5 principales causas de muerte en politraumatizado:
4. Tórax inestable
Ocurre cuando se fracturan tres o más costillas en
dos o más sitios. El movimiento paradójico de este
segmento de la pared del tórax que flota
libremente es suficiente para comprometer la
mecánica de la ventilación. Además es de
importancia tener presente que los pacientes con
tórax inestable suelen cursar con contusión
pulmonar subyacente.

5. PATOLOGÍAS QUE COMPROMETEN LA VENTILACIÓN
Neumotórax abierto
Presencia de aire en el espacio pleural.
Cuando existe una disrupción que altere la
hermeticidad de dicho espacio y condicione una
comunicación entre esta y el ambiente ocurre
entonces el paso de aire hacia la misma,
aumentando el volumen del espacio a expensas
de aire y colapsando el pulmón del lado de la
lesión.
6. PATOLOGÍAS QUE COMPROMETEN LA VENTILACIÓN
Neumotórax abierto
Dolor torácico, disnea, taquipnea,
traumatopnea, enfisema subcutáneo,
disminución o ausencia de ruidos respiratorios
del lado afectado.
• RUTINA RADIOGRAFÍA DE TÓRAX O E-FAST.
• NO DUDE COLOCAR UN TUBO DE TORAX EN EL PACIENTE CON
SIGNOS CLAROS DE NEUMOTÓRAX.
• EN UN PACIENTE INESTABLE, SI USTED TIENE SOSPECHA DE
NEUMOTORAX, COLOQUE EL TUBO.
7. PATOLOGÍAS QUE COMPROMETEN LA VENTILACIÓN
Neumotórax a tensión
La presencia de aire en el espacio pleural es
tanta que colapsa totalmente el pulmón
afectado, comprime y desplaza el mediastino
hacia el lado contralateral, comprometiendo la
posibilidad de respuesta ventilatoria por parte
del pulmón sano, y afectando el retorno
venoso, por lo que SIEMPRE se acompaña de
repercusión hemodinámica.
8. PATOLOGÍAS QUE COMPROMETEN LA VENTILACIÓN
Neumotórax a tensión
Clínicamente el paciente presenta:
• TAQUICARDIA
• TAQUIPNEA
• SUDORACIÓN
• HIPOTENSIÓN
• INGURGITACIÓN YUGULAR
• TORAX AFECTO HIPERRESONANTE
• MURMULLO VESICULAR ABOLIDO

Hemotórax
Presencia de sangre en el espacio
pleural. Puede verse en trauma
cerrado o penetrante.

Contusión Pulmonar
Consiste en la lesión del
parénquima pulmonar por trauma
cerrado o penetrante, se presenta
como hemorragia pulmonar severa
y se caracteriza por una alteración
histológica denominada
HEPATIZACIÓN PULMONAR
Se observa hipoxemia moderada a severa con
normocapnia y pH normal. La relación PAO2/FiO2 es de
gran ayuda en la valoración inicial y monitoreo de la
oxigenación.

Contusión Pulmonar
EFECTO MACKLIN
Ruptura alveolar con disección del aire a lo largo
de las vainas broncovasculares hacia el
mediastino.
La traducción clínica del dicho efecto se
evidencia ante la presencia de neumo-mediastino
en la radiología torácica.
LA PRESENCIA DEL EFECTO MACKLIN
REFLEJA LA SEVERIDAD DEL TRAUMA.
Clínicamente, el paciente presenta:
Fracturas costales
Taquipnea
Taquicardia
Roncus o sibilantes
Hemoptisis (en ocasiones)
RAYOS X Y
TOMOGRAFÍA

14. MANEJO DE LA VENTILACIÓN
Todo paciente que ingrese a la unidad de
politraumatizado debe recibir medidas generales
como:
• Suministro de O2 húmedo por mascarilla con
reservorio a 10l/min o 12l/min.
• Posición de la cabeza y mandibula adecuada
(alineados). Con 30° de inclinación (espaldar de
la camilla).
• Cánulas orofaríngeas.
• Valoración del paciente según el esquema antes
propuesto.

Básico
Manejo de la ventilación
Intubación inmediata (endotraqueal o
nasotraqueal) o vía aérea quirúrgica
(cricotiroidotomía o traqueostomia) en:
• Paciente inconsciente
• Respiración dificultosa o sin respiración
espontánea
• ECG menor a 8
• Intoxicación por drogas ilícitas
• Consumo excesivo de alcohol
• Trauma maxilofacial severo
Colocación de mascarilla con 10l/min a
12l/min con O2 Húmedo en:
Paciente consciente (hablando)
Respirando espontáneamente
ECG Mayor a 8
Sin intoxicación por drogas ilícitas
Sin consumo excesivo de alcohol
Sin lesión maxilofacial importante

Glasgow menor a 8 puntos.
Trauma facial severo con compromiso de
huesos nasales y boca.
Secreciones abundantes o presencia de vómito
Tendencia a la somnolencia progresiva.
Presencia de saturación menor a 90% con
mascara de reservorio y 10l/min de O2.
Presencia de Tórax inestable y evidencia clínica
de limitación de la expansibilidad inspiratoria.
