it is an acute highly contagious /infectious diseases caused by a varicella zoster virus. chicken pox is usually a mild self limiting illness and most healthy children recover with no complication. Anyone who has had chickenpox in the past may develop shingles, you can only get shingles if you have previously had chicken pox as it is a recurrence or reactivation of the varicella zoster virus.it is not possible to develop shingles from exposure to a person with chickenpox it is possible however to develop chickenpox as a result of exposure to a person with shingles second attacks of chickenpox are rare but do occur.

Varicella-zoster virus is responsible for causing a primary varicella infection (chickenpox) and a secondary herpes zoster infection (shingles). Although varicella typically manifests as a mild disease in otherwise healthy children, it can also manifest as a moderate-to-severe disease, most notably in immunocompromised and adult hosts. Acyclovir is the antiviral agent of choice for the management of varicella infections. Routine vaccination with Varivax has been very effective in reducing chickenpox incidence

Tuberculosis (TB) is caused by bacteria (Mycobacterium tuberculosis) that most often affect the lungs. Tuberculosis is curable and preventable. TB is spread from person to person through the air. When people with lung TB cough, sneeze or spit, they propel the TB germs into the air. A person needs to inhale only a few of these germs to become infected. The causative agent is Mycobacterium tuberculosis (also known as the tubercle bacillus). Tuberculosis (TB) is an infectious disease that primarily affects the lung parenchyma. The primary infection usually involves the middle or lower lung area. It is also may be transmitted to other parts of the body, including the Meninges, kidneys, bone, joints, pericardium, GI tract and lymph nodes And this condition known as Extra pulmonary TB. The disease also can affects animals such as cattle, this is known as “bovine tuberculosis” which may sometimes be transmitted to man.The primary infectious agent, “ M.Tuberculosis”, is an acid – fast aerobic (AFB) rod that grows slowly and is sensitive to heat and ultraviolet light.

Defecation Normal defecation begins with movement in the left colon, moving stool toward the anus. When stool reaches the rectum, the distention causes relaxation of the internal sphincter and an awareness of the need to defecate. At the time of defecation, the external sphincter relaxes, and abdominal muscles contract, increasing intrarectal pressure and forcing the stool out The Valsalva maneuver exerts pressure to expel faeces through a voluntary contraction of the abdominal muscles while maintaining forced expiration against a closed airway. Patients with cardiovascular disease, glaucoma, increased intracranial pressure, or a new surgical wound are at greater risk for cardiac dysrhythmias and elevated blood pressure with the Valsalva maneuver and need to avoid straining to pass the stool. Normal defecation is painless, resulting in passage of soft, formed stool CONSTIPATION Constipation is a symptom, not a disease. Improper diet, reduced fluid intake, lack of exercise, and certain medications can cause constipation. For example, patients receiving opiates for pain after surgery often require a stool softener or laxative to prevent constipation. The signs of constipation include infrequent bowel movements (less than every 3 days), difficulty passing stools, excessive straining, inability to defecate at will, and hard feaces IMPACTION Fecal impaction results from unrelieved constipation. It is a collection of hardened feces wedged in the rectum that a person cannot expel. In cases of severe impaction the mass extends up into the sigmoid colon. DIARRHEA Diarrhea is an increase in the number of stools and the passage of liquid, unformed feces. It is associated with disorders affecting digestion, absorption, and secretion in the GI tract. Intestinal contents pass through the small and large intestine too quickly to allow for the usual absorption of fluid and nutrients. Irritation within the colon results in increased mucus secretion. As a result, feces become watery, and the patient is unable to control the urge to defecate. Normally an anal bag is safe and effective in long-term treatment of patients with fecal incontinence at home, in hospice, or in the hospital. Fecal incontinence is expensive and a potentially dangerous condition in terms of contamination and risk of skin ulceration HEMORRHOIDS Hemorrhoids are dilated, engorged veins in the lining of the rectum. They are either external or internal. FLATULENCE As gas accumulates in the lumen of the intestines, the bowel wall stretches and distends (flatulence). It is a common cause of abdominal fullness, pain, and cramping. Normally intestinal gas escapes through the mouth (belching) or the anus (passing of flatus) FECAL INCONTINENCE Fecal incontinence is the inability to control passage of feces and gas from the anus. They also focused on the urgency of large-scale controlled trials of antimicrobial-use regulation employing sophisticated epidemiologic methods, molecular typing, and precise resistance mechanism analysis. Antimicrobial Stewardship(AMS) refers to the optimal selection, dosing, and duration of antimicrobial treatment resulting in the best clinical outcome with minimal side effects to the patients and minimal impact on subsequent resistance. According to the 2019 report, in the US, more than 2.8 million antibiotic-resistant infections occur each year, and more than 35000 people die. R INSTITUTIONS , BANGALORE 2 TONSILITIS: Definition: CAUSES: Bacterial infection – Streptococcus, Staphylococcus aureus, pneumonia, strep throat, whooping cough. Viral infection – cold virus, HSV, measles. Allergic reaction - hay fever.

TYPES: 1.Acute Tonsillitis: Sudden
onset of symptoms, usually caused by infections. 2.Chronic Tonsillitis: Recurrent inflammation of the tonsils, often lasting for an extended period. SYMPTOMS: 1.Sore throat: Pain or discomfort in the throat. 2.Swollen tonsils: Enlarged and red tonsils. 3.Difficulty swallowing: Due to throat pain. 4.Fever: Often present, especially in bacterial infections. 5.Headache and ear pain: Common accompanying symptoms. 6.Bad breath: Resulting from the accumulation of pus on the tonsils (in bacterial infections). DIAGNOSIS: 1.Medical History: Including recent illnesses and symptoms. 2.Physical Examination: Inspection of the throat, checking for redness, swelling, or pus. 3.Throat Culture: To identify the causative organism, especially in cases of bacterial tonsillitis. 4.Blood Tests: To detect a bacterial infection or to check for mononucleosis. 1/30/2024 © R R INSTITUTIONS , BANGALORE 3

TREATMENT: 1. Antibiotics: If
bacterial infection is confirmed. 2. Antiviral medications: For viral tonsillitis. 3. Pain relievers: Over-the-counter medications to alleviate discomfort. 4. Warm saltwater gargle: Helps soothe the throat. 5. Rest: Adequate rest to support the body's healing process. 6. Hydration: Drinking plenty of fluids to stay hydrated. SURGICAL TREATMENT: Tonsillectomy. NURSING MANAGEMENT: 1. Comfort Measures: Provide throat lozenges, warm beverages, and a cool-mist humidifier. 2. Monitor Vital Signs: Regularly check temperature, pulse, and respiratory rate. 3. Encourage Fluid Intake: Ensure the patient stays well-hydrated. 4. Administer Medications: Administer antibiotics or other prescribed medications as per the healthcare provider's instructions. 5. Nursing diagnosis: Ineffective airway clearance related to pus or mucus formation secondary to inflammation. Acute pain related to the presence of inflammation in the tonsil. Imbalanced nutritional less than body requirement related to inadequate intake. Hyperthermia related to acute infections by microorganism. Disturbed sleep pattern related to pain in the tonsil area. NURSING INTERVENTION: Suctioning or sight lying position to facilitate drainage of secretion. Administer analgesic and antipyretic as order. A soft and liquid diet is provided.
