Let me provide an example. This is a survival curve that was constructed for patients like our 7 year old. The y-axis is proportion of patients that do NOT experience an asthma-related rehospitalization or ED revisit following an index admission. The x-axis is time, measured in days. You see 3 lines based on risk level. At roughly 30 days post discharge, you see these lines diverge and remain diverged for the rest of the follow up period. At 365 days, those with 1 of our predefined risks are at a 30% greater hazard of reutilization. Those with 2 or 3 risks have an 80% greater hazard of reutilization. So what are the risks?
Here, risk was not calculated by genetic polymorphisms or differing degrees of eosinophilia. Here, risk was calculated more simply, by identifying geomarkers commonly and publically available from the US Census that are capable of being linked to any individual’s street address or neighborhood.
We also consider how the overlaying of key data elements enhance meaningful partnerships supporting both neighborhoods and patients. The Cincinnati Child Health-Law Partnership or Child HeLP is a medical-legal partnership between Cincinnati Children’s primary care centers and the Legal Aid Society of Greater Cincinnati. Child HeLP allows us to more routinely identify and act on risks related to common social determinants, including housing. Recently, we showed the map depicted here to our legal partners.
Instead of having healthcare data alone or housing data alone,
This map presented both together. Avondale has a blue background that shows housing code violation density. Each dot that is present atop represents a patient cared for at our primary care centers. Red dots represent those diagnosed with asthma. When we showed this to housing attorneys like Virginia Tallent who you met earlier, they identified a pattern, a cluster of red in building complexes with known problems, sites where they were trying to make in-roads. By merging healthcare and housing data, we, together, identified a ‘hot spot’ of both medical complexity and substandard housing. In the problem complexes, including the one circled and the Burton building you saw earlier, the asthma rate is three times the national average. There are also a staggering number of housing or building code violations at each site. We are now working together to improve housing and reduce morbidity by leveraging our collective expertise.
