The greatest TRUE challenges to Ayurveda physician today • Not unemployment and low income • Rather it is in reversing diseases fast and quick • No precision or predictability in prescribing • Need to depend on Western Medicine for health care challenges of self and family . • Inability to stay confident in learning and practice of Ayurveda. • Inability to explain to younger generations of the community, the real and intended prospects of this system
  3. 3. What is Evidence in EBP of Ayurveda ? • Evidence in Ayurveda is meant for two purposes : a. To know Science ( of patient, disease and treatment) b. To show data (of response of treatment in every individual patient) • The first evidence is meant for the physician community while the second one is meant for the world • The precision and predictability of second one is entirely dependent on pertinent choice and application of the first

What is going on today ? • Eminence Based Practice • Tradition Based Practice • Experience ( self and shared) based Practice • Allopathy Practice • Integrated Medicine Practice • Business led practice ( the prescribing and disposing unit are same) There is no Evidence Based Practice for at-least last one century in Ayurveda which means that our patients are not getting intended cure by our science

The solution Integrating evidences of Science Based Medicine into practice to make intended effects in patient outcomes instead of uncertainity led benefits and risks Jnana Poorva Karma is Evidence Based Practice in Ayurveda Jnana refers to Pratipatti jnana ( the knowledge of shsthraarthas and their application) and not statistics

SBEBA – Practice of JPK • Irrational prescribing is silent killer of Ayurveda and patients as well. • More than 98 percent of practitioners all over the world are doing irrational prescribing • Irrational prescribing refers to absence of practice of JPK • Irrational prescribing paves for morbidity, mortality and heavy financial burden for patients with poor outcomes in their health status • The only solution is to provide specific and specialised training for physicians and students of Ayurveda for JPK oriented Practice • The goal of SBEBA wisdom series is to enable physicians proficient in JPK

SBEBA had recognised these facts • Compared to conventional practice of Ayurveda, SBEBA induces high level of precision and predictability in prescriptions • Reduces medicine / treatment load and financial burden of patients • Fast and on the target recovery in comparison to slow and uncertainity led benefits and risks. • Happy healthy patients out of medicine and treatments who were once on the same . • Protection from future diseases and maintenance of quality of lifeby health after recovery from existing morbidities

Science- Physician- Patient Trio • SBEBA values patient health and money with greatest priority • For the patient, his topmost priority is undoubtedly earliest recovery and retaining routines • SBEBA integrates the pertinent Science Based Evidence corresponding to patient specific cause mechanisms to produce intended effect on intended time . • Excellence of well co-ordinated Science – Physician- Patient Trio contributes to success of therapeutic accomplishment

The myth of Yuktijnana • Yuktijnana is personalised logic • A Vaidya has the right to diagnose and deal with a patient in his own way . • Former experiences are the basement of Yukti and such Yukti is the basement for latter experiences • It is normal to get variable Yukties for the same disease of same patient by different physicians

The reality about Yuktijnana • Yukti is analysis of cause – effect relationship of the status by applying karya tatwas ( diagnostic principles of Ayurveda). • As Karya tatwas are standardised in Ayurveda, Yukti is also a standardised parameter . • Yukti is Shasthra sahita tarka ( Science Based Reasoning) and not blind assumption or imagination led by previous experience • Yukti is said to be based on shasthraarthas and not empirical data analysed by principles of statistics . • Yukti is developed by learning and culturing of shasthraarthas and does not develop automatically with advanced age and experience

SBEBA methodology for scientific decision making • SBEBA has developed a systematic standardised Methodology to develop Yuktijnana called Evidence Triad Approach (ETA) evaluating patient based on Hetu- Hetuvyadhi – Pancha Samprapthies . Aptopadesha jnana- Anumana – Yuktijnana – Application –Pratipattijnana • Right Shasthraarthas are to be chosen first • Anumana and Yuktijnana of Shasthraarthas based on ETA • Application of shasthraarthas by ETA • Confirmation of Cause- Mechanism- Effect by application – Upasaya/ Anupasaya • Attainment of Pratipattijnana ( confirmatory strategy) after confirmation of Yukti for further standardised application in identical situations

SBEBA wisdom series • Initiated in 2015 for the purpose of initiating Science Based Evidence Based Practice • 43 seminars in different parts of India and four webinars • Our goal is to establish the scientificity and intended potential of our medical system • We create leaders of Science Based Medicine and not followers for us • Our motto is “educate and motivate” • Every participant has a crucial responsibility to take the mission forward by working hard in science and exposing the results with the spirit of spreading the necessity of learning SBM

SBEBA wisdom series • Participants are to get off from other distractions • SBEBA has copyrights and any attempts for plagiarism of the same is legal offence. • SBEBA is meant to become scientific practitioner from the status of medicine prescriber. • You can start applying right from the first patient you get after the webinar and by very careful and judicious application of the logics you acquire here, you can make intended predictable results on time • Your cases are to be documented systematically along with logics of SBEBA applied in every situation • SBEBA is focussing on application of HOW TO CHOOSE instead of WHAT TO CHOOSE .
