Aproximadamente un 65% de los infectados
viven en la Región de Asia Sudoriental, el 30%
en la Región de África, y los demás en otras
zonas tropicales. La filariasis linfática es
transmitida por diferentes tipos de mosquitos,
entre ellos: Culex, que está muy extendido en
las zonas urbanas y semiurbanas; Anopheles,
que está presente principalmente en las zonas
rurales, y Aedes, que predomina en las islas
endémicas del Pacífico.

26. Caso clinico
Paciente masculino de 45 años, sin antecedentes mórbidos conocidos.
Con historia de 7 años de evolución de edema, calor y dolor en la rodilla
derecha, el cual involucró durante el transcurso de varios meses, todo el
miembro inferior derecho y el escroto. Durante 4 años, el paciente tuvo
múltiples evaluaciones médicas, sin poder definirse el diagnóstico ni
lograr mejoría clínica. Fue evaluado durante el periodo 2008- 2009, por el
Servicio de Cirugía Vascular Periférica, en donde se le diagnosticó
“posible linfedema”. Se le prescribió doxiciclina 100mg/día por 3 meses,
sin cambios clínicos evidentes.

27. Estudios de Gabinete
●Un año después fue valorado por el servicio de Infectología, que sugirió el diagnóstico de elefantiasis,
secundaria a filariasis. Las pruebas diagnósticas por filariasis fueron negativas y se le indicó amoxicilina 1
gramo/día, por 6 meses. Posteriormente fue valorado por el servicio de CPRHM. Se solicitó un US
escrotal, que describió: grosor de la pared escrotal aumentada difusamente, llegando a medir 7cm, así
como un diámetro del testículo derecho de 4,2 x 1,9 cm y de 3,8 x 1,7 cm en el testículo izquierdo, sin
alteraciones en el epidídimo, ni hidrocele asociado.
US de abdomen
US Doppler ----- No hubo reportes de trombosis venosa profunda.
Linfocintigrafia: obstrucción drenaje linfático en miembros inferiores, predominio
derecho, permeabilidad del conducto torácico.
TAC: reportó líquido, por debajo de la superficie escrotal bilateral, sin otros hallazgos
patológicos.

28. Se le realizó escrotectomía más reconstrucción del
pene y del escroto, con extirpación del 95% de la piel
escrotal, sin incidir en la fascia de Dartos o fascia
espermática externa, ya que los testículos no
presentaban ninguna afección.
Se realizó cierre primario con el 5% de piel posterior no
afectada (técnica de Homans), con colgajos
fasciocutáneos locales e injerto cutáneo de espesor
parcial. El reporte anatomopatológico mostró una pieza
quirúrgica de 8 kg, con dimensiones de 32 x 32 x 8 cm,
con edema generalizado e inflamación crónica, sin
presencia de filarias desde el punto de vista histológico.
El paciente tuvo controles postoperatorios cada
semana o cada dos semanas durante los
primeros 3 meses, reportando excelentes
resultados funcionales y buen patrón de
cicatrización. No hubo complicaciones
postoperatorias ni recidiva de la linfedema. El
paciente es sexualmente activo después del
procedimiento quirúrgico
