High Level Solution Description
• Automated speech recognition system that collects clinical data from patients with
heart failure to support more efficient population management.
HealthCare/Business problem addressed
• Heart Failure (HF) is the most expensive preventable condition, regardless of
patient ethnicity, race, socio-economic status, gender, or insurance status. HF
results in more than $30 billion in medical costs annually. Patient-centered
telephonic communication and monitoring positively impact patient adherence,
satisfaction, and outcomes. Communications involving skilled clinical staff are
expensive; HF-ARMS provide a cost effective solution with equal clinical efficacy.
The application developed addresses the following

Patient communication is triggered automatically based
on specific clinical conditions in Allscripts
Communication results with details of the patient
interaction are available via a secure web interface in
real-time to care providers.
Automatic provider alerts are triggered based on
responses by patient

HF-ARMS is timed perfectly for the changing health care

risk for avoidable expense; Emergency Department and
Inpatient use.
Those who take risk are expanding from traditional
health plans to Accountable Care Organization and full-
risk medical groups
