Basic Information
To turn on the phone, press and hold Power key for 3 seconds.
To Turn off the phone, press Power key for 3 seconds; touch Power Off, and touch Ok.

Connecting to Wi-Fi
1. On the home screen, touch Applications tab and select
Settings > Wireless & networks > Wi-Fi settings.
2. Touch Wi-Fi to turn it on and begin scan for available Wi-Fi networks.
• List of available Wi-Fi networks are displayed. Secured networks are indicated by a
lock icon. Touch a network to connect to it.
3. If the network is open, you are prompted to confirm that you want to connect to that
network by touching Connect.
• If the network is secured, you're prompted to enter a password or other
credentials.
4. When connected, you will see the Wi-Fi icon in the status bar.
If not using Wi-Fi, disable it to save battery life!

Unlocking the Screen
Whenever your LG-P500 is not in use, it will return to the lock screen. Touch the
lock icon and drag (swipe) your finger to the right to unlock the screen. This will
reveal the home screen.

Power
• How long will the Smartphone battery last?
• This depends on if wireless services are turned on, and if a SIM card is installed.
• The brightness can also affect battery life – keep it low if possible.
• How can I maximize battery life?
• Keep Wi-Fi off until you’re ready to submit data!
• Keep Bluetooth off!
• Disable GPS, especially if you have a SIM card installed.
• When should I charge my Smartphone?
• Whenever you can! Every night, if possible.
• If you are not using it, plug it in!
• If the battery indicator is less than 1/3, you should start planning to charge the battery.
You can see the battery indicator in the upper right corner of the display.

When not needed, disable wireless services. Do this by touching the status bar
and bringing your finger down to the bottom of the screen.

delicate and can crack if dropped or stepped/sat on.
Do not:
• Put it in your back pocket!

Troubleshooting
If Smartphone screen turns off
• It just went to sleep to save battery life. This happens after
about 15 seconds. Press any of the bottom keys or the power
button to wake it up again.
If Smartphone seems frozen or non-responsive:
1. Try pressing the power button for 3 seconds.
2. If this does not work, remove the battery and re-insert again.
Then, hold the power button for 3 seconds.
3. If the screen will not turn on after replacing the battery and
pressing power, plug it in. The battery may be dead.

Downloading new Forms
This process is only necessary when
there are new forms
1. In EpiSurveyor, tap “Manage Files”
2. Tap "Remote" in the upper right
3. Tap "Public", and then press OK.
• You should see all of the forms.
4. Tap each form so that the green checkbox
appears, and click the "Add Selected
File(s)" button.
5. Press the back button (to the right of
home) to go back to the EpiSurveyor main
menu.

1. With EpiSurveyor open, tap Enter
New Data
2. Tap the form to select it
3. Swipe finger to the left to move
forward

5. If you are going to pause taking the
survey, press the Menu button and
then tap Save and Exit. Then click
Save as Incomplete.

Navigating questions
There are several types of questions in the forms:
• Multiple Choice & Yes/No
• Numeric
• Date
• Text
You can turn the Smartphone sideways to change the screen orientation and
reveal a larger keyboard. This is especially useful for text entry.

Resuming where you left off
1. Once a form is saved, tap Review Saved Data to
resume entering data
2. Select the form you were working on
3. Scroll to the first question you have not
answered, and tap it to select. You can identify
this question by the “~” symbol, which indicates
that is has not yet been answered.
4. Resume taking the survey. When you need to
save again, do it the same as before.
You may save one form, and begin working on
another. You can always go back to the forms to
finish them later, as long as you saved.

Completing the forms
When you have reached the send of the form, you will see
the completion screen.
• If you want to go back and change answers
before submitting, tap Save as Incomplete.
• If you are finished entering data for that form,
tap Save as complete
In both cases, you can still edit your data on the phone
before submitting it. On the home screen, tap Review
Saved Data. You will see tabs for both Complete and
Incomplete.
To jump to the end of the form when editing:
• Press the Menu button
• Tap Jump to Question
• Tap the Jump to button
• Tap End of the form

Uploading data
This requires the Smartphone is online via a Wi-Fi or cellular data connection
The most important step is uploading your data once it is completed.
1. Tap Send Completed Data
2. Tap each form so that the green
checkbox appears
3. Tap Send Selected File(s). You should see a
confirmation “X files have sent successfully”
4. Confirm that files have been sent by looking at the Send Completed Data
button. The number in (parentheses) should read 0.

Troubleshooting
If you are unable to upload the data, check that:
1. Wi-Fi is on. Drag your finger from the status bar to the
bottom of the screen. The Wi-Fi icon should be green. If it’s
grey, tap it to turn it on (green).
2. Wi-Fi is connected. Even if Wi-Fi is on, you still need to be
connected to a network. If you do not see the icon on the
right, follow the instructions on the Connecting to Wi-Fi slide.
3. If you are still unable to submit data, submit a ticket to the
Help Desk at http://helpdesk.tbdata.info/
You may also use a cellular data connection with a SIM card to
upload data. This may be easier if you are already using your
phone for email or internet.
