

Review 1. Press Continue
 All information entered in the request is displayed for review
prior to finalization.
2. If all information is correct, press Submit to finalize
 An email is automatically sent to the requestor’s Manager for
approval of the request.

Leones 73 Application References Guide Appendix D Requesting Access to Shared Drives and Folders General Information Scope: The User Access Request (“UAR”) is utilized by Users who have standard access rights, i.e. Network, Internet, and Exchange, but require additional access to systems and applications. Access Information Accessing and opening a New UAR 1. UAR: http://uar/sites/uar/Admin_Pages/UAR_admin.aspx 2. On the left hand-side of the User Access Request screen, under the heading, UAR Actions, select Create New UAR 3. Under Systems not handled by SSA, press the Create UAR button located at the bottom of the window How to Request Access What kind of request is this? 1. Request Type: Open the drop-down menu and select Additional Access to existing ID 2. Type in a business reason why access is required. Use the Additional Comments field to enter additional information Who is this request for? 1. User (Prefill): Enter the name or User ID of the employee for whom access is being requested.  Names can either be typed into the name field or identified using the Directory icon located to the immediate right of the name field

The User Submitting the request for access must provide the following information about themselves: 1. User 2. Name 3. Phone# 4. Mail Code 5. Manager Selecting Applications From the Application Menu 1. Check XXXXXXXXXOC-AD Folder Server Path (e.g. lahq-groups) 2. Press Continue 3. Select Server Access 4. Under Service Access Require, select Folder Access 5. Under Provide Folder Name, enter the name of the folder 6. Select Save Review 1. Press Continue  All information entered in the request is displayed for review prior to finalization. 2. If all information is correct, press Submit to finalize  An email is automatically sent to the requestor’s Manager for approval of the request.
