11. Tratamiento
BLOQUEADORES BETA
ofrecen alivio rápido de los síntomas adrenérgicos de hipertiroidismo, como
temblor, palpitaciones, intolerancia al calor, y el nerviosismo.
YODUROS
bloquean la conversión periférica de tiroxina (T4) en triyodotironina (T3) e inhiben la
liberación de hormonas. Yoduros también se utilizan como terapia adyuvante antes de la
cirugía de emergencia no tiroideas
ANTITIROIDEOS
actúan principalmente al interferir con la organificación del yodo, por lo tanto la
supresión de los niveles de hormona tiroidea.
YODO RADIOACTIVO
es el tratamiento de elección para la mayoría de los pacientes con enfermedad de Graves y
bocio nodular tóxico.
12. CIRUGÍA
Una tiroidectomía subtotal se realiza con mayor frecuencia. Esta cirugía se conserva
parte del tejido de la tiroides y reduce la incidencia de hipotiroidismo a 25 por
ciento, pero hipertiroidismo persistente o recurrente se produce en un 8 por ciento de
los pacientes.
Tiroidectomía total se reserva para pacientes con enfermedad severa o grandes bocios en
los que las recurrencias se muy problemático, sino que lleva a un mayor riesgo de
hiperparatiroidismo y el daño del nervio laríngeo.
NUEVAS POSIBILIDADES
Nuevas opciones de tratamiento bajo investigación incluyen la tiroidectomía subtotal
endoscópica , la embolización de las arterias tiroides , la plasmaféresis, y la inyección
percutánea de etanol de tóxicos nódulos de la tiroides. (5)
14. Abstract
Background: Structural details of vertebrate extraocular muscles (EOMs)
have shown an anatomically and functionally distinct laminar organization
into an outer orbital (OL) and an inner global layer (GL). Since
hyperthyroidism alters tissue oxidative metabolism through mitochondrial
enzymes, it is expected that structural/mitochondrial changes may be seen in
hyperthyroid EOMs. We investigated the alterations in the laminar
organization and mitochondrial changes in hyperthyroid mouse EOMs.
Methods: Hyperthyroidism was induced in C57BL/6 mice and fresh rectus
muscles were obtained to identify functional mitochondria using
MitoTracker® Green and confocal microscopy; frozen sections from rectus
muscles were stained with anti-rabbit Troponin T (selectively present in the
OL) to demonstrate changes in the OL and GL of the EOMs. Ultrastructural
features of EOMs were studied using transmission electron microscopy
(TEM).
15. RESULTS: Of all four rectus EOMs studied, the maximum change was seen
in the inferior rectus muscle (IR) followed by medial rectus (MR). Myofiber
cross-sectional area measurements and Troponin T staining in the control
IR EOMs demonstrated a smaller OL (113.2 ± 3.66 μm(2)) and higher density
staining with Troponin T (90%) and a larger GL (411 ± 13.84 μm(2)) with low
intensity staining (10%), while hyperthyroidism resulted in an increased
OL (205.9 ± 5.3 μm(2)) and decreased GL (271.7 ± 7.5 μm(2)) p = 0.001.
Confocal microscopy demonstrated an intense staining especially in the
outer rims in the hyperthyroid IR which was confirmed by TEM showing
structural alterations in the mitochondria and a subsarcolemmal
migration. Conclusions: The outer, thinner, OL of the mouse EOM
contains smaller diameter myofibers and fewer mitochondria while the
inner, larger GL contains larger diameter myofibers and larger density of
mitochondria. Hyperthyroidism results in a significant alteration in the
laminar organization and mitochondrial alterations of mouse EOMs.

Este articulo es interesante ya que nos habla de la introducción del
hipertiroidismo en ratones con músculos rectos frescos para poder identificar
las mitocondrias funcionales utilizando MitoTracker ® Green y microscopía
confocal, los cortes congelados de los músculos rectos se tiñeron con T anti-
conejo de troponina (selectivamente en la OC) para poder demostrar cambios
en el OC y GL de , las misiones de observación electoral, De las cuatro misiones
de observación electoral de los rectos estudiados mostraron el cambio
máximo el músculo recto inferior (IR), seguido del recto medial (RM).
18. Abstract
A 63-year-old female was admitted to our hospital with a tender abdominal wall
mass about 15 cm in diameter, which she had for 1 month. About 1 week earlier, the
patient had also perceived a mass in the neck area. Computed tomography revealed
huge thyroid and periumbilical masses. The thyroid hormone levels were
consistent with a hyperthyroid state. Pathological examination of the thyroid mass
was compatible with anaplastic thyroid carcinoma (ATC) and the abdominal
cutaneous mass was shown to be metastatic ATC. Despite palliative radiotherapy
and chemotherapy, the patient died of respiratory failure on her 63rd day of
hospitalization. This case demonstrates that abdominal cutaneous metastasis and
hyperthyroidism can occur as initial manifestations of ATC. To our knowledge, this
is the first reported case.

El articulo nos habla de el primer caso reportado por la metástasis
cutánea abdominal y el hipertiroidismo ocurrido por la manifestación
inicial de la ATC , en una mujer de 63 años de edad, que fue admitida
en un hospital con una masa blanda de la pared abdominal de unos 15
cm de diámetro, que tenía por 1 mes. Alrededor de 1 semana antes, el
paciente había percibido también una misa en la zona del cuello. La
tomografía computarizada reveló enormes masas de tiroides y
periumbilical. Los niveles de hormonas tiroideas eran compatibles con
un estado de hipertiroidismo.
