3. INTRODUCCIÓN
1. Las enfermedades de transmisión sexual (ETS) o infecciones de transmisión sexual (ITS) son
infecciones que se transmiten de una persona a otra a través del contacto sexual
2. El contacto suele ser vagina, pene, oral o anal, pero tambien pueden ser por contacto fisico.
3. Afectan mayormente a hombres y mujeres adultas.
4. Si una embarazada tiene una ETS, puede causar graves problemas de salud al bebé
4. SÍFILIS: EPIDEMIOLOGIA EN CHILE
CHILE
Jupiter is the biggest
planet of them all
It’s composed of
hydrogen and helium
5. SÍFILIS: AGENTE ETIOLOGICO
Dominio: Bacteria
Filo: Spirochaete
Clase: Spirochaetes
Orden: Spirochaetales
Familia: Spirochaetaceae
Género: Treponema
Especie: T. pallidum
• Es una espiroqueta muy fina, que
no puede observarse por tinción de
Gram
• T. pallidum no presenta factor de
virulencia alguno que induzca el
daño de los tejidos. Posee
lipopolisacárido (LPS), pero es
atóxico.
6. SÍFILIS: PATOLOGIA
• Si se diagnostica
la sífilis en este
período y se la
trata con
antibióticos
adecuadamente
la sífilis se cura
sin dejar
secuelas.
• Aún sin
tratamiento
antibiótico, la
lesión
involuciona
espontáneament
e y cicatriza en
un período de 3 a
8 semanas.
Período de
incubación
• Ingreso de la bacteria al
epitelio.
• El período de
inoculación varía según
el tamaño del inóculo,
que inicialmente es local
pero rápidamente
avanza con la aparición
en minutos en ganglios
linfáticos y diseminación
en pocas horas a todo el
organismo
Sífilis
primaria
• En la zona de la
inoculación se
produce una
induración.
• La zona afectada
adquiere el aspecto
ulcerado típico de la
lesión primaria o
chancro sifilítico.
• La lesión primaria es
muy rica en
treponemas
7. SÍFILIS: PATOLOGIA
ii) Lesiones erosivas de las
mucosas, en la cavidad bucal,
faringe, vagina y canal anal,
entre otros sitios.
Y iii) lesiones verrugosas
características (condilomatas)
en pliegues húmedos, que
aparecen especialmente en la
región perianal y genital
externa femenina y también
en zonas de pliegues cutáneos
en zonas con alta humedad
• Las lesiones cutáneas de la
sífilis secundaria incluyen:
i) una erupción diseminada,
caracterizada por la presencia de
lesiones maculosas en todo el
cuerpo, de 3 a 10 mm de
diámetro, inclusive en la planta
de los pies y palmas de las manos;
las lesiones son muy ricas en
treponemas, por lo que son
altamente contagiosas.
• Una vez que la lesión
primaria ha
involucionado, la
enfermedad atraviesa
por un período silencioso
de 3 a 8 semanas.
Sífilis
secundaria
• Si el paciente no es
tratado, tras una
evolución de varias
semanas los signos
remiten y la sífilis entra
en un período de
latencia
8. SÍFILIS: PATOLOGIA
• La ausencia de tratamiento
de la madre sifilítica es lo
que origina la infección del
feto
• Esta etapa (sífilis tardía) la
enfermedad es inflamatoria y
crónica, y muchos tejidos están
afectados. Una manifestación
común es la patología aórtica
debido a la vasculitis obliterativa.
La manifestación más común es
la presencia de lesiones típicas
(gomas), que pueden aparecer
en diversos órganos y están
asociados a la patología vascular
de la sífilis.
• La sífilis terciaria no es
contagiosa
• La sífilis latente se caracteriza
por la presencia de
anticuerpos séricos contra
antígenos treponémicos, sin
manifestaciones clínicas de la
enfermedad.
• La sífilis latente se caracteriza
por la presencia de
anticuerpos séricos contra
antígenos treponémicos, sin
manifestaciones clínicas de la
enfermedad.

11. SÍFILIS: DIAGNOSTICO
La prueba de sífilis generalmente involucra
dos pasos.
En la mayoría de los casos, es detectar
anticuerpos que están relacionados con la
infección.
En general, las pruebas de detección
primaria de sífilis incluyen:
Reagina plasmática rápida (RPR) una
prueba de sangre.
Pruebas del laboratorio de investigación
de enfermedades venéreas (VDRL, por su
sigla en inglés), la cual se puede hacer en
sangre o en el líquido cefalorraquídeo.
La segunda prueba busca anticuerpos que
el sistema inmunitario sólo produce para
combatir la sífilis, a diferencia de la primera.
Ensayo de aglutinación de partículas
Treponema pallidum (TP-PA)
Prueba de absorción de anticuerpos
treponémicos fluorescentes (FTA-ABS)
Ensayo de microhemaglutinación de
anticuerpos T. pallidum (MHA-TP)
Ensayo de hemaglutinación T. pallidum
(TPHA)
Inmunoensayo de enzimas T. pallidum
(TP-EIA)
Inmunoensayos de quimioluminiscencia
(CLIA)
12. SÍFILIS: CONTROL Y PREVENCIÓN
Para reducir el riesgo usar preservativo si
tienes relaciones sexuales.
Exámenes de
rutina para
personas
sexualmente
activas y
embarazadas
• VDRL (es una prueba no
Treponémica), que busca
detectar la infección por
Sífilis o Treponema
pallidum, una Infección de
Transmisión Sexual (ITS)
• Un examen No
treponémico Reactivo
(RPR) (mide anticuerpos
inespecíficos producidos
en infecciones supuesta la
material liso ideal liberado
desde las células dañadas
del huésped, así como
respuesta a material de
estructura proteica,
liberada desde los
treponemas)

15. GONORREA: AGENTE ETIOLOGICO
Dominio: Bacteria
Filo: Proteobacteria
Clase: Beta Proteobacteria
Orden: Neisseriales
Familia: Neisseriaceae
Género: Neisseria
Especie: N. gonorrhoeae
• Neisseria gonorrhoeae es una cocácea
gram negativa que se agrupa en pares
(diplococos) con lados adyacentes
aplanados, y apariencia de granos de
café. Es un microorganismo intracelular
facultativo, no móvil, no esporulado,
oxidasa y catalasa positiva.
• Neisseria gonorrhoeae se considera un
patógeno humano obligado que puede
crecer y multiplicarse en las mucosas
incluyendo el cérvix, útero, y trompas de
Falopio en las mujeres, así como en la
uretra masculina. Aunque también
puede encontrarse en la boca, faringe y
ano. Esta bacteria causa la gonorrea,
una infección altamente contagiosa,
casi exclusivamente de transmisión
sexual
16. GONORREA: PATOLOGIA
• Complicaciones en
el hombre,
abscesos
prostáticos,
epididimitis y por
consecuencia
infertilidad. En
mujeres,
enfermedad pélvica
inflamatoria y
subsecuentemente
esterilidad
• Los gonococos infectan
las mucosas de las vías
genitales, el recto y la
faringe, dependiendo de
las prácticas sexuales de
los individuos
• En el hombre uretritis y se
extiende hacia la uretra
• En la mujer infección
primaria en el endocérvix,
la vagina y puede
progresar hasta las
trompas uterinas y
producir salpingitis con
fibrosis.
• La Neisseria
gonorrhoeae
como patógeno
genitourinario
debe ser capaz
de colonizar la
superficie de la
mucosa del
tracto genita
• Proteína de superficie
Pilina (infección en el
huésped).
i. PI estructural, forma
poros para ingresar
nutrientes y serovariedad
(IA y IB)
ii. PII adherencia de los
gonococos, para su
fijación a las células del
hospedero
iii. PIII antigénico
• La toxicidad de las
infecciones gonocócicas se
debe en gran medida a los
efectos endotóxicos del
LPS
18. TRATAMIENTO
• Los pacientes que son alérgicos a las cefalosporinas, se
tratan con una de las siguientes:
1. Gemifloxacina 320 mg por vía oral, más azitromicina 2 g
por vía oral una vez.
2. Gentamicina 240 mg IM, más azitromicina 2 g por vía
oral una vez.
19. GONORREA: DIAGNOSTICO
La tinción de Gram es sensible y
específica para confirmar la
gonorrea en hombres con secreción
uretral y visualiza diplococos
intracelulares gramnegativos.
La tinción de Gram es mucho menos
precisa en presencia de infecciones
del cuello uterino, la faringe y el
recto y no se recomienda para el
diagnóstico de la enfermedad en
estos sitios.
.
Cultivos especiales, las muestras
obtenidas con un hisopo deben
prepararse rápidamente sobre una
placa en un medio apropiado (por
ejemplo Thayer-Martin modificado) y
transportarse al laboratorio en un
ambiente rico de dióxido de
carbono.
20. GONORREA: CONTROL Y PREVENCIÓN
Para reducir el riesgo de
gonorrea usar
preservativo si tienes
relaciones sexuales.
Limita el número de
parejas sexuales
