Main Directive

       The purpose of this proposal is to provide the information necessary for you to
consider using our firm as a primary source of marketing that will give you a consistent
flow of qualified prospects on a daily basis. Our services are priced on a performance
fixed fee basis keeping your reimbursement capabilities and profitability in mind.

       Our firm provides our clients with excellent live (real-time) in bound calls of peo-
ple looking for Back Braces among other products and services including testing sup-
plies, power chairs, shoes, pain products and organic supplements that will drop the
blood sugar counts.

       Adverting methods used to generate Medicare and insurance Compliant inbound
calls are: National PI—Radio and TV ads, online marketing, direct mail, relationship
marketing with associations and churches, print advertising and referral based pro-
grams. We are also one of the largest source of diabetic prospect data in the US. See

      Our system will help you provide products and services by delivering them in a
timely fashion using the upmost care and the highest respect to patients.
      We will do whatever is necessary to maintain a professional and user friendly sys-
tem that will benefit all of the patients, and most importantly your organization.

Challenges and Process...

       Our process is to first qualify the inbound caller and then find the appropriate
program to help them with their health needs. We filter out people that are unwilling to
provided needed information to determine eligibility. Medicare is by far the program
that will monetize the quickest for most providers so our initial goal will be to deter-
mine which people have that coverage. Through this process we will also determine
which patients qualify for private insurance, Medicaid, cash programs as well as
monthly supplement programs.

Qualification steps we will take are as follows:

       1. Our call centers take inbound calls as ads run throughout the US.
       2. Our online system allows us to qualify self reported medical needs and insur-
ance coverage as well as other pertinent details.
             A. We confirm their age and obtain complete contact information.
             B. You must obtain a doctor order confirming their medical need for Back
Bracing so we obtain doctor information. (See enclosed screen shots with our internal
questionnaires)
       3. After determination and qualification, the patient is transferred to your office
for a phone conference call immediately. A three way hand shake is done between the
patient, our representative and your sales agents.
4. Once the patient is qualified and transferred from our call center you will be
able to see the details online while you are talking to the patient. You will also receive a
confirmation email of the content. At this point our representative will back out of the
call and allow you to finish the intake on your end.

        5. We schedule in bound calls for you based upon your hours of operation and the
number of people you have taking those calls. As a rule our clients tell us that each
customer service rep can handle from 5 to 10 calls per day depending the other respon-
sibilities they have in your office.

Basic Products Offered

      We will primarily offer advertising and qualification for these products:

            1. Diabetic Testing Supplies
            2. Other Diabetic Products—Back Braces, Shoes,CPAP etc.
            3. Power Chairs
            4. Data Bases of diabetics for direct mail
            5. Organic Supplement To Lower Blood Sugar Count— You Earn Reoccurring
            Monthly Fees on these products

Fees & Commission

      We charge approximately $50 to $70 in marketing fees for each intake we pro-
duce on diabetic and mobility products. These fees are based upon volume of orders,
consistency of orders and other specifics that will be discussed.

Data is priced per thousand records obtained.

DME partners that participate in our supplement program earn from $10 to $20 per

Our Capabilities

       We currently have one in house call center with 30 stations and two other loca-
tions that represent another 150 stations.

      We can immediately handle your projected needs from as little as 50 calls per
week to over 100 calls per day for any given client.
John White, Marketing Chief has been in this business for 22 years and has been in-
volved in diabetic marketing and operations for 18 years since the inception of Liberty
Medical. We are intimately familiar with Medicare compliance and are experts in many
types of marketing and media management for diabetic venues.
What’s Next
