3. RUOLO del TSH
Measurement of serum TSH is the
primary screening test for:
• thyroid dysfunction
• evaluation of thyroid hormone replacement in
patients with primary hypothyroidism
• assessment of suppressive therapy in patients with
follicular cell–derived thyroid cancer

7. CHI TRATTARE
• There is general agreement that patients with primary
hypothyroidism with TSH levels above 10 mIU/L
should be treated
• Which patients with TSH levels of 4.5–10 mIU/L will
benefit is less certain.
• The possible exception to this statement is pregnancy
because the rate of pregnancy loss, including
spontaneous miscarriage before 20 weeks gestation
and stillbirth after 20 weeks, have been reported to be
increased in anti-thyroid antibody–negative women
with TSH values between 2.5 and 5.0
8. IPOTIROIDISMO SUBCLINICO (TSH <10 µU/ml)
• Con la eccezione della gravidanza (in atto o
programmata), di base questa condizione NON
RICHIEDE TRATTAMENTO:
• non ha documentate conseguenze cliniche
significative
• induce una inutile medicalizzazione del paziente
• espone il paziente al rischio di sovradosaggio di
tiroxina
9. IPOTIROIDISMO SUBCLINICO (TSH <10 µU/ml))
La terapia con tiroxina può essere
considerata in pazienti con:
• dislipidemia
• storia di scompenso cardiaco
• sintomi suggestivi per ipotiroidismo

Non sono indicazioni
trattamento con tiroxina:
• aumento di peso
• sintomi psichici
• deterioramento cognitivo
