Thời gian thông suốt của cầu nối là một tiêu chuẩn quan trọng để đánh giá kết quả
phẫu thuật. Thời gian này phụ thuộc vào nhiều yếu tố, trong đó vật liệu làm cầu nối
là yếu tố rất quan trọng. Ngày nay, động mạch ngực trong, động mạch quay và tĩnh
mạch hiển được sử dụng rộng rãi để làm cầu nối [67], [78], [100].
Sử dụng động mạch quay để làm cầu nối động mạch vành được Carpentier giới
thiệu lần đầu tiên vào năm 1971 [54]. Động mạch quay tương tự chiều dài và kích
thước động mạch ngực trong, đường kính của nó gần với đường kính của động
mạch vành hơn so với tĩnh mạch hiển, nó dễ lấy và hiếm khi bị ảnh hưởng bởi xơ
vữa động mạch. Tuy nhiên, bất chấp những lợi thế tiềm năng, năm 1975, Curtis và
các đồng nghiệp báo cáo tỷ lệ thất bại của cầu nối động mạch quay trong một
nhóm 79 bệnh nhân là 64,7% ở thời điểm 12 tháng sau phẫu thuật. Tỷ lệ thất bại
này cao hơn đáng kể so với cầu nối tĩnh mạch hiển và động mạch ngực trong trái
được sử dụng trong các bệnh nhân tương tự.
ĐÁNH GIÁ HIỆU QUẢ MẢNH GHÉP ĐỘNG MẠCH QUAY TRONG
PHẪU THUẬT BẮC CẦU ĐỘNG MẠCH VÀNH Năm 1981, Christophe Acar,
Carpentier và cộng sự bắt đầu thực hiện một nghiên cứu mới gồm 910 bệnh nhân
[21]. Một số cải tiến so với kỹ thuật ban đầu đã được áp dụng trong lúc phẫu thuật
lấy động mạch quay và sử dụng thêm các thuốc chống co thắt mạch máu mà nổi
bật là sử dụng thuốc ức chế canxi sau mổ. Kết quả lâm sàng và chụp động mạch
vành sớm sau phẫu thuật cho thấy kết quả rất tốt, với tỷ lệ hoạt động tốt của cầu
nối là 92% sau 10 năm. Nghiên cứu của các tác giả khác cũng cho kết quả tương tự
[51], [58], [64], [65].
Ở nước ta, phẫu thuật bắc cầu động mạch vành được triển khai lần đầu tại Việt
Nam vào năm 1997 tại Bệnh viện Việt Đức, Hà Nội; năm 2000 tại Bệnh viện Chợ
Rẫy và năm 2001 tại Viện Tim Thành phố Hồ Chí Minh [1], [8], [12]. Một số

nghiên cứu ở Việt Nam đã đánh giá về vai trò của các mảnh ghép trong phẫu thuật
bắc cầu động mạch vành như động mạch ngực trong, động mạch quay, tĩnh mạch
hiển, toàn bộ cầu nối sử dụng động mạch.
Mặc dù đa số những báo cáo trong nước và ngoài nước liên quan đến sử dụng
mảnh ghép động mạch quay trong phẫu thuật bắc cầu động mạch vành xảy ra trong
10 năm gần đây đều cho rằng động mạch quay là một mảnh ghép tốt và việc sử
dụng mảnh ghép này trong phẫu thuật bắc cầu động mạch vành có nhiều ưu điểm,
hiệu quả và an toàn, vẫn còn đó những câu hỏi chưa được trả lời xác đáng và vẫn
còn tranh luận. Với mong muốn có thêm một mảnh ghép ưu việt trong phẫu thuật
bắc cầu động mạch vành, tôi thực hiện đề tài với những câu hỏi nghiên cứu:
S Tỷ lệ thành công và tỷ lệ thông nối của mảnh ghép động mạch quay trong phẫu
thuật bắc cầu động mạch vành là bao nhiêu?
S Có sự khác biệt không giữa kỹ thuật nối đầu gần của động mạch quay – động
mạch chủ và nối động mạch quay – động mạch ngực trong trái kiểu

MỤC TIÊU NGHIÊN CỨU
1. Đánh giá kết quả sớm và kết quả trung hạn của mảnh ghép động mạch quay
trong phẫu thuật bắc cầu động mạch vành.
2. Đánh giá kết quả giữa nhóm nối đầu gần của mảnh ghép động mạch quay – động
mạch chủ và nối động mạch quay – động mạch ngực trong trái kiểu Y.
