How Race, Age and Gender Shape Attitudes Towards Mental HealthThinkNow
Mental health has been in the news quite a bit lately. Dozens of U.S. states are currently suing Meta for contributing to the youth mental health crisis by inserting addictive features into their products, while the U.S. Surgeon General is touring the nation to bring awareness to the growing epidemic of loneliness and isolation. The country has endured periods of low national morale, such as in the 1970s when high inflation and the energy crisis worsened public sentiment following the Vietnam War. The current mood, however, feels different. Gallup recently reported that national mental health is at an all-time low, with few bright spots to lift spirits.
To better understand how Americans are feeling and their attitudes towards mental health in general, ThinkNow conducted a nationally representative quantitative survey of 1,500 respondents and found some interesting differences among ethnic, age and gender groups.
Technology
For example, 52% agree that technology and social media have a negative impact on mental health, but when broken out by race, 61% of Whites felt technology had a negative effect, and only 48% of Hispanics thought it did.
While technology has helped us keep in touch with friends and family in faraway places, it appears to have degraded our ability to connect in person. Staying connected online is a double-edged sword since the same news feed that brings us pictures of the grandkids and fluffy kittens also feeds us news about the wars in Israel and Ukraine, the dysfunction in Washington, the latest mass shooting and the climate crisis.
Hispanics may have a built-in defense against the isolation technology breeds, owing to their large, multigenerational households, strong social support systems, and tendency to use social media to stay connected with relatives abroad.
Age and Gender
When asked how individuals rate their mental health, men rate it higher than women by 11 percentage points, and Baby Boomers rank it highest at 83%, saying it’s good or excellent vs. 57% of Gen Z saying the same.
Gen Z spends the most amount of time on social media, so the notion that social media negatively affects mental health appears to be correlated. Unfortunately, Gen Z is also the generation that’s least comfortable discussing mental health concerns with healthcare professionals. Only 40% of them state they’re comfortable discussing their issues with a professional compared to 60% of Millennials and 65% of Boomers.
Race Affects Attitudes
As seen in previous research conducted by ThinkNow, Asian Americans lag other groups when it comes to awareness of mental health issues. Twenty-four percent of Asian Americans believe that having a mental health issue is a sign of weakness compared to the 16% average for all groups. Asians are also considerably less likely to be aware of mental health services in their communities (42% vs. 55%) and most likely to seek out information on social media (51% vs. 35%).

2. La evaluación y tratamiento inicial en trauma incluye:
Ø Preparación.
Ø Triage.
Ø Valoración primaria (ABCDE) con reanimación inmediata de lesiones que amenazan la vida.
Ø Adjuntos a la valoración primaria y resucitación.
Ø El examen de la necesidad de traslado de pacientes.
Ø Evaluación secundaria (de la cabeza a los pies).
Ø Monitoreo continuado posterior a la revaluación.
Ø Tratamiento definitivo.
Durante el taller, el profesor, presento el siguiente caso clínico:
Paciente masculino de 32 años quien sufre un accidente automovilístico (era el conductor).
Presenta dificultad respiratoria y dolor torácico. Se encuentra inquieto, pide que le salven la vida.
Al examen físico: pálido, sudoroso, frío. Fc: 140 lpm; Fr: 40 rpm; Pa: 80/50 mmHg.
**Pálido, sudoroso, frío à shock hasta que se demuestre lo contrario.
ABCD se puede evaluar rápidamente (menos de 10 segundos) mediante
la identificación de sí mismos, preguntando el nombre a los pacientes y
lo sucedido. Una respuesta apropiada sugiere que no hay compromiso
importante de las vías respiratorias (es decir, la capacidad de hablar
claramente), la respiración no se ve gravemente comprometida (es decir,
la capacidad para generar movimiento de aire para permitir el habla) y el
nivel de conciencia no se redujo notablemente (es decir, está lo
suficientemente alerta para describir lo que pasó). La falta de respuesta
a estas preguntas sugiere anormalidades en A, B, C o D que requieren
evaluación y gestión urgente.
A, airway
• Comenzamos con la vía aérea porque causa la muerte más rápido. La hipoxia, si no mata al
paciente le puede ocasionar una morbilidad importante.
• La hipoxia también puede causar agitación psicomotora. En el caso de nuestro paciente, ponerle
O2 suplementario es una opción viable frente a su estado de agitación, probablemente debido a
hipoxia cerebral.
• Máscara facial a 10 l/min, para lograr saturación arriba del 95%.
• Permeabilizar la vía aérea mediante reposición de la cabeza.
o No mover la cabeza hacia atrás bruscamente, puedo causar daño medular. (Todo
paciente de trauma, se considera con lesión en columna cervical hasta demostrarse lo
contrario).
o Es necesaria la protección cervical al momento de realizar este paso
o Movilizar la mandíbula hacia adelante, traccionar sujetando el ángulo de la mandíbula.
Esto empuja hacia adelante la lengua, que en la mayoría de las veces es la que obstruye
la vía.
o Con una cánula de Berman podemos mantener la lengua en posición que no obstruya la
vía aérea.
3. • Asegurar la vía aérea: colocar tubo endotraqueal, con balón inflado por debajo de las cuerdas
vocales. En niños puede no tener que inflarse el balón, porque el diámetro traqueal es menor y
el tubo se ajusta adecuadamente.
• Todo paciente en trauma debe tener: canalización, O2 suplementario, Monitor.
• En caso de una urgencia o que no pueda colocar un tubo à Cricolaringotomía, se hace a nivel de
la membrana cricotiroidea.
• Es necesario aspirar secreciones o sangre si obstruyen las vías respiratorias.
• REEVALUAR
B, breathing (Ventilación).
• Examen físico del paciente: inspección, palpación, auscultación, percusión.
• La permeabilidad de las vías aéreas no asegura una ventilación adecuada, se requiere un
intercambio gaseoso adecuado para maximizar la oxigenación y la eliminación de CO2. Requiere
la función adecuada de pulmones, pared torácica y diafragma.
• Las lesiones que deterioran significativamente la ventilación en el corto plazo incluyen:
Neumotórax a tensión, hemotórax masivo, neumotórax abierto y lesiones traqueales o
bronquiales.
• Lesiones que comprometen la ventilación en menor grado, identificadas principalmente durante
la evaluación secundaria incluyen: neumotórax simple, hemotórax sencillo, fracturas costales,
tórax inestable y contusión pulmonar.
• Neumotórax a tensión: se produce compresión del mediastino, se comprime la vena cava y
disminuye el retorno venoso, cayendo así el gasto cardíaco y llevando a shock obstructivo.
o Hay compresión de la vena cava superior que se resuelve al ponerle un catéter en el 5to
espacio intercostal entre la línea axilar media y la línea axilar anterior. El catéter entre
más grande es mejor, suele usarse uno 14 Fr. Con esta acción volvemos el neumotórax a
tensión en uno simple.
o El shock es lo que mata a un paciente con neumotórax a tensión.
• Después se debe poner un tubo pleural (recordar hacer una radiografía de tórax luego de este
procedimiento)
• REEVALUACIÓN.
C, circulación
4. • Examen físico: llenado capilar (normal es < 2 segundos), pálido, sudoroso, frío.
• El volumen sanguíneo, el gasto cardíaco y el sangrado son los aspectos más importantes a
considerar.
• Las hemorragias son la causa predominante de muerte prevenible luego de una lesión.
• Una vez se ha descartado un neumotórax a tensión como causa del choque, considere que la
hipotensión es ocasionada por la pérdida de sangre hasta que se demuestre lo contrario.
• Se debe identificar la fuente de sangrado como externa o interna.
• Se puede obtener información importante en segundos evaluando:
ü Nivel de consciencia: cuando se reduce la volemia se puede afectar la perfusión
cerebral.
ü Perfusión de la piel: pacientes con piel facial y de extremidades de color rosa, rara
vez tienen hipovolemia crítica.
ü Pulso: un pulso filiforme y rápido es señal típica de hipovolemia.
La pérdida de sangre externa se puede detener con presión manual sobre la herida. Los torniquetes son
eficaces en el desangramiento masivo de una extremidad, pero conllevan el riesgo de lesión isquémica y
síndrome compartamental. Se usa torniquete sólo cuando la presión directa no es eficaz y la vida del
paciente está en peligro.
• Pasar líquidos RÁPIDO, la rapidez es:
o Directamente proporcional al diámetro del catéter
o Inversamente proporcional a la longitud del catéter
• Se pasan cristaloides, mediante accesos venosos periféricos y no catéter venoso central (muy
largos, bajo flujo).
o En proporción 3:1 dependiendo de la gravedad del shock.
o También puede ser en bolos de 500 cc.
• Se transfunde sangre también.
• En caso de que no mejore con líquidos, buscar hemorragias.
Las hemorragias internas se pueden buscar con FAST (Focused Assesment with Sonography in Trauma) à
ve si hay o no líquido. Si hay líquido es sangre hasta que se demuestre lo contrario. FAST evalúa: 1.
Pericardio 2. Cavidad esplenorrenal 3. Cavidad hepatorrenal 4. Cul-de-sac.
Las principales áreas de hemorragia interna son tórax, abdomen, retroperitoneo, pelvis y huesos largos.
Diuresis, se debe medir. Se debe colocar sonda urinaria.
o REEVALUAR
D, déficit neurológico
Una evaluación rápida que establezca el nivel de conciencia y el tamaño pupilar, así como la reacción del
paciente. Permite identificar signos de lateralización y determina el nivel de lesión de la médula espinal
de estar presente. Una disminución del nivel de conciencia puede indicar disminución de la oxigenación
y/o perfusión cerebral o puede ser causado por lesión cerebral directa. Amerita reevaluar oxigenación,
ventilación y perfusión del paciente. Hasta que se demuestre lo contrario, se debe suponer que los
cambios en el nivel de conciencia son el resultado de una lesión en el SNC.
• Escala de Glasgow: motor verbal, ocular.
o Buscar la mejor respuesta
5. • Que el paciente hable representa:
Ø A, vía aérea permeable
Ø B, buena reserva de oxígeno
Ø C, adecuada perfusión cerebral
Ø D, conversa
• REEVALUAR
La lesión cerebral puede no ser evidente en el momento de la evaluación inicial, por ende, es crucial
repetir el examen.
E, exposición
Durante la valoración primaria, quite toda la ropa del paciente, por lo general, cortando sus prendas para
facilitar un examen completo y evaluación. Posteriormente se debe cubrir al paciente con mantas
calientes o dispositivos de calentamiento externo para evitar el desarrollo de hipotermia. No se deben
infundir líquidos a temperaturas muy bajas, de ser necesario se puede calentar con microondas o
dispositivos calentadores de líquidos de infusión.
REEVALUAR
