• Campylobacter jejuni es el patógeno
entérico mas frecuente en los países
desarrollados
• Causa importante de diarrea del turista
• La mayoría de las infecciones se asocian
a la ingestión de pollo poco cocinado
• También puede deberse a leche no
pasteurizada o agua contaminada

Morfologia
• No se conoce con detalle
• Hay cuatro propiedades principales de la virulencia que contribuye a ella:
motilidad, adherencia, producción de toxinas e invasión.
• Los flagelos permiten que Campylobacter sea móvil, lo que facilita la
adherencia y la colonización.
• En algunos casos C. jejuni libera citotoxinas que provocan daño epitelial,
similar a la toxina del colera.
• La disentería se asocia a invasión y solo se presenta en una minoría de
cepas de Campylobacter.
• Microorganismo flagelado en forma de espiral o en “S”
• Los infiltrados de neutrófilos mucosos e intraepiteliales
prominentes
• Puede haber criptitis (infiltrado de neutrófilos en las
criptas) y abscesos en las criptas
• Es importante que la arquitectura de la cripta este
conservada.
6. Complicaciones
Manifestaciones Clinicas
• Puede provocar artritis reactiva
• Eritema nudoso
• Sx de Guillain-Barret (el 15%
presentan Ac frente a C. jejuni)
• La ingesta de tan solo 500 u de C. jejuni puede causar la
enfermedad después de un periodo incubación de hasta 8
días
• La diarrea acuosa aguda o después de un pródromo
pseudogripal, es el síntoma principal
• La disentería aparece en el 15% de los casos
• Los pacientes pueden diseminar las bacterias durante 1
mes o mas después de la resolución clínica
7. Shigelosis
● Shigella son bacilos gramnegativos
que no están encapsulados, son
inmóviles y se comportan como
anaerobios facultativos, responsables
de una de las causas más frecuentes
de diarrea sanguinolenta.
● Shigella se transmite con mucha
facilidad por vía fecal-oral o mediante
la ingesta de agua o alimentos
contaminados; la dosis infectiva es
inferior a 100 gérmenes y en cada
gramo de heces se encuentran hasta
109 gérmenes durante las fases
agudas de la enfermedad.
8. Epidemiologia
En EE. UU. y Europa, los niños que
acuden a guarderías, los trabajadores
inmigrantes, las personas que viajan a
países en vías de desarrollo y los
ancianos internados en residencias
son los que con más frecuencia se
afectan.
La mayor parte de las infecciones y
muertes por Shígella ocurren en niños
menores de 5 años; en los países en
los que este germen es endémico,
produce aproximadamente el 10% de
todos los casos de diarreas
pediátricas y hasta el 75% de las
muertes secundarias a diarrea

Shigella es resistente al duro entorno ácido
del estómago, lo que explica, en parte, la
dosis infectiva tan baja. Cuando llega al
intestino, el germen es captado por las
células epiteliales M (micropliegues), y la
captación de los antígenos luminales
Tras la proliferación intracelular, las
bacterias se escapan hacia la lámina
propia. Estas bacterias infectan el
epitelio del colon y del intestino
delgado atreves de las membranas
baso laterales, que expresan
receptores para las bacterias.
10. ● Las infecciones por Shlgella
son más llamativas en el
colon izquierdo, pero el
íleon también puede
resultar afectado, lo que
quizá refleja la abundancia
de células M en el epitelio
que reviste las placas de
Peyer.
● El aspecto histológico de los casos
precoces es similar al que se
encuentra en otras colitis agudas
autolimitadas. En los casos más
graves, la mucosa aparece
hemorrágica y ulcerada, y es posible
identificar seudomembranas. Dado el
tropismo por las células M, quizá
puedan encontrarse también úlceras
de aspecto aftoso parecidas a las
descritas en la enfermedad de Crohn.

Tras el período de incubación de 1-7 días,
la diarrea acuosa inicial evoluciona a una
fase disentérica en aproximadamente el
50% de los pacientes, y los síntomas
generales pueden persistir hasta durante 1
mes.

En una minoría de los adultos se produce un cuadro
subagudo. La antibioterapia acorta la duración del
cuadro clínico y reduce el tiempo durante el cual se
eliminan gérmenes por las heces, pero los fármacos
antidiarreicos están contraindicados porque pueden
prolongar los síntomas al retrasar la eliminación de las
bacterias.
Caracteristicas clinicas
Shigella provoca una enfermedad
autolimitada caracterizada por un
cuadro de unos 6 días de evolución
de diarrea, fiebre y dolor abdominal.
12. Escherichia coli
● Son bacilos gramnegativos que
colonizan el tubo digestivo. La mayoría
no son patógenos, pero un subgrupo
puede causar enfermedad en el ser
humano:
E. Coli enterotoxígena (ETEC)
E. Coli enteropatógena (EPEC)
E. Coli enterohemorrágica (EHEC)
E. Coli enteroinvasiva (EIEC)
E. Coli enteroagregante (EAEC).

Enterotoxigena
• Diarrea del viajero.
• Vía fecal-oral.
• Niños menores de 2 años son más
susceptibles.
• Producen dos toxinas: una termolábil
(TL) y otra termoestable (TE).
• Diarrea secretora no inflamatoria,
deshidratación y, en los casos más
graves, shock.
Enteropatogena
• Se caracterizan por su capacidad de
producir lesiones por unión y
borramiento.
• Prevalente en países desarrollados y
en vías de desarrollo.
• Diarrea endémica y brotes de
diarrea, sobre todo en niños menores
de 2 años.
enterohemorragica
• Se clasifican como serotipos
0157:H7 y no 0157:H7
• Las vacas son su reservorio natural
• La infección está asociada al
consumo de carne de ternera mal
cocinada, leche y verduras.
• Ambos serotipos producen toxinas
parecidas a Shiga.
• Diarrea sanguinolenta, colitis
isquémica y síndrome hemolítico
urémico en niños <2 años.
14. Enteroinvasiva
• Bacteriológicamente similares a
Shigella.
• Se transmiten a través de los
alimentos, el agua o el contacto
entre personas.
• No producen toxinas, invaden las
células epiteliales y causan un
cuadro inespecífico de colitis aguda
auto- limitada (diarrea
sanguinolenta)
• Frecuente en niños pequeños
Enteroagregante
• Se identificaron por su patrón
exclusivo de adherencia a las células
epiteliales
• Causa de diarrea en los niños y
adultos en los países desarrollados y
en desarrollo
• Elaboran TL y una toxina similar a
Shiga,
• Lesiones histológicas son mínimas
• Diarrea no sanguinolenta
