Este documento discute la combinación de fibrosis pulmonar y enfisema (CFPE), un síndrome recientemente definido donde coexisten enfisema en los lóbulos superiores y fibrosis en los lóbulos inferiores en un mismo individuo. Los pacientes con CFPE presentan una función pulmonar aparentemente normal pero una grave alteración en la difusión de monóxido de carbono y una hipoxemia que empeora con el esfuerzo. La tomografía computarizada de alta resolución es la herramienta clave para el diagnóstico.

pulmonary parenchyma (or both) that results in airflow obstruction. Manifestations of COPD range
from dyspnea, poor exercise tolerance, chronic cough with or without sputum production, and wheezing to
respiratory failure or cor pulmonale. Exacerbations of symptoms and concomitant chronic diseases may
contribute to the severity of COPD in individual patients. A diagnosis of COPD is confirmed when a
patient who has symptoms of COPD is found to have airflow obstruction (generally defined as a
postbronchodilator FEV1–FVC ratio less than 0.70, but taking into account that age-associated
decreases in FEV1–FVC ratio may lead to overdiagnosis in elderly persons) in the absence of an
alternative explanation for the symptoms (for example, left ventricular failure or deconditioning) or the
airflow obstruction (for example, asthma). Clinicians should be careful to avoid attributing symptoms to

Definición
La combinación de fibrosis pulmonar y enfisema (CFPE) es un síndrome definido
recientemente, en el cual coexisten en un mismo individuo enfisema en lóbulos
superiores y fibrosis en lóbulos inferiores. Estos pacientes presentan un perfil funcional
respiratorio característico, con volúmenes pulmonares dinámicos y estáticos
aparentemente normales o mínimamente alterados que contrastan con una grave
alteración de la difusión del monóxido de carbono (DLCO) e hipoxemia arterial, la cual
empeora durante el esfuerzo. La prevalencia de hipertensión pulmonar es elevada y
representa la principal condición que determina el pronóstico.
La tomografía axial computarizada de alta resolución (TCAR) constituye la herramienta
primordial para confirmar su diagnóstico. Se ha postulado al humo del tabaco como el
principal agente etiológico.

Daño tabáquico ya no sería solamente sinónimo de EPOC.
Un porcentaje no despreciable de fumadores tendrían “lesiones
intersticiales” que a su vez causarían un porcentaje no despreciable de
patrones restrictivos.
Las lesiones intersticiales contribuirían a la subestimación EPOC.
El concepto de susceptibilidad se modificaría.
Se comprendería mejor la inconsistencia del FEV 1 y la consistencia del
BODE.
El valor predictivo de salud de la ↓ FVC no se tendría que vincular
exclusivamente a S. Metabólico.

Se reforzaría la necesidad de una nosología más precisa
El diagnóstico de las enfermedades respiratorias, sobretodo EPOC,
requeriría regularizar la exploración funcional más allá de la espirometría.
La exposición a tabaco no debe verse como toxicidad a una única
substancia, hay demasiadas variables: 6.000 compuestos, intensidad,
características inhalación, filtro / no filtro, temperatura de combustión,
incluso marca [los aditivos se respetan como secreto = FDA no obliga a
desvelar].
Seguramente se tendría que añadir / definir un fenotipo “fibrótico”
Probablemente el “fenotipo” clásico “Blue Bloater”, en parte era fibrótico.

1) La combinación de fibrosis pulmonar y
enfisema es más frecuente de lo que
pensamos y relativamente fácil de
identificar.
2) Es secundaria a tabaquismo, pero puede
observarse en enfermedades del
colágeno.
