2. What is (IVF) In Vitro Fertilization?
From the last 35 years, IVF has assisted thousands of intended couples
around the world overcome the infertility diagnoses enabling them to
realize their fantasy of enjoying parenthood. The treatment is often
recommended once other ART methods or infertility treatments have failed.
It is primarily tried for severe cases of male infertility or blocked Fallopian
tubes of a woman.
Although the IVF treatment cycle is
adjusted as per couple’s individual
needs and every clinic’s protocol
will be slightly different, here are
some steps of what normally takes
place during an IVF cycle.
3. Step 1: The Cycle Before Treatment
Before treatment, your doctor may put you on birth control pills. Seems
backward- are not you trying to conceive? Basically birth control pills
before treatment are proven to decrease risk of OHSS (Ovarian
Hyperstimulation Syndrome). In case rarely you get cycles,
progesterone is suggested to bring on your cycle. This initial process
of IVF may vary patient to patient, therefore it’s good to follow your
doctor’s instructions carefully.
4. Step 2: When You Get Your Period
Once you get your period, your
treatment cycle starts. On the day
2nd, your doctor will do a blood test
and an ultrasound in order to look
your estrogen levels and size of
your ovaries respectively. Doctor
also looks for ovarian cyst, if it is
present, treatment will be delay for
a week because most of the cysts
will resolve with time on their own
otherwise the cyst may be sucked
or aspirated with a needle. If tests
will be fine, treatment moves on 3rd
step.
5. Step 3: Monitoring & Preparing the Ovaries for Stimu
Normally, one egg matures per month in an ovulation cycle. However in
IVF, it is good to have as many mature eggs as possible to increase the
chances of success. In order to stimulate the ovaries, medications that
are derivatives of FSH and LH hormones are given as injectables for 8-
14 days approximately. Additionally during this phase 7-8 times you
must be available for monitoring phase which includes transvaginal
ultrasound and blood work.
6. Step 4: Egg Retrieval
With a trigger shot that ultimately provides maturation to the developing
follicles and sets ovulation in motion, your stimulation phase ends. Timing is
the most critical factor in this phase as retrieval of eggs must be done before
expected time of ovulation. The size of the follicles and the estrogen level
will help to decide when you are ready to trigger. Your partner’s sperm is
also collected on the same day of egg retrieval process.
You will be under light anesthesia during the
process and a needle is place via the vaginal
opening into the ovaries located through
transvaginal ultrasound and the follicles are
aspirated, will be immediately transferred to
the embryology lab for artificial fertilization.
7. Step 5: Inside The Lab, Egg Fertilization
Once eggs and sperm are collected from the intended parents or
donors, these are analyzed for maturity. The healthy eggs and sperm
are separated and sperm is then mixed with eggs or a single sperm
can be directly injected via ICSI (Intra-cytoplasmic Sperm Injection).
The embryos will usually stay in lab for 2 to 5 days as per growth of
embryos.
8. Step 6: Embryo Transfer
After few days of egg retrieval
and fertilization, the healthy
matured embryos will be
transferred which is a
painless process where
embryos will be placed into
your uterus with the help of a
thin tube or catheter passed
via cervix. Anesthesia
generally won’t require in this
process. The quality of
embryos will decide how
many embryos will be
transferred. Around 18 days
after the egg retrieval
process, a pregnancy test will
be performed.
