Este documento resume la enfermedad arterial de los miembros inferiores, incluyendo su anatomía, factores de riesgo, presentación clínica, diagnóstico, y tratamiento no farmacológico, farmacológico y quirúrgico. Afecta al 7% de la población mexicana general y al 39% de los adultos mayores. El diagnóstico incluye la historia clínica, examen físico y el índice tobillo-brazo. El tratamiento involucra modificación de estilo de vida, medicamentos anti
9. DEFINICIÓN
• Enfermedad arterial oclusiva que condiciona
un insuficiente flujo sanguíneo a las
extremidades produciendo isquemia
• Limitado a enfermedad arteriosclerótica.
11. EN MÉXICO
• Afecta el 7% de la población general.
• Incluye al 39% de los adultos mayores.
• Factores de riesgo: tabaquismo y diabetes.
12. Sólo 1 de cada
10 de estos pacientes
presenta los síntomas típicos
de claudicación intermitente

13. POBLACIÓN EN RIESGO
• ≥ 65 años
• 50 – 64 años con factores de riesgo para
aterosclerosis (diabetes, tabaquismo, hiperlipidemia,
hipertensión) ó historia familiar de EAP
• < 50 años con diabetes tipo 1 y un factor de riesgo
adicional
• Historia personal de enfermedad aterosclerótica

21. RECOMENDACIÓN
• Historia clínica orientada
• Examen vascular
• Medición de presión arterial de ambos brazos
• Realizar ITB con o sin presiones segmentarias

22. HISTORIA CLÍNICA
AHA/ACC Guideline on the Management of Patients With Lower Extremity Peripheral Artery Disease. 2016
• Claudicación intermitente
• Dolor en las extremidades inferiores no
relacionados con la articulación
• Deterioro en la capacidad de caminar
• Dolor isquémico en reposo
• Heridas en extremidades inferiores que no cicatrizan
23. CLASIFICACIÓN FONTAINE
• Estadio Clínica
• I Asintomático (detectable por ITB)
• IIa Claudicación ligera intermitente no limitante (>
150 mts)
• IIb Claudicación moderada-severa limitante (< 150 mts)
• III Dolor isquémico en reposo Ulceración o gangrena
