These cytokines depress the secretion of adiponectin, which is
considered the most powerful insulin sensitizer, thus reducing insulin sensitivity of the tissues. Fatty infiltration of the pancreas decreases its capacity to secrete adequate amounts of insulin to withstand the high demand caused by insulin resistance. In abdominal obesity, a high level of fat infiltration of the omental adipocytes (usually devoid of fat) is present. Thus, an increased influx of portal fatty acids, hormones, and cytokines is seen by the liver. In response, there is a higher production of very low density lipoprotein (VLDL) and increased secretion of insulin and pancreatic polypeptides generating gluco- and lipotoxicity

with a BMI of 112 kg/m2 for an incision and drainage of the lower extremity for fasciitis, and a colleague recently intubated a 500-pound gravid woman for Cesarean section for twins due to failure of placement of an epidural.
