RESUMEN
Resumen del Proyecto
Introducción: La estimulación cerebral profunda (ECP) consiste en la aplicación de
estímulos eléctricos de alta frecuencia a un objetivo específico de las estructuras
subcorticales. El presente estudio surge de la necesidad de obtener un método de
imagen por resonancia magnética (IRM) de alta resolución y definición que permita la
adecuada visualización de los núcleos cerebrales en la planeación prequirúrgica, ya
que el éxito del procedimiento depende de la precisión en la ubicación de los
electrodos de neuroestimulación y la eficiencia con el tiempo requerido para su
colocación. Objetivo: Evaluar la precisión de las imágenes obtenidas en Resonancia
Magnética de 3 Tesla, mediante la secuencia T2 3D Brain View de Phillips en
pacientes candidatos a neuroestimulación profunda. Metodología: Se realizará un
estudio prospectivo observacional en 40 pacientes candidatos a neuroestimulación
profunda en un período de 2 años. A los pacientes se les realizará secuencias
volumétricas T1 y T2 3D Brain view, en un equipo Philips Ingenia de 3 Teslas, a los
cuales se les coloca un marco estereotáctico LeksellR
de Elekta, compatible con
imágenes de resonancia, el mismo con el cual se han realizado adquisiciones en 42
pacientes previos.
Resultados esperados: Con las imágenes adquiridas se realizará la planeación
quirúrgica con una gran precisión en la ubicación de las estructuras objetivo (targets).
Conclusión: Se demostrará que las secuencias T1 y T2 3D Brain view permiten
realizar una planeación quirúrgica mucho más acertiva, disminuyendo los tiempos de
intervención y los efectos adversos en los pacientes sometidos a un procedimiento de
neuroestimulación profunda.
Criterio de inclusión de sujetos.
● Pacientes llevados a estimulación cerebral profunda secundario a alteraciones
del movimiento o enfermedad psiquiátrica (que cumplan los criterios del staff
evaluador), previa firma del consentimiento informado para la intervención.
Criterio de exclusión de sujetos.
● Mujer Embarazada o en periodo de lactancia.
● Contraindicaciones para exámenes de resonancia magnética (Por ejemplo,
incapacidad de contener la respiración, arritmias severas, muy bajo desempeño
cardíaco, claustrofobia severa, desfibriladores, o cualquier dispositivo metálico
que no sea aprobado por resonancia magnética).
● Electrodos de estimulación cerebral profunda implantados.

1. DESCRIPCIÓN DEL EQUIPO DE INVESTIGACIÓN
Para el proyecto de investigación será utilizado un equipo de resonancia magnética
Philips Ingenia 3T , ubicado en Centros Especializados de San Vicente Fundación de
Rionegro, Antioquia, operado por el Instituto de Alta Tecnología Médica (IATM).
2. OBJECTIVOS
Objetivo General:
Evaluar la calidad diagnóstica y precisión de las imágenes obtenidas en Resonancia
Magnética de 3 Teslas, mediante la secuencia T2 3D Brain View de Phillips en pacientes
candidatos a neuroestimulación profunda.
Objetivos específicos:
- Determinar la precisión en la localización de los diferentes targets de los pacientes
llevados a estimulación cerebral profunda, por medio del registro de los tiempos de
planeación preoperatorios, tiempos operatorios totales y eventos adversos tempranos.
- Determinar los tiempos de adquisición de las diferentes secuencias.
- Seguimiento de los pacientes llevados a ECP secundario a trastornos del movimiento o
enfermedades psiquiatricas para determinar:
● Percepción de satisfacción por el paciente y acompañantes.
● Patrón de consumo de medicamentos.
● Comparación con escalas de Hoehn y Yahr, UPDRS y Schwab y England.
- Análisis de eventos adversos.
3. DESCRIPCIÓN DEL PROYECTO
La estimulación cerebral profunda es una opción de manejo en diferentes patologías,
entre las que se encuentran el temblor idiopático, enfermedad de Parkinson, trastorno
obsesivo compulsivo (TOC), depresión mayor, síndrome de Gilles de la Tourette entre
otros;; siendo éstas enfermedades de difícil control y/o refractarias a los manejos
habituales ( 1-5).
El procedimiento consiste en la implantación de electrodos de estimulación en los núcleos
basales del cerebro de manera guiada. Dependiendo de la naturaleza de los síntomas y
signos, los electrodos están dirigidos a diferentes núcleos del cerebro. Por ejemplo, los
núcleos subtalámicos son un objetivo común para el tratamiento de las fluctuaciones
motoras, discinesias, rigidez, temblor y lentitud;; características de los pacientes con
enfermedad de Parkinson avanzada. Para el temblor esencial, el núcleo subtalámico
(NST) y el núcleo ventral intermedio del tálamo son usados como objetivos.

La ventriculografía, tomografía computarizada y las imágenes por resonancia han sido
utilizadas como guías en neurocirugía funcional. El método convencional consiste en la
fusión de imágenes secuenciales de tomografía estereotáxica con imágenes de
resonancia magnética sin marco, utilizando un software especializado que da un enfoque
imaginológico multimodal permitiendo de manera subsecuente la localización de puntos
guía como la comisura anterior y posterior, punto intercomisural, núcleos rojos etc, que
servirán posteriormente para alcanzar el target final (6).
Desde hace más de una década se ha venido estudiando la diferencia entre el método
convencional de tomografía para la planeación de neurocirugía funcional con métodos
que usan imágenes de resonancia magnética con marco, encontrando una diferencia
estadística importante a favor de la IMR con el mínimo de eventos adversos (7).
Para llevar a cabo la estimulación cerebral profunda con resonancia es necesario colocar
un marco estereotáctico al paciente con el que serán obtenidas posteriormente imagenes
de diferentes secuencias. Por ende el marco de estereotaxia debe ser compatible con los
diferentes equipos y software utilizados para la planeación quirúrgica y la localización de
los targets.
El marco estereotáctico LeksellR
de Elekta (ver anexo 1) con el cual se desarrolla la
presente investigación, está diseñado en titanio, con una variedad de tornillos en titanio y
aluminio, todos compatibles con equipos de tomografía y resonancia (8). El marco ha sido
utilizado en aproximadamente 40 pacientes en el equipo de resonancia Philips 3Teslas
entre el año 2013 y el 2014 en Centros Especializados de San Vicente Fundación sin
ningún evento adverso relacionado a la utilización de éste con el equipo de resonancia.
El marco es específico para ser usado con el software SurgiplanR
de Elekta, que permite
la fusión de imagenes de tomografía y resonancia, la localización de la línea
intercomisural y la obtención de coordenadas para la consecución posterior de los
objetivos (targets).
El procedimiento también es asistido por la integración de un atlas estereotáctico de los
núcleos basales que hacen más preciso el procedimiento permitiendo la disminución de
los eventos adversos.
Luego de la realización de los orificios de trépano en el craneo, se insertan los electrodos
de registro que sutilmente avanzan y permiten el microregistro de los sitios donde se
encuentran implantados, dando así evidencia objetiva de la actividad neuronal en los
diferentes targets. Luego se insertan los electrodos de estimulación profunda, los cuales
se encuentran unidos por una extensión a un generador de impulsos colocado en un
bolsillo subcutáneo subclavicular.
A pesar de la cuidadosa planificación, se han reportado en la literatura complicaciones
significativas relacionadas con la cirugía tales como la hemorragia intracerebral, trombosis

venosa cerebral, convulsiones, fístula de LCR, confusión perioperatoria y colocación
errónea de electrodos, etc. Se busca así que la planeación quirúrgica lleve a determinar
mejores trayectorias que eviten sitios neurálgicos como venas, arterias, áreas elocuentes
cerebrales entre otros (9).
El éxito de la estimulación cerebral profunda consiste no sólo en seleccionar
adecuadamente a los pacientes que serán llevados a cirugía, sino que se debe asegurar
que los electrodos queden adecuadamente posicionados. Es allí donde las técnicas de
resonancia permiten localizar los targets de una forma adecuada y efectiva.
El NST puede ser delimitado actualmente con las IRM, aunque no siempre con la
precisión que el caso requiere. El globo pálido interno (GPI) es más visible y facil de
localizar. El tálamo se localiza gracias a las coordenadas arrojadas y relacionadas por el
punto intercomisural. Por ende la importancia de la precisión de las ayudas
imagenológicas, sin las cuales la tarea de precisar los targets estereotácticos sería
dispendiosa (10).
Se han descrito varios estudios comparativos entre las secuencias de resonancia
magnética para realizar la planeación quirúrgica incluyendo imágenes en T2, FLAIR, SWI
y DTI con número de muestras pequeñas, incluso algunos de ellos en población de
voluntarios (11-14).
El protocolo clínico de Centros Especializados de San Vicente Fundación realiza
exploración anatómica con secuencias potenciadas en T1 y T2 para la visualización del
cerebro y los vasos sanguíneos principales. El neurocirujano utiliza el software
SurgiplanR
que permite determinar las trayectorias más seguras.
Además, la planeación por imágenes es importante porque, según los recursos utilizados,
se reporta en la literatura que se puede invertir un total de 10 a 45 minutos por target a
localizar (15). Se pretende así evaluar los tiempos de adquisición de las imagenes,
tiempos operatorios, resultados finales y eventos adversos.
El presente estudio nace de la necesidad de obtener un método de imagen por
resonancia magnética (IRM) en equipos de 3T que permita, sin utilizar imágenes de TAC,
una adecuada visualización de los núcleos cerebrales en la planeación prequirúrgica,
debido a que el éxito de la intervención depende en gran medida a la precisión dónde se
ubican los electrodos de neuroestimulación. Hasta la fecha en el hospital de Centros
Especializados se ha realizado cirugía a 42 pacientes con resultados exitosos y
complicaciones menores comparadas con otras series de estudios similares. Este es un
estudio prospectivo observacional en el que se pretende captar aproximadamente 40
pacientes en un período de 2 años.
A los pacientes se les realizará secuencias volumétricas T1 y T2 3D Brain view, en un
equipo Philips Ingenia de 3 Teslas con marco estereotáxico compatible con resonador, el

mismo con el cual se ha realizado planeación en los 42 pacientes.
Consideramos que la secuencia 3DT2 Brain View puede convertirse en una aplicación de
gran beneficio para obtener mejores resultados en los pacientes con una menor tasa de
complicaciones y mejor precisión para alcanzar los diferentes targets a estimular
disminuyendo el tiempo quirúrgico del procedimiento.

5. DISEÑO DEL ESTUDIO
5.1 General
Se realizarán secuencias T1 y T2 3D Brain View en pacientes candidatos a
neuroestimulación profunda, previa colocación de un marco estereotáxico marca LeksellR
de Elekta compatible con equipos de resonancia, empleando la antena 3T TR Quadrature
head Coil.
Las imágenes serán copiadas en un cd para la visualización y análisis en el software
Leksell SurgiPlanR
de Elekta en el cual se realizará la determinación de trayectorias, se
evidenciaran los diferentes targets y las coordenadas a aplicar en el marco de
estereotaxia.
Se realizarán registros del tiempo empleado en la planeación en el software SurgiPlanR
de
Elekta así como la duración de la cirugía para la ubicación de los electrodos y se tomará
registro de los procesos de recuperación del paciente así como de las complicaciones que
puedan surgir.
Se evaluará de manera secundaria la efectividad de la intervención por medio de la
percepción de satisfacción por el paciente y acompañantes, patrón de consumo de
medicamentos previo y luego de la intervención, comparación con escalas de Hoehn y
Yahr, UPDRS y Schwab y England, deterioro cognitivo, afasias e hipomanía. .
5.2 Sujetos
5.2.1 CRITERIOS DE INCLUSIÓN
● Individuos de ambos sexos que cumplan con los requisitos para la estimulación

cerebral profunda.
● Individuos con edades entre los 18 y 70 años de cualquier grupo étnico.
● Voluntad de someterse a exámenes de rutina de resonancia.
● Voluntad y capacidad de seguir las indicaciones y completar todos los
procedimientos del estudio especificados en el protocolo.
● Mujeres en edad fértil solamente si manifiesta verbalmente no estar en embarazo.
5.2.2 CRITERIOS DE EXCLUSIÓN
● Mujer Embarazada o en periodo de lactancia.
● Contraindicaciones para exámenes de resonancia magnética (por ejemplo,
incapacidad de contener la respiración, arritmias severas, muy bajo desempeño
cardíaco, claustrofobia severa, desfibriladores, o cualquier dispositivo metálico que
no sea aprobado por resonancia magnética).
● Electrodos de estimulación profunda cerebral implantados.
5.2.3 INSCRIPCIÓN
Los sujetos se consideran incluidos en el estudio después de haber firmado el
consentimiento informado y ningún procedimiento podrá empezar antes de firmar dicho
consentimiento. Los sujetos tienen que ser capaces y estar dispuestos a entender y firmar
el consentimiento informado.
Es claro que se debe cumplir con los criterios de inclusión establecidos mundialmente
para la realización de la ECP. Para ello se cuenta con la participación de un staff de
neurocirugía funcional compuesto por trabajo social, psicología, medicina física y
rehabilitación, neuropsiquiatría, neurología clínica y neurocirugía funcional, quienes
determinarán la pertinencia de la intervención quirúrgica.
- Enfermedad de parkinson: diagnóstico preciso de enfermedad de Parkinson,
expectativa de vida mayor a 5 años, no contraindicaciones para la cirugía de
estimulación cerebral profunda, presencia de síntomas que limiten la calidad de
vida según los cuestionarios PDQ39 y el Unified Parkinson’s Disease Rating Scale
(UPDRS) y en los cuales se haya visto respuesta luego de la ECP (fluctuaciones
on/off, disquinesia inducida por L-dopa o temblor resistente al manejo médico).
Mejoría con el test de L-dopa mayor o igual al 30%y ausencia de deterioro
cognitivo en los 6 meses previos a la evaluación.
- Distonía: realización de un diagnostico de distonía por un especialista en el tema,
descartando una causa psicógena desencadenante, no contraindicaciones para la
cirugía de estimulación cerebral profunda, limitación marcada de las actividades de
la vida diaria, la distonía es la principal causa de su discapacidad, ausencia de
posturas anómalas significativas y/o deformidades articulares, manejo previo
adecuado por mínimo 2 meses con L-dopa para excluir distonía con respuesta al
medicamento y finalmente no ser candidato a otro tipo de intervenciones o manejos
farmacológicos.
- Temblor esencial: realización de un diagnostico de temblor esencial por un
especialista en el tema, limitación marcada de las actividades de la vida diaria, no
contraindicaciones para la cirugía de estimulación cerebral profunda, no ser
candidato a otro tipo de intervenciones o manejos farmacológicos. Debe haber

fallado o no ser candidato a manejos con beta bloqueadores, primidona,
topiramato, gabapentin y anticolinergicos (16).
5.2.4 DURACIÓN DEL ESTUDIO PARA CADA SUJETO
Los protocolos de adquisición de las imágenes serán construidos de tal forma que cada
voluntario esté como máximo un tiempo de 45 minutos dentro del equipo de resonancia
magnética Philips Ingenia 3T resonador.
5.2.5 TAMAÑO DE MUESTRA
Se realizará un muestreo por conveniencia de los sujetos que sean candidatos para la
cirugía de estimulación cerebral profunda, de los cuales se elegirán 40 pacientes.
5.2.6 RETIRO SIN PREJUICIO
El sujeto mediante consentimiento informado declara su intención de participar en este
estudio, de manera libre, consciente y voluntaria, entendiendo que tiene derecho de
negarse a participar y de retirarse de la investigación en cualquier momento sin sanción o
pérdida de los beneficios a que tendría derecho. Si el voluntario toma la decisión de
retirarse del estudio sus datos no serán incluidos en el estudio y no será reemplazado por
un nuevo voluntario.
5.2.7 RIESGOS
Los eventos adversos que pueden suceder en RM son mínimos si se siguen las
precauciones de seguridad adecuadas. Complicaciones serias pueden suceder en
personas con marcapasos cardíacos, implantes cocleares, electrodos de
neuroestimulación, esquirlas metálicas en los ojos o ciertos tipos de implantes metálicos,
prótesis o clips quirurgicos. Por está razón se excluirán los pacientes que no cumplan con
los requisitos mínimos de seguridad para un estudio convencional de Resonancia
Magnética.
Algunas personas pueden sentirse incómodas por el nivel de ruido presente durante el
examen, además se puede presentar ansiedad, pánico o claustrofobia mientras están en
el equipo de RM. El estudio siempre será supervisado por un médico radiólogo que pueda
atender las necesidades del paciente y manejar las situaciones que puedan presentarse,
entre ellas estrés, pánico o claustrofobia.
Hay riesgos inherentes a la anestesias y al procedimiento quirúrgico. Entre ellos se
encuentran: Apraxia, discinesia, distonía, alteraciones de la marcha, fluctuaciones
motoras, síndrome de disregulación de dopamina, hemorragias intracerebrales, isquemia
cerebral, infecciones locales o sistémicas, problemas relacionados con la batería del
estimulador, daño de los electrodos, alteraciones psiquiátricas (compras compulsivas,
hipersexualidad, hipomanía, ludopatía, intentos de suicidio), declinación cognitiva,
confusión, demencia, alteraciones de memoria, cambios en la fluencia verbal, anhedonia,
apatía, depresión, cambios del ánimo, afasia, disartria, limitación en las actividades de la
vida diaria, disfagia, hiperhidrosis, alteraciones en la discriminación de emociones,
hipermovilidad, mareo, fatiga, insomnio y trastornos del sueño, episodios ictales,
parestesias, alucinaciones visuales, ganancia de peso anormal y obesidad (17).

5.2.8 BENEFICIOS
El paciente no puede beneficiarse directamente de la participación en este estudio, sin
embargo, su participación puede proveer información valiosa para la comunidad médica.
Los resultados de este estudio pueden ayudar a mejorar las técnicas de resonancia
magnética para futuros pacientes.
5.3 SEGUIMIENTO
El desarrollo de la investigación es responsabilidad del IATM, sin embargo se mantendrá
un contacto regular con Philips para discutir temas relacionados, como por ejemplo:
avance en la investigación, asesoría por parte de Philips, actualización de CSKs, entre
otras.
6. CONSIDERACIONES ESTADÍSTICAS
Para el análisis descriptivo se utilizarán distribuciones absolutas, relativas e indicadores
de resumen como la media aritmética, la desviación estándar, los cuartiles y coeficiente
de variación. Se establecerá el criterio de normalidad de los datos mediante la prueba de
Shapiro Wilk y con base a esta se aplicará la t-Student para la diferencia de medias
independientes o la prueba U de Mann-Whitney. Para establecer la diferencias de tiempos
respecto a varias secuencias se aplicará análisis de varianza (ajustando por ANCOVA) o
la prueba de Kruskal Wallis. Para la correlación entre variables cuantitativas se utilizará
regresión lineal simple mediante el análisis del coeficiente de correlación de Pearson o
Spearman y el coeficiente de determinación. Para el análisis multivariado se aplicará
regresión lineal múltiple para ajuste de variables confusoras y como modelo explicativo.
Un valor p inferior de 0,05 se considerará estadísticamente significativo.
7. MANEJO DE DATOS
La información recolectada durante este estudio se mantendrá confidencial en la medida
de lo permitido por la ley. En los resultados no se publicará información que identifique al
sujeto de estudio por su nombre. Las imágenes de RM en este estudio pueden ser
utilizadas para publicaciones científicas, sin embargo, la identidad del paciente no será
revelada en ninguna situación.
8. RENDICIÓN DE CUENTAS DEL EQUIPO
El IATM mantendrá un registro de la instalación y actualización de las CSKs de
investigación liberadas por Philips en el equipo de resonancia magnética Philips Ingenia
3T.
9. DECLARACIONES DE CONFORMIDAD
Por tratarse de datos obtenidos de la RM y almacenados en un sistema de archivo y
transmisión de imágenes, esta investigación, se clasifica de acuerdo con la declaración
internacional de Helsinki, el informe Belmont, ANSI ISO 14155: Clinical investigation for
human subjects y la resolución colombiana 8430 de 1993 del Ministerio de Salud como
una investigación sin riesgos biológicos, fisiológicos, psicológicos y sociales. Además se

tendrá en cuenta la resolución Colombiana 1995 de 1999 por la cual se establecen
normas para el manejo de la Historia Clínica. Se establecerá el debido proceso frente al
derecho de escuchar y entender el consentimiento informado de las limitaciones y
alcances del estudio a considerar. La base de datos que se obtendrá no tiene en cuenta
registros que permitan individualizar al sujeto de estudio, en ellas se omite el nombre,
apellidos y el documento de identificación. No se declara ningún conflicto de interés
académico, financiero o político. Este proyecto no atenta, ni afectará en forma negativa el
lugar donde se configura la investigación.
10. REPORTE DE EVENTOS ADVERSOS
Para todos los pacientes incluidos en este estudio, el radiólogo notificará al Investigador
Principal cualquier acontecimiento adverso que se produzca durante la exploración por
resonancia magnética.
En caso de presentarse un acontecimiento adverso grave (AAG) durante la exploración
por RM, el radiólogo notificará rápidamente al investigador principal y a su vez al promotor
local (contacto de farmacovigilancia local). Este último realizará todo el procedimiento
estandarizado en el IATM para estos casos. Toda la información frente a un evento
adverso será reportada en la historia clínica del sujeto.

Connection feet permit rapid, simple attachment to other components of the Leksell

The design of the Insulated Fixation Posts, with their sterile, single-use screw inserts,
insulates the fixation screw from the post. The inserts minimize the occurance of radio
frequency generated thermal effects in MR scanners.
The risk of persistent cross-contamination is reduced when Insulated Fixation Posts are
used together with Disposable Fixation Screws since the insert provides a single-use
interface between the patient and the frame.
An additional benefit of the plastic inserts is the smooth thread interface to the fixation
screw. This increases the lifetime of the screws and metal posts and makes for a
smoother procedure when attaching the stereotactic frame.
The rounded slimmed top allows for more space in tight MRI head coils and increases the
available treatable volume for Gamma Knife® surgery.
The posts are fixed at the internal corners of the frame by means of a 5 x 19 mm screw
passing through a slot in the center of the post and secured with a specially-shaped
locking piece. This arrangement allows you to adjust the height of the frame in relation to
the patient's head.

The x-coordinate is set on both axes before mounting, which assures fast and correct
positioning when fitting the arc to the frame.
Leksell® Multi Purpose Stereotactic Arc with counter scale is designed to allow the
surgeon to select any desired trajectory to the predetermined target, regardless of the
patient's position, when using Leksell Coordinate Frame G.
Leksell® Multi Purpose Stereotactic Arc with counter scale can easily be mounted onto the

frame in either A-P or lateral orientation. Lateral orientation is most commonly used, but
the A-P orientation is especially useful for the implantation of depth electrodes in lateral
targets.
The unique option to attach several sliding carriers on the arc in combination with the
center-of-arc principle allows the surgeon to simultaneously reach and/or observe any
selected target through two or more trajectories.

treatment, i.e. the mechanical stress on the frame is the same during surgery as during

used to attach the frame to the operation table headrest. The clamp provides an

Needle straightness is checked by rotating the instrument and comparing with engraved

Clear see-through drape for fast, safe & sterile draping of Leksell Stereotactic System®.
Equipped with an adhesive for fixation to the top of the patient's skull, two additional slits
for the arc supports as well as a drain pouch and instrument pouches.
Elekta Stereotactic Drape is ideal for quick and easy sterile draping of Leksell Stereotactic
System®
when performing open stereotactic procedures. Drape size is 213 x 243 cm (84 x
96 inches).
The tailor-made design ensures a large and accessable sterile field. Elekta Stereotactic
Drape is equipped with an 18x16.5cm (7x6.5 inches) adhesive for fixation to the top of the
patient's skull, thus allowing both flexibility of burr hole placement and bilateral burr holes.
Two additional slits for the arc supports have adhesive strips for securing the drape and
