HIV is a minimal virus that can cause AIDS. AIDS is not a disease in itself but a syndrome. It reduces human's natural immunity to fight other diseases. CHAPTER 13 WORKING WITH PEOPLE WHO LIVE WITH HIV AND AIDS: THE PROBLEM AND HUMAN SERVICES WM. LYNN MCKINNEY As a human service worker, you are almost certain to have clients who are people living with human immunodeficiency virus (HIV) and persons living with AIDS (PWAs). This is true because AIDS affects people of all ages, sexual orientations, and ethnic and minority groups and because the needs of PWAs and people living with HIV are numerous and span virtually all human service programs. Because AIDS is an illness, there are medical needs. Since most PWAs eventually must stop working, they have income needs. The number of children who have HIV is growing, and these young people may have educational needs. For several reasons, there are likely to be psychological and social needs. First of all, AIDS occurs primarily in people younger than fifty, which means that PWAs must deal with a fatal illness at an early age. Second, AIDS is found mainly in marginalized people such as homosexual men, injection drug users, and racial and ethnic minorities. Still another reason that PWAs may have psychological needs is that many of them will have many friends who are seriously ill or who have died. The effects of such losses are potentially enormous. Thus, the entire human service system is involved in working with people living with AIDS and HIV. So, as you enter the field, it is important that you know about HIV and AIDS and society’s reactions to the disease. Working with people with AIDS and HIV presents particular challenges. Many people with this illness will not have close ties with their families who may have turned their backs on what is perceived to be a social embarrassment. Most will be poor, some because they were poor when they became sick and others because the disease is impoverishing. Death from AIDS-related causes can be horrible; the diseases and infections that affect a PWA can leave people thin and weak and seemingly defenseless for long periods of time. Unlike those with other illnesses, PWAs may be very sick and close to death for a while and then go through long periods of good health when they can lead happy, productive lives. As more cases of AIDS are diagnosed among drug users, more clients may be difficult to work with. Finally, with more women contracting the virus and dying, there are orphaned children who are not infected as well as children who were born with the virus. Many of these children are not or cannot be cared for by their mothers or other relatives. However, working in the HIV field can be rewarding. Priester April 30, 2018 HIV/AIDS Abstract HIV is a virus that causes AIDS. The term in full stands for Human Immunodeficiency Virus. This virus has led to massive deaths globally with many more people being infected as well as being affected by the effects of the virus. In this paper, we shall describe the virus in depth, showing how it gets into a person's body, its causes and symptoms. We shall also discuss its prevalence and how it has affected people's lives. Most importantly, we shall discuss how a person can be treated after being infected with the virus. Finally, we shall study various preventative measures that can be used against the virus. HIV Definition The term HIV means Human Immunodeficiency Virus. This is a virus that makes the immune system of a person weak through destruction of vital cells that fight infection and diseases. To date, there is no existence of a cure in the United States nor in other parts of the world. Nevertheless, it is easy to prevent HIV infection through varying control measures. . Depending on different factors, some people are more prone to getting infected than others. Such factors include risky sexual behaviors, sex partners and environment (CDC, 2018).HIV is to a large extent spread through unsafe sex, blood contamination, needles, breastfeeding, and delivery and from a mother to her child during pregnancy. AIDS Definition Unlike HIV that is a virus, AIDS is a full-blown disease caused by HIV virus. Once a person's immune system becomes extremely weak, then he\she is said to have AIDS. At this moment, a person's immune system is not able to fight diseases or infection. There are certain illnesses and symptoms that develop as a result of a person contracting AIDS. This can also be described as the last HIV stage since at this moment the infection is highly advanced. If the symptoms of this disease are not controlled or treated, a person ends up losing life (Avert, 2017). How a Person Get Infected with HIV and HIV Life-cycle HIV attacks T- helper cells which are also known as CD4 cells. This are some forms of white cells. A person is able to have a strong immune system through CD4 cells being healthy because it's the only way they can be able to fight infections and diseases. It is not possible for HIV to reproduce or grow without being in T-helper cells. HIV makes many copies or reproduces while inside these cells. As a result of the reproduction, the immune system gets damaged which causes it to weak a person's natural immune system. The overall health of a person is the main determinant of how the HIV virus grows. Other determinants of HIV growth is how constant a person takes treatment and how early a person gets diagonalzed. Thoracotomy is the surgical procedure with an incision made to access the pleural space and the contents of the thoracic cavity. Given the structures to be accessed with in the cavity, different incisions have been established to easy the procedure and these incisions qualify the types of thoracotomies to be studied hereunder. PRE OP OPTIMIZATION FOR SPECIFIC FACTORS The outcome of surgical procedures is not measured only by clinical end points but also shorter stays and lower costs. Patients’ discharge is delayed commonly due to inadequate pain relief, infection, arrhythmias, prolonged air leak and debility. Many complications that occur from thoracic operations can be anticipated. An aggressive preoperative work up mitigates morbidity and shortens convalescence. APPROACH CONSIDERATIONS There are about three principles that can guides the choice of the thoracotomy incision to be used I. Adequate exposure must be achieved. The choice of incision is aided by a thorough understanding of the surface anatomy and a comprehensive review of the radiographic images that are obtained preoperatively. II. Chest-wall function and appearance should be preserved to the extent possible. This principle include non-spreading video-assisted thoracoscopic surgery (VATS) procedures, muscle-sparing techniques, avoidance of excessive rib retraction, and rib preservation when possible. III. The third principle is that closure must be meticulous and appropriate. Strict layered closure is the rule for thoracic surgical incisions. Every effort should be made to approximate the individual divided chest-wall muscles in appropriate layers; otherwise, a significant delay in the recovery of range of motion (ROM) may result. Care must be taken to avoid over approximating the ribs and to prevent an override; this will help minimize postoperative pain. POST-OPERATIVE CARE AND MONITORING
