healthy for your family. And you’re ready to quit. Follow
these 4 tips to help you stop using tobacco for good.
1 Pick a quit day and mark it on your calendar. Let
your friends and family know.
2 Throw away all cigarettes, ashtrays and lighters—
anything that reminds you of smoking.
3 Stock up on items that will distract your
hands and mouth. Carrots, gum, toothpicks
or cinnamon sticks are good options.

How to Stay Out
of the Hospital
Do you have an ongoing, or
chronic, illness? Maybe you
have diabetes? Or CHF?
COPD? Or asthma? If you do,
it’s especially important to stay
on top of your health. Be an
active part of your care so that
you can feel well and stay out
of the hospital.
- Work with your doctor.
Keep your regular
appointments with the
doctor. Your doctor will
track your symptoms and
your condition.
- Take your medication
properly. Don’t skip doses
or stop taking medicine
because you feel better.
Medication works best
when it’s taken regularly
and correctly.
- Understand your illness.
How do you manage it
every day? What warning
signs should you look for?
When should you call
your doctor? Knowing the
answers to these questions
will help you stay healthy.
MetroPlus Case Managers
may be able to help you stay
well. Call 1-866-986-0356
(TTY/TDD 711) to learn about
our Case Managers.
Cancer screenings are an important
part of your health care. Most
women should be screened for
breast cancer. Speak with your
doctor about what’s best for you.
Here are general guidelines:
Women over 40: Starting at age
40, women need a mammogram
every year. Women should also get
a breast exam yearly as part of their
annual health checkup.
MetroPlus can help you
schedule cancer screenings. Call
1-866-986-0356 (TTY/TDD 711).
Do You Need a Mammogram?
JUST FOR YOU
Members and Plan staff meet over lunch several times a year to work on
improving our health plan. If you would like to join the Member Advisory
Committee (MAC), call 1-800-303-9626.

Clear the Air
Your asthma can get worse if
your home is very dusty or
moldy. Follow these at-home
tips to help your asthma:
- Dust with a damp
cloth. Don’t use harsh
chemicals to clean.
- Don’t let people smoke
in your home. If you
smoke, quit.
- Keep pets out of
your bedroom.
- Use an air conditioner
instead of opening
up windows.
Your Asthma Action Plan
If you have asthma, then you need an asthma action plan. Your doctor can
help you create your plan.
Work closely with your doctor and keep all of your appointments. Your
doctor will begin by checking your asthma. You may need to breathe into a
“peak flow meter,” which tests how your lungs work. Your doctor may ask you
to track your symptoms.
Your asthma action plan should tell you what to do based on what your
symptoms are. Asthma action plans are often broken into three sections:
GREEN ZONE: You are breathing well. Your asthma is
under control. Take your long-term medication as you
normally do.
YELLOW ZONE: You are coughing or wheezing. You are
having trouble breathing. Take your quick-relief medicine.
RED ZONE: Your quick-relief medicine did not work.
You are coughing and wheezing more. Take the medicine
as your doctor told you to. If you aren’t better in 15
minutes, call your doctor.
Sleeping near your
pets could make your
symptoms worse.

Purifique el aire
El asma puede empeorar
si su hogar tiene mucho
polvo o moho. Siga estos
consejos en su hogar para
ayudar a tratar el asma:
- Limpie el polvo con un
trapo húmedo. No utilice
sustancias químicas agresivas
para limpiar.
- No permita que la gente
fume en su hogar. Si tiene el
hábito de fumar, déjelo.
- Mantenga a sus mascotas
fuera de la habitación.
- Utilice el aire acondicionado
en lugar de abrir las
ventanas.
ZONA VERDE: Usted respira bien.
resultado. Esta tosiendo y sibilando más. Tome su medicamento
tal como se lo indicó su médico. Si no mejora en 15 minutos,
llame a su médico.
Dormir cerca
de sus mascotas
puede empeorar
sus síntomas.

Los análisis de detección del cáncer
constituyen una parte importante de
su atención médica. La mayoría de las
mujeres debe realizarse un análisis
de detección del cáncer de mama.
Consulte a su médico acerca de qué
es lo mejor para usted. Aquí tiene
algunas pautas generales:
Mujeres mayores de 40: A
partir de los 40 años, las mujeres
deben realizarse una mamografía
por año. Las mujeres también
deberían hacerse un examen de
mamas anualmente como parte de
su control de salud anual.
MetroPlus puede ayudarla
a programar un análisis de detección
del cáncer. Llame al 1-866-986-0356
(TTY/TDD: 711).
¿Necesita una mamografía?

Comó mantenerse
alejado del hospital
¿Tiene alguna enfermedad en
curso o crónica? ¿Tal vez tiene
diabetes? ¿O insuficiencia cardíaca
crónica? ¿Enfermedad pulmonar
obstructiva crónica (EPOC)? ¿O
asma? Si es así, es muy importante
que controle su salud. Participe
activamente de su atención para
poder sentirse bien y manterse
alejado del hospital.
- Trabaje en conjunto con su
médico. Cumpla con sus citas
regulares con su médico. Su
médico puede dar seguimiento
a sus síntomas y a su afección.
- Tome sus medicamentos en
forma adecuada. No omita

medicamento porque se siente
mejor. El medicamento trabaja
mejor cuando se lo toma en
forma regular y correcta.
- Comprenda su enfermedad.
¿Cómo lidia con ella a diario?
¿A qué señales de advertencia
debe estar atento? ¿Cuándo
debería llamar a su médico?
Conocer las respuestas a
estas preguntas lo ayudará a
mantenerse sano.
