Resumen
La prediabetes (hiperglucemia intermedia) es un estado de alto riesgo de diabetes que se define por variables glucémicas superiores a lo normal, pero inferiores a los umbrales de diabetes. Cada año, el 5-10 % de las personas con prediabetes al año evolucionará a diabetes, y la misma proporción volverá a la normoglucemia. La prevalencia de prediabetes está aumentando en todo el mundo, y los expertos han previsto que más de 470 millones de personas tendrán prediabetes en 2030. La prediabetes se asocia a la presencia simultánea de resistencia a la insulina y disfunción de las células beta, anomalías que comienzan antes de que se detecten cambios de la glucosa. Las pruebas observacionales muestran asociaciones entre la prediabetes y formas precoces de nefropatía, enfermedad renal crónica, neuropatía de fibras pequeñas, retinopatía diabética y aumento del riesgo de enfermedad macrovascular. Las puntuaciones de riesgo multifactoriales que utilizan medidas no invasivas y rasgos metabólicos sanguíneos, además de los valores de glucemia, pueden optimizar la estimación del riesgo de diabetes. En las personas prediabéticas, la modificación de los hábitos de vida es la piedra angular de la prevención de la diabetes, y hay pruebas de una reducción del riesgo relativo del 40-70%. Los datos acumulados también muestran los posibles efectos beneficiosos de la farmacoterapia.

El Programa de Desarrollo Clínico de IDegLira se diseñó sobre una amplia gama de datos clínicos obtenidos de los estudios de liraglutide (LEADTM) e insulina degludec (BEGIN).
DUAL I se diseñó para confirmar la superioridad de IDegLira vs liraglutide y la no inferioridad respecto a insulina degludec en términos de cambio en los niveles de HbA1c respecto al nivel basal en la semana 26 en pacientes que nunca habían recibido insulina.
DUAL II fue diseñado para confirmar la superioridad de IDegLira respecto a insulina degludec en términos del cambio de HbA1c, respecto al nivel basal a la semana 26 así como para evaluar la contribución de la fracción de liraglutide en la eficacia de IDegLira.
El objetivo de DUAL III fue medir los beneficios clínicos en lo concerniente a la reducción de HbA1c que los pacientes sin control adecuado usando agonistas del receptor GLP-1 lograron al cambiar a IDegLira.
DUAL IV estuvo diseñado para evaluar ya seguridad y la eficacia de IDegLira combinado con SU anticipando que este será el caso en una proporción significativa de pacientes con IDegLira.
El objetivo de DUAL V fue comparar la seguridad y eficacia de la titulación de IDegLira vs. insulina basal (insulina glargina) en pacientes con DM2.
DUAL VI se diseñó para comparar la eficacia y seguridad de dos algoritmos de titulación diferente de IDegLira en pacientes con DM2 mal controlados con uno o dos HOs.
DUAL VII estuvo diseñado para comparar la eficacia y seguridad de IDegLira después de 26 semanas de tratamiento respecto a el esquema de bolos de insulina basal en pacientes con DM2 mal controlados con insulina glargina U100 (20–50 U) y metformina.
DUAL IX se diseñó para comparar el control glucémico y la seguridad de IDegLira frente a insulina glargina U100 como tratamiento añadido a un inhibidor del cotransportador 2 de sodio-glucosa (SGLT2i).
DUAL VIII se concibió para comparar el control glucémico a largo plazo de insulina degludec /liraglutide (IDegLira) vs. insulina glargina (IGlar U100) en pacientes con diabetes tipo 2 que nunca recibieron insulina y no lograron control glucémico adecuado con hipoglucemiantes orales.
