- 29. El sangrado uterino anormal es un trastorno muy común y forma el 70% de la consulta de ginecología. Debemos utilizar la clasificación de la FIGO para unificar criterios y tener una estimación real de la prevalecía de las patologías que originan el SUA. En base a la edad de la paciente, sus antecedentes, hallazgos clínicos y de imagen se deberá orientar la etiología del sangrado para en primer lugar descartar malignidad y posteriormente elegir el manejo adecuado acorde con sus deseos reproductivos. La meta del manejo médico del SUA es identificar y tratar la causa del sangrado (sistémica o anatómica), prevenir sus complicaciones y recurrencia para mejorar la calidad de vida de la mujer.

- Abnormal uterine bleeding (AUB) is a common clinical problem, affecting up to 14% of women during their reproductive years and impairing their quality of life by creating significant physical, emotional, sexual, social, and financial burdens El sangrado uterino anormal es la causa más común de perdida hemática en la mujer en edad reproductiva. Se estima que alrededor de 10 millones de mujeres sufren de hemorragia uterina y de ellas 6 millones buscan atención medica al año. Las mujeres con hemorragia pueden padecer anemia crónica, dolor pélvico e incapacidad, enfrentando de tal manera un problema médico debilitante que afecta de manera adversa su vida diaria.
- En mujeres con eje hipotálamo-hipófisis-ovario sin alteraciones y con sangrado uterino anormal la pérdida de la regulación ocurre en la capacidad hemostática y vasoconstrictora del endometrio, con un aumento en la producción de prostaglandinas (PG) que promueve la vasodilatación, así como un aumento en los receptores de PGE2 y PGI2 (inhibidor de agregación plaquetaria). Esta elevación desproporción elevada en la producción de PG interfiere con el control en la cantidad de sangrado menstrual en mujeres con ciclos ovulatorios normales.
