15. TeleCare Phone Outreach Services
“TeleCare” is a phone outreach service for patients who need a little monitoring to make sure
everything is going smoothly with them. The client is called on designated days and are asked
10 key questions about how they are getting along for that day. The client's answers to those
questions determine if there are any "yellow or red flags" present that could be a potential harm
or negative consequence to them. The "yellow or red flag" information is then scrutinized a little
more in order to determine if a home visit is needed for further investigation. The designated
family member is then notified with all information.
• Proactive calls can help ward off a crisis, ER visit, or a hospitalization
• Proactive calls are less costly, less disruptive and easier than reactive
interventions
• Ideal for clients with cognitive deficits, memory loss, or beginning
Alzheimer's
• Can be used as stand alone service or with other services

16. Emergency Help System with 2-way Voice Unit
The Emergency Help System or “EHS” is a personal emergency response system
with 2-way voice communication that makes live help just the push of a button
away. Installation of the unit base will connect your parent or loved one to a "live
person" at the emergency response center by pushing a single “ help” button on
the main transmitter unit. In addition there is the remote “help” button that is
carried with them on their belt, pocket, on a chain around their neck, or on their
wrist for that 24/7 availability of help. This life saving unit is rented monthly and is
connected to the patient's telephone system for continuous peace of mind service.

The Computerized Medication Dispenser “CMD” is a safe, convenient and
automated solution that organizes, reminds and dispenses medications so pills are
taken properly and on-time. The event reporting features make it a true cutting
edge device that improves medication adherence and reduces the risk of missed
and dosing errors. Reports are immediately generated to the family and/or the
nurse so phone follow up can determine if a home visit is needed to take care of an
issue. This unit can be installed and rented monthly with the services of the nurse
filling the medications regularly or with the family filling the unit with medications
for the patient instead. Nursing management services in conjunction with this CMD
rental, provide a complete stress-free and hassle-free experience for your parent
