Este documento discute los límites del régimen acortado de tratamiento para la tuberculosis farmacorresistente y la importancia de realizar pruebas de sensibilidad a medicamentos antes del tratamiento. Analiza datos de cohortes en Europa y América Latina que muestran que menos del 12% de los pacientes con tuberculosis farmacorresistente son elegibles para el régimen acortado debido a la alta prevalencia de resistencia a medicamentos. Es importante conocer que la población mundial desde mediados de los 90 tiene una alta prevalencia de casos de HIV Y TBC. La coinfección ya sea en la era PRETARGA (antes del 2004) o posterior, ha demostrado a través de estudios randomizados ensombrecer el pronóstico de vida en ambos sentidos. Es por ello que se hace una revisión de este artículo publicado en el Clinical Infectious Diseases en agosto del 2014 , que toma como centro referencial a la India y compara 4 estudios de temporalidades distintas. Se concluye que la emergencia de resistencia a rifampicina está dada por la resistencia previa a isoniazida,a la seropositividad y a la carga bacilar en esputo. Estos resultados aplicables sólo en el contexto de TBC pulmonar. Aún queda pendiente saber si la terapia antituberculosa diaria vs la terapia intermitente tiene impacto en poblaciones seropositivas.

• Las cohortes estudiadas en Europa y América Latina

• Tienes que probar la sensibilidad ANTES!!
• Atención! no se puede prescribir ciegamente, basado
en datos epidemiológicos!!
• Gestión basada en la evidencia adecuada es necesaria
