Initiated integration process of a 600-physician hospital organization to develop health interventions at the
physician and community levels, involving: physicians, four hospitals, and a data vendor to develop evidence-
based clinical practice guidelines, collecting and measuring outcomes over time from inpatient, outpatient,
emergency room, and ancillary services.
Developed PCMH NCQA program for 53 practices within the PHO, submission to NCQA 2012-2013 achieving
level 2 and level 3 recognition.
