2. La secuencia de Pierre Robin es una
malformación congénita presentada al
nacer caracterizada por la presencia de
una triada de anomalías;
micrognatia, paladar hendido
y glosoptosis.

3. ANTECEDENTES
1923 Descrito por primera vez, por
Lannelongue y Menard, en 2 pacientes
con micrognatia, paladar hendido y
retroglosoptosis
Pierre Robín, como un síndrome
completo. En 1974 se denominó
síndrome de Pierre Robín a esta triada.

4. La prevalencia de este síndrome se ha estimado en 1 de cada 10.000
nacimientos. Sin embargo, es difícil obtener valores precisos porque la
definición ha variado históricamente y, con frecuencia, los estudios
incluyeron casos que ocurrieron como parte de un síndrome reconocido

6. Esta condición se conoce como secuencia
porque la fisura palatina posterior es un defecto
secundario asociado a un desarrollo
mandibular anómalo y glosoptosis
La hipoplasia mandibular, que se
produce en la etapa inicial de la
gestación, hace que la lengua se
mantenga alta en la cavidad bucal, lo
que impide la fusión normal de los
estantes palatinos (paladar
secundario).
El defecto de crecimiento mandibular puede ser la
consecuencia de una hipomovilidad orofacial prenatal
(generalmente relacionada con una disfunción del
tronco encefálico o de origen periférico) o de un
defecto de crecimiento primario de origen tisular
(hueso, tejido conectivo).

con disminución en la
proyección anteroposterior
de la mandíbula y su
consecuente aparición
retrognática clásica. (91.7%)
Además hay retrogenia o el
desplazamiento posterior de
la barbilla

de la lengua.
Es el segundo rasgo característico.
La posición de la lengua está
determinada en gran medida
por el tamaño y la orientación
de la mandíbula.

La secuencia de Pierre Robin se
asocia comúnmente a una
amplio paladar hendido en
forma de U, pero esto no es
esencial para el diagnóstico.

Alimentacion
Puede suscitar una secuencia de eventos:
glosoptisis, obstrucción de la vía aérea, llanto,
disminución de la ingesta oral y por tanto
retraso del crecimiento y desarrollo

El mentón es desplazado posteriormente causando la caída de la
lengua hacia la región posterior de la pared faríngea. Esto genera la
obstrucción de la vía aérea durante la inspiración.
-La secuencia de Pierre-Robin generalmente se diagnostica al nacimiento en base a la presentación
clínica. Cada vez más se evoca con mayor frecuencia en la etapa prenatal

La mayoría de las veces, los problemas respiratorios y
de alimentación se resuelven durante el primer año
de vida.
El crecimiento de la mandíbula favorece la
corrección de la retrognatia y de la posición de la
lengua durante los primeros años.
En casos de disostosis mandibulofacial, puede ser
necesaria la cirugía mandíbular.
La fisura palatina se corrige mediante una
intervención quirúrgica (en una o dos fases) antes del
año de edad.
Manejo

Las alteraciones sistémicas se presentan en un
10 % a 85 % de los casos reportados, e
incluyen defectos congénitos oculares,
auditivos, cardiovasculares, musculo
esqueléticos, neurológicos y
genitoutinarios.
Por lo que durante el seguimiento clínico del caso
se evaluará su desarrollo psicomotor acorde
a la edad y se realizarán estudios
imagenológicos renales y de potenciales
evocados visuales y auditivos.

Si un bebé con síndrome
de Pierre Robin tiene
dificultad respiratoria,
gírelo boca abajo (decúbito
prono). En esta posición la
lengua puede moverse
hacia adelante y abrir la vía
aérea.

Si la posición boca abajo
no soluciona el problema,
inserte un tubo
endotraqueal pequeño (2.5
mm) a través de la nariz,
con la punta colocada
profundamente en la
faringe posterior, pasando
la base de la lengua y por
encima de las cuerdas
vocales.

Si el bebé tiene una dificultad respiratoria grave y requiere
reanimación, la ventilación con máscara facial y la
intubación endotraqueal pueden ser muy difíciles. Si
ninguno de estos procedimientos previos resulta en un
movimiento adecuado del aire y los intentos de ventilación
con máscara facial e intubación endotraqueal no son
exitosos, la máscara laríngea
puede ser una vía aérea de rescate que puede salvar la vida.
