Khôùp caén lyù töôûng coù taàm quan troïng haøng ñaàu ñoái vôùi thaønh coâng laâu daøi cuûa phuïc hình
ñöôïc naâng ñôõ treân implant, tuy nhieân vaãn toàn taïi söï tranh caõi veà sô ñoà khôùp caén phuø hôïp

TYÛ LEÄ THAØNH COÂNG CUÛA HAØM PHUÛ
Ngöôøi ta ñaõ ghi nhaän raèng haøm phuû ñöôïc naâng ñôõ treân implant coù tyû leä thaát baïi cao hôn
so vôùi phuïc hình coá ñònh treân implant. Goodacre vaø coäng söï ñaõ chöùng minh haøm phuû ñöôïc
naâng ñôõ treân implant coù tyû leä bieán chöùng cao nhaát so vôùi baát kyø loaïi phuïc hình treân
implant naøo. Haøm phuû ñöôïc cho thaáy laø coù caùc vaán ñeà veà löu giöõ (30%), chæ ñònh ñeäm
haøm (19%), gaõy khoùa caøi (17%), vaø gaõy phuïc hình (12%). Haøm phuû treân implant haøm
treân cho thaáy tyû leä thaát baïi laø 19%, trong khi ôû haøm döôùi thaát baïi chæ 4%.1
Haøm phuû haøm

Jacobs keát luaän raèng haøm phuû treân implant raát deã gaëp caùc vaán ñeà veà khôùp caén, do beänh
nhaân coù söï nhaïy caûm keùm nhaát trong vieäc phaùt hieän caùc löïc gaây haïi do tieáp xuùc
sôùm.2
Ngöôøi vôùi boä raêng thaät coù khaû naêng phaùt hieän caùc tieáp xuùc sôùm ôû 20 microns, trong
khi tieáp xuùc sôùm ôû haøm phuû ñöôïc phaùt hieän ôû 108 microns. Do ñoù, beänh nhaân ít coù khaû
naêng nhaän bieát caùc bieán chöùng lieân quan ñeán khôùp caén do giaûm caûm nhaän ñoái vôùi caùc löïc
gaây haïi. Ñieàu naøy raát coù yù nghóa, vì caùc nghieân cöùu ñaõ chæ ra raèng tieáp xuùc sôùm coù theå
gaây ra caùc vaán ñeà lieân quan ñeán loaïn naêng caân cô vaø löïc nhai taêng leân ñaùng keå khi caùc
tieáp xuùc sôùm hoaëc tieáp xuùc cao hieän dieän.3
Do ñoù, sô ñoà khôùp caén cho haøm phuû treân implant laø moät trong nhöõng yeáu toá quan troïng
nhaát coù theå daãn ñeán söï thaát baïi cuûa implant nha khoa. Moät thieát keá khôùp caén khoâng phuø
hôïp coù theå daãn ñeán söï taäp trung löïc vaø öùng suaát leân heä thoáng implant naâng ñôõ, gaây ra tieâu
xöông.4
May maén thay, vieäc aùp duïng sô ñoà khôùp caén phuø hôïp coù theå laøm giaûm thieåu caùc
bieán chöùng vaø do ñoù toái öu hoùa ñieàu trò.

Khi laäp keá hoaïch ñieàu trò cho haøm phuû treân implant, coù nhieàu yeáu toá caàn xem xeùt ñeå toái
öu hoùa tuoåi thoï cuûa implant, khoùa caøi (attachments) vaø phuïc hình. Ñieàu baét buoäc laø baùc só
