En el marco de la Sexta Cumbre Ministerial Mundial sobre Seguridad del Paciente celebrada en Santiago de Chile en el mes de abril de 2024 se ha dado a conocer la primera Carta de Derechos de Seguridad de Paciente, a nivel mundial, a iniciativa de la Organización Mundial de la Salud (OMS).
Los objetivos del nuevo documento pasan por los siguientes aspectos clave: afirmar la seguridad del paciente como un derecho fundamental del paciente, para todos, en todas partes; identificar los derechos clave de seguridad del paciente que los trabajadores de salud y los líderes sanitarios deben defender para planificar, diseñar y prestar servicios de salud seguros; promover una cultura de seguridad, equidad, transparencia y rendición de cuentas dentro de los sistemas de salud; empoderar a los pacientes para que participen activamente en su propia atención como socios y para hacer valer su derecho a una atención segura; apoyar el desarrollo e implementación de políticas, procedimientos y mejores prácticas que fortalezcan la seguridad del paciente; y reconocer la seguridad del paciente como un componente integral del derecho a la salud; proporcionar orientación sobre la interacción entre el paciente y el sistema de salud en todo el espectro de servicios de salud, incluidos los cuidados de promoción, protección, prevención, curación, rehabilitación y paliativos; reconocer la importancia de involucrar y empoderar a las familias y los cuidadores en los procesos de atención médica y los sistemas de salud a nivel nacional, subnacional y comunitario.
Y ello porque la seguridad del paciente responde al primer principio fundamental de la atención sanitaria: “No hacer daño” (Primum non nocere). Y esto enlaza con la importancia de la prevención cuaternaria, pues cabe no olvidar que uno de los principales agentes de daño somos los propios profesionales sanitarios, por lo que hay que prevenirse del exceso de diagnóstico, tratamiento y prevención sanitaria.

La mayoría de los casos de listeriosis son esporádicos. A pesar de ello, los brotes de origen alimentario debidos a
L. monocytogenesse han asociado con queso, leche cruda (sin pasteurizar), embutidos, ensalada, pescado y pescado
ahumado, helado y completos (Swaminathan and Gerner-Smidt, 2007).
Para el caso de Chile, la listeriosis en una enfermedad de denuncia obligatoria en todo el territorio nacional y es
considerada objeto de vigilancia de laboratorio de acuerdo al Decreto Supremo Nº 158/04 (MINSAL, 2015).
Durante el año 2015, se notificó un total de 75 casos de listeriosis en 10 regiones del país, mayor a lo notificado el
año 2014 para el mismo período (n=52), con una letalidad de 26,6% correspondiente a casos no asociados al
embarazo (20 fallecidos). Del total, un 23,5% correspondieron a pacientes asociados al embarazo (embarazadas o
recién nacidos). De los casos restantes, el 37,3% correspondieron a adultos mayores de 60 años, 29,3% tenían
antecedentes de enfermedades crónicas o tratamiento de inmunosupresión y un 8,0% no presentaba un factor de
riesgo. La región Metropolitana concentró el 48% de los casos (n=36), seguido de la región de Valparaíso (9%;
n=7), ambas regiones, presentaron un alza con respecto al 2014 (MINSAL, 2015).
En el año 2017, se generó una alerta sanitaria debido a la detección de listeria en tres productos de la marca
Llanquihue (arrollado de cerdo, arrollado de huaso y pernil).
7. Ocurrencia en alimentos
L. monocytogenesha sido aislada de una amplia gama de matrices tales como: alimentos (queso, carne fermentada,
helado, leche cruda, carne cruda y cocida, verduras crudas, mariscos crudos y ahumados), aguas residuales, ensilaje
y material fecal (Osimani and Clementi, 2016).

Listeria monocytogenes es una bacteria gram positiva, anaerobia facultativa, de disposición cocobacilar,
intracelular, que posee un flagelo polar, no tiene cápsula y no forma esporas (Chanqueo et al., 2008). Pertenece al
género Listeria, encontrándose también L. ivanovii, L. innocua, L. welshimeri, L. selligeri y L. Grayi (FDA, 2012).
Sólo se considera patógena a L. Monocytogenes, la cual gerenera infección en humanos y animales y a L. ivanovii
que genera infección en rumiantes (Guillet et al., 2010). Recientemente, se han identificado cinco nuevas especies:
Listeria aquatica, Listeria floridensis, Listeria cornellensis, Listeria grandensis y Listeria riparia (Osimani and
Clementi, 2016). Se conocen trece serotipos de L. monocytogenes: 1/2a, 1/2b, 1/2c, 3a, 3b, 3c, 4a, 4ab, 4b, 4c, 4d,
4e y 7. Los serotipos más frecuentemente asociados con enfermedades humanas son 1/2a, 1/2b y 4b (FDA, 2012).
