HLTENN015 Apply Nursing Practice In The Primary Healthcare Setting
Answers:
Report
Answer 1
The first intervention that the nurses are required to undertake is to identify the risk of falls
for their patient Mrs. Reynold’s. One of the most important factors in her old age as she is 85
years. Her stiffness in her body and the pain in her knees, as well as hips, make her feel that
she might fall. Therefore the nurses are required to identify these factors and warn the
patient of such factors and adversities. they must also provide assistive devices along with
other protocols like wristband sticks, no keeping the bed of the patient at a lower position,
and are the modifications in the surroundings and furniture within the residence of the
patient.
Answer 2
The next nursing intervention is as per the respectful practices in order to support the
dignity and privacy of Mrs. Reynold’s in palliative care. This is a specialized form of medical
care that is offered to people with Serious and long-term illnesses. Old age comes with a
variety of issues as prevalent in the case of Mrs. Reynold’s. Some of these include diabetes,
old dentures, difficulty in reading and hearing, bruises on her skin, and many more. this
requires palliative care covering both physical intervention and mental care on the part of
the nurses. this will also help the patient to maintain her dignity as well as privacy as an
individual (Naseri-Salahshour et al., 2019).
Answer 3
One important health literacy issue that inhibits Mrs. Reynold’s capability of accessing,
evaluating, understanding as well as communicating information two maintain as well as
enhance her health is the lack of mental health awareness. She feels depressed and lonely
due to her old age as well as the absence of attention from her daughter. Therefore even if
she sleeps well and wakes up late in the morning, she has refrained from attending church
as well as the local senior citizens club which she previously used to enjoy. therefore this is
a direct sign of depression that she has developed over the years (Ebrahem & Alam, 2016).
Therefore, she must be educated on various mental health issues so that he is able to
identify and become aware of her own. She must also be familiarized with people facing
mental health issues. the practice of becoming aware and catering to one’s mental health
issues must be normalized (Chen, 2021). This will include community visits to mental
health hospitals, one on one consultations with counselors and psychologists, and her
attendance to community workshops on mental health.
Answer 4
Nursing intervention for Mrs. Reynold’s must also include collaboration with her primary
caregivers and interdisciplinary health care teams. this week includes her daughter and
various sectors like health care education, mental health, house physicians and doctors,
community health care services as well as nurses to help her derive holistic care starting
from receiving subsidized services at the hospital, to travel reimbursements as well as
home care (Jin & Chen, 2021). The first step will be to educate Mrs. Reynold’s daughter
about her physical and mental health needs and encourage her to collaborate with the
interdisciplinary health care teams to provide holistic care to her mother.
Answer 5
The primary health goal is to enhance the physical and mental health of Mrs. Reynold’s with
collaboration amongst all stakeholders. The time frame for achieving the primary goal is six
months.
