1. Pharmacy and Therapeutic Committee

2. DEFINITION
PTC is composed of physicians, pharmacist and
medical staff and is concerned with the regulating the
use of medicines in the hospitals.
Or
PTC is the group of advisory peoples which are
engaged in matters related use of medicines in
hospitals and also analysis of drugs, dosage forms
and safe use of investigational drugs.
3. ORGANIZATION
The composition of the PTC is as follows,
Three Physicians from the medical staff of hospital
The Pharmacist
Nursing staff representative
One Ex-officio member as a representative appointed
from Hospital administration
One of the Physician is the chairman of committee
Secretary of committee is Pharmacist
6. FUNCTIONS
The committee has three main roles
1) Advisory
2) Educational
3) ADR and Safety monitoring
7. ADVISORY
PTC is involved in all the matters related with the use of
medicine in hospital
PTC assists in preparing the guidelines regarding
evaluation, selection and therapeutic use of medicines in
hospitals
The committee guides the hospital administration and
medical staff in the matters related with medicines use
8. ADVISORY
Further the committee also provides the guidelines for
hospital pharmacy related with the dispensing of drugs to
patients
Policy includes various issues, like who may prescribe or
administer drugs, direction during prescription and
guidance, to assure safe and proper use of high-risk,
high-volume, high-cost, or problem-prone drugs
9. ADVISORY
Moreover PTC also gives recommendation for stock of drugs in
hospital pharmacy
PTC also assist in the formulation of broad professional
policies relating to drugs, regarding their selection,
prescribing, procurement, storage, distribution, administration,
safety procedures, monitoring, and use
Furthermore PTC engaged in making rules and regulations for
administration, monitoring and outcomes of drugs and
diagnostic testing materials
For patients care PTC also recommends the improvement of the
efficacy and safety of drugs
10. EDUCATIONAL
PTC recommends or assists in the arrangement of
programs for Pharmacists, Nursing staff, Physicians
and other medical staff, which can help in
improvement of knowledge of medicines and their
use
11. ADR AND SAFETY MONITORING
PTC establishes proactive subcommittees for
medication safety and drug utilization evaluation
PTC overlook safe, economic and rational use of
pharmaceuticals across the healthcare facility
PTC monitors and reports all the ADR related with
the drugs (Pharmacovigilence)
12. SOME OTHER FUNCTIONS OF PTC
Recommends the protocols concerned with evaluation and approval
of investigational new drugs
Preparing, monitoring, assisting and implementing drug policy
Regulation of inventory at hospital Pharmacy
Monitoring and reporting drug Interactions
Formulation of Hospital Formulary and addition and deletion of
Drugs
Prepares emergency medicines list helpful in emergency situation
Managing, reporting and minimizing medication errors
13. POLICIES OF THE PTC IN INCLUDING DRUGS
INTO FORMULARY
PTC develops policy and procedure governing
hospital formulary
PTC maintains and prepares formulary based on
following principles
Effectiveness
Safety
Efficient
Cost
Patient centered
14. AUTOMATIC STOP ORDER
PTC formulates the rules and guidelines for the automatic
stoppage of dangerous drugs administration to patients
Dangerous drugs includes Narcotic, Sedatives, Hypnotics,
Antibiotics, Anticoagulants
These dangerous drugs should be automatically discontinued
after 48 hours after prescription
The Narcotic, Sedatives and hypnotic drugs should be re-
prescribed after 24 hours
unless and until the drug should be prescribed in specified
doses or may be reordered by the physician
15. AUTOMATIC STOP ORDER
These drugs should be only continued or prescribed if,
Physician writes the number of doses to be administered
Physician specifies the time period for administration of
drugs
Physician may re-prescribes the medicine
16. EMERGENCY DRUG LIST PREPARATION
In emergency situation it is important to administer the
exact drug to save the patients life, hence PTC prepares
the guidelines to keep the emergency drugs at the bed
side.
PTC prepares the list of such emergency drugs to be kept
in box at bed side
PTC gives instructions for keeping emergency drug kit to
Pharmacist and Nursing staff
