En Chile el Centro de Derivación Nacional para Quemados Graves es el Servicio de Quemados del HUAP. La entrada en vigencia de una Garantía Explícita de tratamiento exigió una intervención para responder a nuevas necesidades
Objetivo: Dar cuenta de la primera fase de la intervención en la que, un desfase en la llegada de recursos, produjo una situación particular. Sólo se logró modificar el Protocolo de Tratamiento Quirúrgico y su aplicación, manteniéndose constantes el resto de los factores. Esto permitió evaluar aislada y prospectivamente el impacto de este factor en los resultados de mortalidad.
Metodología. Evaluamos prospectivamente la mortalidad por Grupo de Gravedad de Garcés: Graves, Críticos y Letales. Usamos como base de comparación los resultados del 2000 al 2004. Definimos como indicador prospectivo de resultado la mortalidad en el grupo crítico.
Resultados: Produjimos reducciones significativas en la mortalidad en todos los grupos. En críticos de 75.4 % a 35.5
Discusión: Una situación excepcional permite evaluar aisladamente el impacto de la modificación del Tratamiento Quirúrgico en la mortalidad de Quemados Graves.

La cirugía conservadora de tumores músculo esqueléticos ha mejorado significativamente sus resultados funcionales incorporando, en la reparación primaria, aloinjertos óseos o prótesis, evitando así la ablación. Sin embargo, cuando esos elementos se exponen, se infectan y deben ser retirados. Esto eleva el porcentaje de amputaciones sobre el 50% haciendo fracasar un gran esfuerzo terapéutico. El desafío es conseguir evitar la amputación protegiendo oportuna y eficazmente los elementos expuestos.
Objetivo: Comunicar los resultados de la aplicación de colgajos en el tratamiento de complicaciones de la cirugía conservadora de tumores músculo esqueléticos.
Material y Método: Se trataron quince pacientes inicialmente reparados con aloinjertos 9, con prótesis 6. Localizados en: pelvis 1, rodilla 7, piernas 4, tobillo 3. Utilizando principios similares a los usados en la reparación de osteomielitis y fracturas expuestas III B la reparación secundaria se realizó con colgajos: recto abdominal distal 1, gemelo 8, sóleo 3, sural 4. Un paciente requirió dos colgajos. En dos se asoció aspiración continua.
Resultados: Se evitó la amputación en todos los casos.
Discusión: La aplicación de principios utilizados en la reparación de osteomielitis y fracturas expuestas IIIB permite evitar en un alto porcentaje la amputación en complicaciones de cirugía conservadora de tumores musculoesqueléticos.

El Abdomen abierto complicado es un cuadro crítico de elevada mortalidad, resultado de traumatismos, infecciones, malformaciones en los que se establece un circulo vicioso que se retroalimenta produciendo sepsis, desnutrición, inmunodeficiencia, fístulas, FOM y muerte.
Objetivo: Comunicar los resultados y la estrategia terapéutica diseñada para mejorar la sobrevida, romper el círculo vicioso, cerrar la brecha cutánea, cerrar las fístulas y reparar la pared basada en uso de homoinjertos, autoinjertos, colgajos cutáneos y musculares.
Resultados: Se tratan 17 pacientes 9 hombres y 8 mujeres. Edades: entre tres semanas y 64 años. Cuatro lesiones congénitas y 13 adquiridas.
En 15 pacientes se consigue el objetivo. Dos fallecen.
Discusión: La combinación de recursos diversos de la cirugía plástica en una estrategia adecuada de reparación puede contribuir a mejorar significativamente la oportunidad de sobrevivir de los pacientes que presentan un abdomen abierto complicado.

El último estudio de la consultora CB revela que el presidente Javier Milei comenzó a mostrar flancos en su imagen positiva. Su aceptación bajó en catorce de las 24 provincias comparado al mes de Mayo. La mayor imagen positiva la encuentra éste mes en Córdoba (64.8%), mientras que la imagen más baja la obtiene en Santiago del Estero (41.5%). El distrito donde más cayó comparado a la medición anterior fue Buenos Aires (-3.5%) y donde más creció fue Córdoba (+2.9%).

Mental health has been in the news quite a bit lately. Dozens of U.S. states are currently suing Meta for contributing to the youth mental health crisis by inserting addictive features into their products, while the U.S. Surgeon General is touring the nation to bring awareness to the growing epidemic of loneliness and isolation. The country has endured periods of low national morale, such as in the 1970s when high inflation and the energy crisis worsened public sentiment following the Vietnam War. The current mood, however, feels different. Gallup recently reported that national mental health is at an all-time low, with few bright spots to lift spirits.
To better understand how Americans are feeling and their attitudes towards mental health in general, ThinkNow conducted a nationally representative quantitative survey of 1,500 respondents and found some interesting differences among ethnic, age and gender groups.
Technology
For example, 52% agree that technology and social media have a negative impact on mental health, but when broken out by race, 61% of Whites felt technology had a negative effect, and only 48% of Hispanics thought it did.
While technology has helped us keep in touch with friends and family in faraway places, it appears to have degraded our ability to connect in person. Staying connected online is a double-edged sword since the same news feed that brings us pictures of the grandkids and fluffy kittens also feeds us news about the wars in Israel and Ukraine, the dysfunction in Washington, the latest mass shooting and the climate crisis.
Hispanics may have a built-in defense against the isolation technology breeds, owing to their large, multigenerational households, strong social support systems, and tendency to use social media to stay connected with relatives abroad.
Age and Gender
When asked how individuals rate their mental health, men rate it higher than women by 11 percentage points, and Baby Boomers rank it highest at 83%, saying it’s good or excellent vs. 57% of Gen Z saying the same.
Gen Z spends the most amount of time on social media, so the notion that social media negatively affects mental health appears to be correlated. Unfortunately, Gen Z is also the generation that’s least comfortable discussing mental health concerns with healthcare professionals. Only 40% of them state they’re comfortable discussing their issues with a professional compared to 60% of Millennials and 65% of Boomers.
Race Affects Attitudes
As seen in previous research conducted by ThinkNow, Asian Americans lag other groups when it comes to awareness of mental health issues. Twenty-four percent of Asian Americans believe that having a mental health issue is a sign of weakness compared to the 16% average for all groups. Asians are also considerably less likely to be aware of mental health services in their communities (42% vs. 55%) and most likely to seek out information on social media (51% vs. 35%).
