18. Clasificación - Fisiopatología
1. SÍNCOPE REFLEJO (mediado neuralmente)
– Grupo HETEROGENEO de condiciones
– Reflejos CV normalmente útiles en controlar la PA, se
vuelven inapropiados en respuesta a desencadenante
– Resultado vasodilatación y bradicardia bajo gasto
cardíaco, disminución RVP hipoperfusión cerebral
20. Síncope Reflejo EFERENTE
TIPO CLASIFICACIÓN
Tipo 1 Mixto Hipotensión precede bradicardia
Bradicardia no <40’
(<40’ por <10 s)
Tipo 2 A Cardioinhibitorio FC < 40 lpm por > 10 s
La PA disminuye antes que la FC
Tipo 2B Cardioinhibitorio Asistolia > 3 seg.
PAS < 80 mmHg al momento o después de la
disminución de la FC
Tipo 3 Vasodepresor puro •La FC no cae >10% de máx al momento del síncope
•La caída de la PA precipita el síncope.
21. Clasificación - Fisiopatología
2. OH y otros síndromes de intolerancia ortostática
– La respuesta eferente simpática se encuentra
crónicamente dañada por lo que la
vasoconstricción es deficiente.
– Intolerancia ortostática hace referencia a signos y
síntomas durante la posición de pie, el síncope es
uno de ellos.
22. Clasificación - Fisiopatología
SINCOPE DEBIDO A HIPOTENSION ORTOSTATICA
• PRIMARIA (enfermedades Neurológicas, Parkinson)
• SECUNDARIA (DM, Amiloidosis, Uremia, SCI)
• INDUCIDA POR DROGAS (OH, VD, diuréticos)
• DEPLECIÓN DE VOLUMEN
23. Clasificación - Fisiopatología
3. SÍNCOPE CARDIOVASCULAR
– Arritmias
• Sind. Del nodo sinusal enfermo
• Bloqueo A-V (mobitz II, completo)
• Torsade Pointes
• Síndrome del QT prolongado
– Enfermedad estructural

28. evaluación inicial
OBJETIVOS DEL DIAGNOSTICO
• Distinguir sincope verdadero vs imitadores
• Determinar presencia de cardiopatia estructural
• Establecer la causa con la suficiente certeza para:
– Evaluar pronóstico
– Iniciar tratamiento
29. evaluación inicial
T-LOC x 3 Preguntas iniciales
• 1. Es un sincope?
• 2. Etiología es precisable?
• 3. Evento de alto riesgo?
30. T-LOC x 3 Preguntas iniciales
• 1. Es un sincope?
• 2. Etiología? 1. LOC completa?
2. LOC transitoria, rápida y
• 3. Evento de alto riesgo? breve?
3. Recupera espontáneo y
completamente?
4. Perdió tono postural?
31. T-LOC x 3 Preguntas iniciales
23-50% se logra identificar etiología
inicialmente
• 1. Es un sincope? CIRCUNSTANCIAS
Previas: posición, actividad,
• 2. Etiología? factores predisponentes
Onset: palpitaciones, snvg
• 3. Evento de alto riesgo? durante (testigos): tipo de caida,
duración, movimientos
final: confusión, snvg,
palpitaciones
antecedentes: historia scd,
medicamentos, am
CARDIACA vs no CARDIACA
(MORT)
33. T-LOC x 3 Preguntas iniciales
• 1. Es un sincope? BASADA EN:
Edad
• 2. Etiología? Comorbilidades
Antecedentes
• 3. Evento de alto riesgo? Sistemas de puntaje
EKG basal

Edad, Comorbilidades
• 2. Etiología? Antecedentes
Sistemas de puntaje
• 3. Evento de alto riesgo? EKG basal
PREDECIR
1. Cardiopatía estructural severa,
CAD (ICC, baja FEVI, IAM
previo)
2. Sospecha de S-Arritmico: EKG
o Clínica. Bloqueos BIF, IV
QRS>120, BS<50 inadecuada,
PE, LQT, Brugada, DAVD
3. Comorb importantes (medio
interno, anemia)
35. evaluación inicial
Evaluación inicial (FASE 1)
• Viejo truco de Historia clínica y examen físico
• PART con challenge ortostático (Active Standing)
• EKG
y OTROS
CSM > 40a
ECO TT si cardiopatía conocida o sincope debido a causas CV
EKG ambulatorio si sospecha de arritmia
Challenge Ortostático (sospecha de mecanismo reflejo)
37. Carotid Sinus Massage (CSM)
• METODO 1 • CONTRAINDICACIONES 2
– Masaje x 5-10 s – Carotid bruit, known significant
carotid arterial disease, previous CVA,
– No oclusivo MI last 3 months
– Decubito y de pie • COMPLICACIONES
• OUTCOME – Ng <0.2%3 (1/5.000 TIA)
– Asistolia 3 s y/o – Transitorias
– Caida PAS >50 mmHg +
reproducción de clínica
= Carotid Sinus Syndrome

CSS en población de pacientes referidos a centro
terciario por síncope de etiología no precisada

39. CSS distinto a CSH
• Hipersensibilidad SC
– MSC 5’’ produce >3’’ asistolia o >50mmHg caida de PAS
• CSH condición necesaria, pero no suficiente para
diagnostia CSS.
• CS Syncope
– Requiere: bradicardia, mareos, pre-syncope, syncope y
caisa producto de CSH
