What is stroke?
A stroke occurs when the blood supply to a part of your brain is ¡nterrupted or
_ severely reduced and brain tissue is deprived of oxygen and nutrients. Within a

AVIAN FLU
1 Avian influenza s also known as bird f lu or avian f lu. It s a f lu due to a type of
influenza virus that ¡s hosted by birds, but may infect several species of
.mammals. It was f irst ¡dentif ied in Serbia Montenegro in the early 1900s and ¡s
now known to exist würldwidej A strain of the H5Nl-type of avian influenza virus
5 that emerged in 1997 has been ¡dentif ied as the most likely source of a future
influenza pandemia and ¡s known to have infected 137 people in Asia since 2003,
killing 70.
Wild fowl act as natural asymptomatic carriers of avian f lu virus. Prior to the
current H5N1 epizootic, strains of avian influenza virus have been demonstrated
10 to be transmitted from wild fowl to birds, pigs, horses, seáis, whoJes and
humans; and between humans and pigs and between humans and domestic fowl.
H5N1 has been shown to be also transmitted to tigers, leopards, and domestic
cats that were fed uncooked domestic fowl (chickens) with the virus
Avian influenza virus spreads in the air and ¡n manure and survives longer in cold
15 weather. It can also be transmitted by contaminated feed, water, equipment and
clothing; however, there is no evidence that the virus can survive in well-cooked
meat. The incubation period ¡s,3 to 5 days. Symptoms in animáis vary, but
virulent strains can cause death within a f ew days.
In humans, avian f lu viruses cause similar symptoms to other types of f lu. These
20 include fever, cough, sore throat, muscle oches, conjunctivitis and, in severe

cross the species barrier from birds to humans, the new subtype created could
be both highly contagious and highly lethal in humans. If a human infected with
influenza virus also acquires H5N1, a mutant strain of bird f lu that can be

Ultrasound-guided needle biopsy of primary bone tumours

1 Needle biopsy is an established technlque for the histological diagnosis of bone tumours, usually guided by
fluoroscopy or CT. Surface lesions and aggressive tumours which have extended through the cortex are
also amenable to imaging with ultrasound (US). We have assessed the dlagnostic accuracy of US-gulded
Trucut needle biopsy in a consecutive series of patients referred to a Bone Tumour Unit wlth suspected
5 primary bone tumours. Of 144 patients (83 men, 61 women; mean age 34.7 years) referred over a period of
two years, 63 were considered suitable for US-gu¡ded biopsy. This was based on the presence of a relatively
large extraosseous component, seen typicaily ¡n osteosarcoma and malignan! round-cell tumours. The
resulte of needle biopsy were compared with those of surgical biopsy. The diagnostic accuracy was 98.4%,
with only a single failed biopsy.
10 Thus, in a selected group of patients, US ¡s a very reliable technique of guidance for percutaneous needle
biopsy of bone tumours.

Patients and Methods
Between November 1995 and November 1997, a record was kept of all patients who had needle biopsy afíer
referral by the surgeons of the London Bone and Soft-tissue Tumour Unit. There were 144 patients (83 men,
61 women) with a mean age of 34.7 years (2 to 81 years) and all had a suspected primary bone tumour of
15 the appendicular skeleton, pelvis or sacrum. Before biopsy, they had plain radiography and MRI of the
lesión. Based on these results a decisión was made by the radiologist to biopsy the lesión using either
fluoroscopic, CT or US guidance. [...] Since the aim of the study was to determine when US could
appropriately replace fluoroscopy as a guidance technique, the 29 patients biopsied using CT were not
considered further. All biopsies were made through a small stab incisión. This left a scar which allowed
20 ¡dentification of the site for excisión of the biopsy track at the time of definitive surgery.
Patients over the age of 16 years had local anaesthesia (3 to 5 mi 1% lignocaine) to the skin and
subcutaneous tissues. All adults were treated as outpatients, unless they had been admitted for other
reasons. The 35 patients aged under 16 years were admitted as day cases and biopsied under general
anaesthesia. Between two and four passes were made depending on the quality of the specimens obtained.

25 This allowed sufficient tissue for any special techniques required by the pathologist. [...]
The unfixed samples were taken direct to the Department of Histopathology for imprint cytology. This has
several advantages in that it gives a rapid assessment of the presence of sufficient viable tissue, and in most
cases, can immediately distinguish between benign and malignant lesions. With the latter it can differentiate
between primary tumours, metastases and myeloma. The use of an alkaline phosphatase stain allows a
30 diagnosis of osteosarcoma to be made when little or no tumour osteoid ¡s present in the sample. Stored
imprints can be used for more sophisticated techniques including cytogenetics. [...]
Discussion
Biopsy is an essential investigaron for all suspected primary bone tumours after local staging. It may be
carried out either as an open operation or as a closed percutaneous procedure by an orthopaedic surgeon
or a radiologist. It must, however, be undertaken in the centre where the definitive management of the
35 patient will take place. A biopsy which has been inadequately obtained remains the commonest reason for
inability to perform limb-salvage surgery.
The major advantage of open biopsy is that a large amount of tissue can be obtained. Without radiological
guidance, however, it is possible that this tissue may be either mainly necrotic or from a low-grade área of a
lesión which has undergone dedifferentiatlon. Other disadvantages of open biopsy include the need for
40 general anaesthesia and increased cosí compared with needle biopsy which is carried out as an outpatient

procedure under local anaesthesia. It is also associated with a higher rale of complications. Percutaneous
needle biopsy has been criticised because of the small amount of tissue obtained, which, it is suggested,
may result ¡n sampling errors. The combination, however, of a needle biopsy and sections of good quality,
reliable immunohistochemistry and pathologists experienced ¡n making diagnoses from specimens of needle
45 biopsy can result in a diagnostic accuracy as high as 97%. Proper placement of the biopsy allows the track
to be excised at the time of surgery. Needle biopsy also preserves the periosteum and is made through a
small stab incisión which leaves a scar visible to the surgeon at the time of surgery. This allows identification
of the biopsy site.

1 After years of research, analysis, and debate, the International Agency for
Research on Cáncer, the U.N.'s cáncer research agency, has reclassified HRT
from "possibly carcinogenic" to "carcinogenic."
The panel of 21 scientists concluded that the evidence from several recent
.5 studies was consistent and reliable enough to link HRT to breast cáncer thereby
fully justifying the reclassification. The normal risk of a woman (not on HRT)
developing breast cáncer ¡s slightly more than 14%. Long-term use of HRT
raises that risk to almost 17%. The panel also concluded that HRT slightly
¡ncreases the risk of endometrial cáncer when progestin is taken fewer than 10
10 times per month. While most experts agree with tríese conclusions and see no
real departure from what was already generally accepted with respect to HRT,
the panels finding that a common form of birth control pill was linked to a
slightly increased risk of certain cancers was met with skepticism by many
doctors.
15 HRT has been a long-standing and widely accepted treatment for women
experiencing some of the more uncomfortable effects of menopause. In the
past few years, however, this popular therapy has come under fire as a result
of evidence linking HRT to many serious health risks. A number of studies
suggest that women undergoing HRT are exposing themselves to long-term
20 harm that far outweighs the severity of the symptoms being treated.
For some women, HRT might consist of an estrogen-only treatment while
others are given a combination of estrogen and progestin. This latter option is
slightly less common but ¡s still a popular solution to menopausal discomfort.
Both types of HRT are used by an estimated 13.5 million women in the United
25 States alone.HRT has been around since the early 1940s when women began
taking high doses of estrogen to counteract many of the temporary, but
recurring, discomforts of menopause. In the 1970s, however, it was discovered
that this particular form of estrogen therapy created an unacceptably high risk
of uterine cáncer. This prompted the trend by doctors to prescribe progestin
30 along with significantly lower doses of estrogen.
Recent studies now show that there are too many health risks associated with
HRT to justify its being used automatically as a "cookie cutter" treatment of
choice or as ¡f it were a woman's only available option.Women should consult
their physicians to discover whether HRT would be a benefit or a detriment
35 given their own particular circumstances. While HRT undoubtedly alleviates a
number of menopausal symptoms in many women, for some, the cost of that

Prempro and Premarin:
Prempro and Premarin are the two major HRT drugs. Both are manufactured by
40 Wyeth Pharmaceuticals. Prempro is a combination estrogen-progestin
treatment and Premarin is an estrogen-only treatment. There are
approximately 6 million women taking Prempro and 11 million women taking

Although Premarin, the first HRT drug, was ¡ntroduced ¡n 1942 for the purpose

a study in the Journal of American Medical Association ("JAMA") has linked ¡t to

Prernpro, on the other hand, has been linked to various serious side effects

The most ¡mportant thing stressed by researchers and medical professionals
alike is that Prempro, Premarin and other HRT drugs should be taken in the
smallest dosages (that produce effective résults) for the shortest duration as

menopause might want to forego HRT therapy altogether.Finally, even women
seeking relief from more severe symptoms should do their own benefit/risk
analysis by asking questions, reading available reports, and keeping in mind

Some women who are skeptical of the pharmaceutical solutions to menopausal
symptoms have sought altérnate treatments such as herbal remedies. But
some herbal hormonal treatments may also ¡ncrease the risk of breast and

ORGANIZACIÓN FÍSICA DEL TEXTO
EL PÁRRAFO
En general, la información contenida en un texto se encuentra organizada en parra
división física facilita la lectura. El párrafo físico se extiende desde un punto y aparte a
comprende los conceptos expresados por dos o más oraciones. El párrafo tiene un sent¡a
mismo y a su vez tiene un sentido más amplio que está dado por su relación con la total::
texto.
Ejemplo:
Párrafo 1 Tensión headaches are the most common type ofprimary headache; as many as 90% of adults have
had or will have tensión headaches. Tensión headaches are more common among women than men.
Párrafo 2 Migraine headaches are the second most common type ofprimary headache. An estimated 28 million
people ¡n the United States (about 12% of the population) will experience migraine headaches.
Migraine headaches affect children as well as adults. Be/ore puberty, boys and girls are affected
equally by migraine headaches, but after puberty, more women than men are affected. An estimated

LA ORACIÓN
La oración es la menor unidad de comunicación con sentido completo, autonomía sintáctica y
entonación propia. En un texto escrito, una oración se reconoce porque comienza con mayúscula
y termina con un punto o con signos de entonación, como son los signos de interrogación,
exclamación o los puntos suspensivos.
Ejemplo:

Cohesión en un texto
La cohesión c i un texto está dada por las relaciones léxicas, gramaticales y lógicas que existen
dentro d~ 61. Estas relaciones le confieren su significado o sentido al texto.

• Cohesión Léxica
Este tipo de cohesión establece relaciones mediante la reiteración, que es la reproducción de un
concepto, la repetición, que es la reiteración de un mismo término, o la sinonimia, que es la
introducción de otra palabra con significado semejante a! término ya mencionado. Observe el
ejemplo abajo en el cual las relaciones entre términos o frases están identificadas con un mismo
número.
Ejemplo:
"Branches of the blood system and the lytnph system I1! are brought ven) cióse to the gut in the
región of the ileum, parí, of the small intestine. The food material crosses the lining of the small
intestine here and enters these systems I1!.

En este caso las relaciones se establecen por medio de la referencia. Algunos términos
adquieren significado completo sólo cuando se los interpreta en función de otros mencionados
anteriormente o a ser mencionados con posterioridad. La referencia puede ser:
- Léxica: Cuando un vocablo remite a otro u otros.
Ejemplo:

36. : Módulo de Inglés
Enumeración: esta función discursiva brinda un listado de objetos o conceptos que tienen
algo en común.
Descripción: enumera características distintivas que califican a un objeto, concepto, persona,
suceso o proceso. Puede tratarse de una descripción física (aspecto), de proceso
(funcionamiento) o de función (utilidad o uso).
Relato de hechos del pasado: Narración y descripción de sucesos pasados. Pueden estar
ordenados cronológicamente. También pueden tener carácter anecdótico o describir una
experiencia pasada.
CATEGORÍAS GRAMATICALES BÁSICAS
Sustantivo: término que designa a una persona, un lugar, una cosa o actividad, una cualidad o
¡dea. El sustantivo puede funcionar como el sujeto u objeto de un verbo o como el objeto de una
preposición.
Ejemplo:
" Risk factors1 include obesíty2 (fat ce lis use up ¡nsulirf), decreases ¡n exercise3 (sugar1 ¡s used by the
muse les) and heredity (adu/t-onset diabetes often runs ¡n families)."

Adjetivo: palabra que describe o califica a un sustantivo o a un pronombre. En inglés, los
adjetivos calificativos no varían ni en género ni en número. Dentro de una frase sustantiva
podemos encontrar sustantivos funcionando como adjetivos.
Ejemplo:
"Thrombotic1 stroke that occurs when a blooc? clot (thrombus) forms ¡n one of
the arteries that supply blood to your brain."

Artículo: término que precede al sustantivo para indicar si éste se refiere a algo en particular o a
algo en general. Cuando se refiere a algo en particular se utiliza el artículo definido "the" (el, la,
los, las), que puede usarse tanto con sustantivos singulares como con plurales. Cuando se refiere
a algo en general se utiliza el artículo indefinido "a / an" (un, una). El artículo indefinido introduce
solamente sustantivos en singular. Se utiliza "a" cuando el sustantivo comienza con un sonido
consonante, y "an" cuando comienza con un sonido vocal.
Ejemplos:

Pronombre: término que reemplaza a un sustantivo o frase sustantiva en la oración. Existen
varios tipos de pronombres que varían según la parte de la oración a la que reemplazan y la
función sintáctica que cumplen. Los Pronombres Personales reemplazan al sujeto de la oración,
los Pronombres Objetivos reemplazan al objeto de un verbo o preposición, los Pronombres
Posesivos reemplazan al dueño o poseedor de algo y los Pronombres Relativos y Demostrativos
hacen referencia a una parte determinada del texto mencionada con anterioridad.
Ejemplo:
"The blood sugar levéis are usually kept in check with oral medications, exercise, and diet, but
left unchecked, they could lead to complications."

Preposición: palabra que se usa antes de un sustantivo, frase sustantiva o pronombre y que
determina la conexión que tienen éstos con otras palabras. Las preposiciones pueden indicar
tiempo, lugar, movimiento, etc.

Verbo: palabra o grupo de palabras que se utiliza para describir una acción, experiencia o estado.
Los verbos se dividen en regulares e irregulares. Los verbos regulares son aquellos que forman su
pasado simple y su pasado participio agregando -ed, -d, o cambiando "y" por "i" en la
terminación -ied. Los verbos irregulares son todos aquellos que de alguna manera transgreden
